Pericardial disease--anatomy and function.

Imaging of patients with suspected or known pericardial disease remains challenging. Echocardiography is the first-line investigation for pericardial disease but it has specific limitations in terms of its abilities to visualise the pericardium fully and to identify extracardiac pathology. Cardiac cross-sectional imaging by both MRI and CT has developed significantly and now has an important role in the investigation of pericardial disease. This article examines the appearances of both healthy and diseased pericardium using CT and MRI. The typical imaging findings across a wide range of condit…

Hydropneumopericardium presenting as an acute coronary syndrome: a rare complication of paraesophageal hernia.

Hydropneumopericardium is a very rare complication of long-standing paraesophageal hernia, occurring as a result of rupture of the intrathoracic gastric volvulus into the pericardium. A chronic paraesophageal hernia that is complicated by gastric volvulus can develop into such surgical emergencies as acute gastric obstruction, strangulation, perforation, and rupture into adjacent structures. Subsequent hydropneumopericardium constitutes an acute emergency that requires immediate surgical treatment and pericardial drainage. Herein, we discuss what we believe to be the 1st reported case of hydro…

[Leaflet extension technique in a congenital aortic valve regurgitation].

A 17-year-old boy who was diagnosed as congenital aortic valve regurgitation underwent the leaflet extension technique in 3 aortic leaflets using glutaraldehyde-preserved autologous pericardium. Intraoperative transesophageal echocardiography and postoperative cardiac catheter revealed grade I regurgitation and the patient has been doing well for more than 4 months after the surgery. The leaflet extension technique is considered to be an acceptable surgical treatment for aortic valve disease especially in young patients to whom valve replacement or Ross procedure might not be suitable. A caref…

Pyopneumopericardium caused by mediastinal granuloma.

We report the case of a previously healthy 32-year-old man who was seen with flulike symptoms, dyspnea, and chest pain. The diagnosis was pyopneumopericardium, and pericardial tap revealed 1.3 L of purulent material. Computed tomography of the chest demonstrated a calcified mass inferior to the carina. Urgent exploration through a right thoracotomy revealed that the mass was adherent to the esophagus and pericardium. The subcarinal mass was resected. Pathological study demonstrated granulomatous lymph nodes, which were likely due to histoplasmosis. This is among the first reports of granulomat…

Constrictive pericarditis localized to left ventricle presented with left pleural effusion: a case report.

An 18-year old woman presented with progressive dyspnea and cough. Physical examination revealed decreased breath sounds at the left hemithorax and distant heart sounds with no murmurs or rub. Electrocardiogram revealed low voltage. Chest X-ray showed unilateral left-sided pleural effusion with no cardiomegaly. Transthoracic echocardiogram showed thickened pericardium localized throughout the left ventricle impairing the diastolic filling. Doppler waveforms were suggestive of localized constrictive pericarditis. A computerized tomographic scan of the chest confirmed the presence of unilateral …

An uncommon cause of pericardial actinomycosis.

Cardiac actinomycosis is rare; the pericardium is the most frequently involved site, but myocardial, endocardial and valvular involvement have all been documented. Most cases originate from a thoracopulmonary site of actinomycosis and spread directly to the pericardium. Widespread dissemination from extrathoracic organs is uncommon; in fact actinomycosis is prevented by anatomical barriers and hematogenous diffusion is rare. We describe an uncommon case of pericardial actinomycosis due to a draining fistula from the liver to the pericardial space across the diaphragm. The massive dissemination…

A 9 cm robotic thymectomy and pericardial repair case report.

In the modern surgical era, improved technology has allowed for the increasing use of the robotic platform for thymoma resection. Historically, tumors >5 cm were deemed inappropriate for minimally invasive approaches; thoracic surgeons, however, have become adept with performing increasingly complex thymectomies using minimally invasive techniques. Excision of large thymomas using the robotic platform is no longer considered a rare event, however few publications have described the use of minimally invasive surgery for en bloc excision of the pericardium with mesh reconstruction. We present a …

Effusive-constrictive pericarditis in the spectrum of pericardial compressive syndromes.

When pericardial fluid accumulates and exceed the reserve volume of the pericardium or when the pericardium becomes scarred and inelastic, one of three pericardial compressive syndromes may ensue, namely, cardiac tamponade (CT), characterised by the accumulation of pericardial fluid under pressure; constrictive pericarditis (CP), the result of scarring and loss of the normal elasticity of the pericardial sac; and effusive-constrictive pericarditis (ECP), characterised by the concurrence of a tense pericardial effusion and constriction of the heart by the visceral pericardium. Although relative…

Morphologic investigation on Perceval S, a sutureless pericardial valve prosthesis: collagen integrity after collapsing-ballooning and structural valve deterioration at distance.

Perceval S is a self-expandable, stent-mounted bioprosthetic valve (BPV), with glutaraldehyde treated bovine pericardium, processed with homocysteic acid as an anti-calcification treatment. The stent is crimpable but the valve insertion is done surgically via a shorter procedure which does not require sutures. OBJECTIVES: MATERIAL AND METHODS: RESULTS: CONCLUSIONS: Collapsing and ballooning do not alter cusp collagen periodicity. Structural valve deterioration with stenosis, due to dystrophic calcification and fibrous tissue overgrowth, seldom occurred in the mid-term. Glutaraldehyde fixed per…

[Successful repair of left ventricular laceration due to traumatic broken ribs].

A 27-year-old woman was hospitalized by emergency due to multiple blunt trauma by traffic accident. Computed tomography (CT) showed pericardial effusion. Because hematothorax and pericardial effusion appeared alternatively, thoracocentesis and pericardiocentesis were performed. Finally, the diagnosis of cardiac rupture was confirmed, and surgery was performed through median sternotomy. The left ventricular free wall was penetrated by the edge of a broken rib through the hole of the pericardium and the pleura. Broken ribs were considered to have penetrated the pericardium and caused cardiac rup…

Pericardial disease: current diagnosis and management methods.

Diseases of the pericardium can be indolent or have a sudden onset. They may be primary, but more often are secondary to a systemic disease or previous therapy. Pericardial diseases imitate more common cardiac diseases and therefore can be difficult to diagnose. This paper's four case presentations illustrate use of recently developed methods that facilitate diagnosis. Magnetic resonance imaging is useful for the diagnosis of congenital absence of the pericardium. Echocardiography and computed tomography are useful for the diagnosis of pericardial cysts. Diagnosis of cardiac tamponade is aided…

Pericardial Effusion: Overview of Aetiology, Pathophysiology, Diagnosis, and Management.

The pericardium is a membrane that envelopes the heart and the proximal great vessels. In normal physiological conditions, there is approximately 10-50 ml of pericardial fluid. However, accumulation of pericardial fluid can occur due to inflammation or infection of the pericardium and adjacent structures. This may be an asymptomatic and incidental finding following imaging for another disease process, or the patient may present in shock due to cardiac tamponade. It is important to note that it is the rate of accumulation, not absolute size, that primarily determines symptoms. Whilst the clinic…

Histopathological findings of pericarditis in a patient with multisystem inflammatory syndrome in children associated with COVID-19: A case report.

Multisystem inflammatory syndrome in children (MIS-C), which is associated with the novel coronavirus disease 2019 (COVID-19), has been described as an inflammatory complication of exposure to the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). It carries a risk of serious and lethal complications, including cardiogenic shock. Here, we report the pathological findings of the pericardium in a 10-year-old child with MIS-C, who developed pericarditis-induced cardiac tamponade. In the patient's pericardium, the numbers of infiltrating CD68+ macrophages; CD3+ , CD4+ , and CD8+ T cells…
