Reconstruction of the hypopharynx: current trends.

PURPOSE OF REVIEW: Reconstruction of the hypopharynx continues to present a clinical challenge for the reconstructive surgeon. Trends continue to evolve as many viable options exist, depending on individual patient characteristics and institutional preferences. The article reviews the most current literature available on the subject. RECENT FINDINGS: Surgical resections resulting in pharyngoesophageal defects require timely reconstruction to minimize patient morbidity while optimizing quality of life, specifically in regards to speech and swallowing. Contemporary reconstructive options includ

A review of the surgical and medical treatment of Frey syndrome.

Frey syndrome represents a fascinating example of how nerve regeneration can go awry. The syndrome is characterized by profuse facial sweating and flushing that occurs when salivation is stimulated. It can develop following a variety of insults but is most commonly encountered as a complication of parotidectomy. Consequently, it is mainly head and neck surgeons who see and treat this disorder; however, it is important for other clinicians to recognize what these unusual symptoms represent. Diagnosis may be based either on clinical presentation or through objective testing methods. Potential ne

[Advance in urethral reconstruction with tissue engineering techniques].

OBJECTIVE: To sum up the research advances in urethra reconstruction with tissue engineering techniques. METHODS: The recent original articles about urethra reconstruction with engineering techniques were extensively reviewed. RESULTS: At present, human urothelium and smooth muscle cells have been successfully harvested, cultivated, and expanded in vitro in sufficient quantities for reconstruction. Tissue engineering for urethral reconstruction includes matrices alone, the body's natural ability which induces new tissue growth, or the use of matrices with cells. CONCLUSION: The tissue engin

A new method for correcting type I and type II constricted (cup and lop) ears.

Tanzer suggested the term "constricted ear," denoting a spectrum of deformities limited to the superior third of the ear. Tanzer classified the constricted ear into three types. Type I ears have involvement of the helix, which usually is flattened. Type II ears show involvement of both the helix and the scapha. With type III ears, the auricle is rolled into a nearly tubular form that some authors regard as a form of microtia. The authors' new method for correcting the constricted ear varies in accordance with the diverse degree of deformity. The new method was used to correct constricted ears

Separation of craniopagus conjoined twins: an evolution in thought.

Advances in medicine have enabled more craniopagus twins to survive separation, but the ultimate goal, to have both twins emerge from their separation with full neurologic function and a chance at leading independent and productive lives, remains elusive. The authors were contacted to evaluate craniopagus conjoined male infant twins for separation. Once it was determined that the brains were separate, they reviewed the literature and elected to design an open-ended multistaged separation. Four major stages over 9 and a half months led to the successful separation and preservation of neurologic

Otoplasty.

Multiple techniques have been advocated over the years for the appropriate and natural-appearing correction of the prominent ear. There is no single technique that can re-create the complex three-dimensional nature of the otherwise normal human ear. We have to evaluate each individual ear and patient and correct the abnormalities with a technique that each surgeon feels comfortable with and that can repeatedly provide gratifying results. Ultimately, the simplest technique that can obtain the maximum effect should be employed. A concentration on the male patient and specific concerns with these

[General principles of lip surgery].

A successful lip reconstruction gives a shape illusion and re-establishes physiological function. We must be able to respect static and dynamic function and the axiom "use of lip to repair lip defect". Local flaps provide optimal tissue to replace the lip in the surrounding structures, to respect aesthetic subunits and commissural function, to give a neuro-muscular continuity respecting fibers direction and mucosal quality. If extra-labial tissue is necessary, the use of the same principles enables the best compromise in shape and function as possible.

[Anterior urethral duplication in a male child. A case report].

Urethra duplication is a rare anomaly with numerous anatomic variations. These fall into four main groups: epispadiac, hypospadiac, fusiform, and Y-type. We report on one case of sagittal incomplete hypospadiac anterior duplication of the urethra. Surgical repair consisted of incision of the septum between the two urethras and hypospadias repair at the same setting. Follow-up was characterized by the occurrence of some complications.

Facial soft-tissue augmentation with allogeneic human tissue collagen matrix (Dermalogen and Dermaplant).

Soft-tissue augmentation lends another dimension to facial rejuvenation. Human collagen tissue matrix offers the most physiologic solution to dermal volume depletion, structural soft-tissue malposition, and the correction of facial rhytides and various soft-tissue contour defects. Results with the use of Dermalogen have been comparable with other injectable dermal agents with the most natural effects. The absolute longevity or persistence of human tissue collagen matrix has not been established fully and depends on facial region, technique, and total volume administered. The natural fate of mo

[PLASTIC SURGERY OF THE VULVA AND THEIR CIRCUMSTANCES].

Plastic surgery of the vulva for aesthetic reasons is recently gaining popularity in the Western world, as well as in Israel. There are different methods of executing these operations with no meaningful difference in the surgical results and the satisfaction of the patients. There are also more complicated plastic operations, in which the vulva is involved. These are performed in cases of pseudohermaphroditism, various states of intersex, sex change operations and developmental defects of the genitor-urinary systems. These are considered successful procedures. Furthermore, the issue of the ill

Smoking as an Independent Risk Factor for Postoperative Complications in Plastic Surgical Procedures: A Propensity Score-Matched Analysis of 36,454 Patients from the NSQIP Database from 2005 to 2014.

BACKGROUND: Smoking has been associated with wound healing complications and overall morbidity in multiple specialties, including plastic surgery. From 2005 to 2014, smoking prevalence among U.S. adults decreased from 20.9 percent to 16.8 percent. This study aims to investigate whether smoking prevalence among plastic surgery patients paralleled the national trend and whether smoking was an independent risk factor for postoperative complications. METHODS: The 2005 to 2014 American College of Surgeons National Surgical Quality Improvement Program database was used to examine smoking prevalence

Surgical Wound Complications in Postbariatric Patients Undergoing Body Contouring Plastic Surgeries: The Challenges of Healing.

BACKGROUND: Healing is the final stage of surgical trauma, and its effective management is essential. However, this complex process is influenced by multiple factors, making its control challenging and susceptible to changes, especially in at-risk populations. In this context, postbariatric patients are more prone to postoperative complications, particularly when they undergo body-contouring plastic surgeries. Therefore, the aim of this study was to evaluate the prevalence of surgical wound complications and the associated factors in postbariatric patients undergoing body-contouring plastic su
