Fluid volume control during short-term space flight and implications for human performance.

Space flight exerts substantial effects on fluid volume control in humans. Cardiac distension occurs during the first 1-2 days of space flight relative to supine and especially upright 1g conditions. Plasma volume contraction occurs quickly in microgravity, probably as a result of transcapillary fluid filtration into upper-body interstitial spaces. No natriuresis or diuresis has been observed in microgravity, such that diuresis cannot explain microgravity-induced hypovolemia. Reduction of fluid intake occurs irrespective of space motion sickness and leads to hypovolemia. The fourfold elevation…

Review: the controversy over the pathophysiology of ascites formation in cirrhosis.

The pathogenesis of renal sodium retention and ascites formation in cirrhosis is a subject of much controversy. The generally accepted 'peripheral arterial vasodilatation hypothesis' seems to best explain the mechanism of sodium retention and other clinical findings, such as the hyperdynamic circulation of cirrhosis. However, recent data in pre-ascites and in early ascites do not seem to conform to the peripheral arterial vasodilatation hypothesis. Sodium handling abnormalities can be demonstrated in pre-ascitic cirrhosis when patients are challenged with a sodium load, in the absence of syste

[Orthostatic hypotension after space flight].

Cardiovascular changes occur during spaceflight. Because of the loss of hydrostatic pressure there is, during weightlessness, a fluid shift of extra- and intra-vascular volumes to the upper part of the body. This provokes objective and subjective symptoms, especially in the first days of spaceflight. The fluid shift is perceived by baroreceptors as a relative central hypervolaemia and induces neuro-hormonal mechanisms which provoke a real hypovolaemia. Cardiovascular deconditioning exists during spaceflight with the following symptoms when astronauts return to earth: decreased orthostatic tole

[Hemodilution in the treatment of cerebrovascular disorders].

Haemodilution includes intravenous administration of plasma-replacing solutions and/or blood letting for changing the rheological properties of blood, and for improvement of microcirculation. Three methods of haemodilution various plasma substitutes are given: dextran, hydroxyethyl starch (HES), electrolyte solutions (e.g. Ringer's solution, normal saline, 5% glucose, colloidal plasma, protein solutions), plasma, albumins. The controversial results of clinical trials of the method for therapeutic purposes are discussed. In the light of the reports as yet, it seems that the best results are obt

Circulatory arrest pressure as a prognostic indicator of cardiopulmonary resuscitation.

In a porcine model of cardiac arrest, the intravascular pressure measured during circulatory standstill was positively correlated with the success of cardiopulmonary resuscitation. When volume was expanded before cardiac arrest, circulatory arrest pressure increased and the success of resuscitation increased. After volume expansion, the hematocrit was reduced and colloid osmotic pressure was decreased. However, neither hematocrit nor colloid osmotic pressure changes were directly related to survival.

[The use of human albumin in infusion therapy].

A survey on physiologic functions of albumin is given. Furthermore the two main-indications of albumin, the treatment of blood-volume-deficiency, including the use of albumin for the iso-volemic-hemodilution and the substitution in hyproteinemia are discussed. A comparison of albumin and plasmaproteinsolution follows. Finally limiting factors in albumin therapy due to the lack of coagulation factors are mentioned.

Increased renal alpha-epithelial sodium channel (ENAC) protein and increased ENAC activity in normal pregnancy.

Pregnancy-mediated sodium (Na) retention is required to provide an increase in plasma volume for the growing fetus. The mechanisms responsible for this Na retention are not clear. We first used a targeted proteomics approach and found that there were no changes in the protein abundance compared with virgin rats of the β or γ ENaC, type 3 Na(+)/H(+) exchanger (NHE3), bumetanide-sensitive cotransporter (NKCC2), or NaCl cotransporter (NCC) in mid- or late pregnancy. In contrast, we observed marked increases in the abundance of the α-ENaC subunit. The plasma volume increased progressively during p…

Large-volume paracentesis in nonedematous patients with tense ascites: its effect on intravascular volume.

In patients with portal hypertension and tense ascites, large-volume paracentesis improves patient comfort and may improve systemic hemodynamics. However, it has been avoided in nonedematous patients because of concern for complications, including intravascular volume depletion. In this study, 12 nonedematous patients with chronic liver disease, portal hypertension and tense ascites underwent 14 large-volume (5-liter) paracenteses for the relief of discomfort and/or respiratory distress. Plasma volume was measured directly by a dilution method with 125I-labeled human serum albumin prior to and…

[Exact measurement of the volume effect of 6% hydoxyethyl starch 130/0.4 (Voluven) during acute preoperative normovolemic hemodilution].

BACKGROUND: What is the effect of preoperative acute normovolemic hemodilution (ANH) with 6% hydroxyethyl starch (HES) 130/0.4 (Voluven) on blood volume? METHODS: In 10 patients undergoing radical hysterectomy, ANH was performed to a hematocrit of 21% using 6% HES 130/0.4 (Voluven) whereby a replacement of blood with 115% of colloid was planned. Plasma volume (indocyanine green dilution technique) and hematocrit were determined before, 30 and 60 min after ANH. Red cell volume (labelling erythrocytes with fluorescein) was determined before and 30 min after ANH. RESULTS: After removal of 1,431…

Primary proliferative polycythaemia without splenomegaly: a diagnostic problem.

We report three cases of polycythaemia with no evidence of clinical splenomegaly and normal splenic red cell pool on isotope spleen scan. In each case, however, a diagnosis of primary proliferative polycythaemia (PPP) was suggested by in-vitro erythropoietin-independent growth of peripheral blood erythroid colonies. In one of these cases two possible causes of secondary polycythaemia were also identified. The use of investigations such as isotope spleen scanning and erythroid cell culture in helping to establish a diagnosis of PPP is discussed.

A new method for evaluating an increased general capillary permeability in patients.

The difference between total plasma volume determined with a substance which escapes from vascular beds in the presence of an increase of general capillary permeability and that determined with a substance which is confined to blood even in the presence of an increased capillary permeability may reflect the degree of an increase of general capillary permeability. The total plasma volume was determined by simultaneous injections of 131I-HSA and 51Cr tagged red cells. The capillary permeability was evaluated by calculating the difference (deltaTPV) between total plasma volume determined with 131…

Effects of physical therapy on hyaluronan clearance and volume regulating hormones in lower limb lymphedema patients: A pilot study.

A paired non-parametric test (Wilcoxon) was used to assess hormonal and plasma volume changes. Correlation was tested using Spearman's correlation. The main findings of this novel study are that lymphedema patients lost volume and weight after therapy. Hyaluronic acid did not significantly change pre- compared to post-CDT. Aldosteron …
