BACKGROUND: Although the timing of continuous positive airway pressure (CPAP) resumption after endoscopic transsphenoidal surgery (ETSS) remains controversial in the absence of definitive evidence, recent literature suggests early CPAP resumption may be safer than historically assumed in carefully selected patients. This review aims to (1) summarize the existing evidence available in this field, (2) identify which factors modulate the risk of pneumocephalus when resuming CPAP early after ETSS, and (3) outline the key perioperative measures known to reduce the need for CPAP by minimizing the ri…

PATIENTS: Individuals with documented CCP on computed tomography (CT).
INTERVENTION(S): CT scans of the head, temporal bone, face, neck, and cervical spine. Cessation of habitual Valsalva maneuver (VM) and insertion of pressure-equalization (PE) tubes.
MAIN OUTCOME MEASURE: The prevalence of CCP on CT examinations performed during two decades in a large academic healthcare system. Documentation of symptomatic and imaging impr

Skull base tumours represent a special challenge to surgeons due to the complex anatomy of the area. While small tumours are easy to remove, large lesions can pose complex situations. The most difficult aspects are not only the approach and removal, but specially the repair of the defects created by the resection of the tumour. We present here our experience with the surgical removal of tumours on the anterior skull base. To achieve a good approach, we resort to a bifrontal craniotomy including the cilliar arches. To obtain a skull base bone flap that can be used for repair at the end of the p

We examined a patient with an orbital stab injury with fatal intracranial extension in whom the initial emergency examination did not reveal the extent of damage. Ophthalmologic consultation showed an afferent pupillary defect (Marcus-Gunn pupil), which prompted radiologic studies. Orbital tomograms and computerized tomography demonstrated an optic canal fracture and penetration to the contralateral right ventricle. Diabetes insipidus and thermal irregularity developed, and death occurred seven days later.

Cerebrospinal fluid (CSF) leakage from the dura mater during craniotomy is a common complication, which is associated with infection, meningitis, pneumocephalus, and delayed wound healing. In the present study, we developed an absorbable fish gelatin-based anti-inflammatory sealant for dura mater sealing to prevent CSF leakage. Gelatin derived from Alaska pollock (ApGltn) was modified with α-linolenic acid (ALA), an omega-3 fatty acid that exhibits anti-inflammatory properties, and cross-linked with a poly(ethylene glycol)-based cross-linker to develop ALA-ApGltn sealant (ALA-Seal). ALA-Seal d…
