Efficacy of pneumococcal polysaccharide vaccine in immunocompetent adults: a meta-analysis of randomized trials.

The use of pneumococcal polysaccharide vaccine (PPV) is low in some countries, maybe because of doubts regarding its efficacy. This meta-analysis aims at combining evidence from randomized trials of PPV assessing its efficacy in preventing Streptococcus pneumoniae related diseases in immunocompetent adults. In the fourteen trials totalling 48,837 patients retrieved, PPV prevents definite pneumococcal pneumonia by 71%, presumptive pneumococcal pneumonia by 40%, and mortality due to pneumonia by 32%, but not all-cause pneumonia or death. No preventive effect was seen in the subgroup of patients

Influenza and pneumococcal vaccination among Native American elders in a primary care practice.

BACKGROUND: More than 2 million Native Americans (ie, Native Americans and Native Alaskans) live in the United States; 60% reside in cities. This population, especially its elders, is especially susceptible to respiratory diseases; yet, adherence to guidelines for influenza and pneumococcal immunizations is unknown. OBJECTIVES: To evaluate how frequently older and high-risk adults received vaccinations for influenza and pneumococcal infection and to identify patient characteristics associated with adherence to published recommendations. METHODS: Retrospective medical record review of 550 Nat

Assessing costs and cost effectiveness of pneumococcal disease and vaccination within Kaiser Permanente.

OBJECTIVE: To review studies of the costs of pneumococcal disease and the cost effectiveness of pneumococcal conjugate vaccination conducted in association with the Kaiser Permanente Pneumococcal conjugate Efficacy Trial. RESULTS: for each birth cohort of 3.8 million infants, routine pneumococcal conjugate vaccination program for healthy infants would prevent more than 12000 (78% of potential) meningitis and bacteremia cases, 53000 (69% of potential) pneumonia cases, and 1 million (8% of potential) otitis media episodes. Before accounting for vaccine costs, the vaccination program would reduc

The role of pneumococcal and haemophilus type B vaccination in the prevention of acute otitis media.

Acute otitis media is an extremely common disease, most children being subject to it once or more during their first 5 years of life. Thirty … n the 23-valent vaccines began to appear. Since 1992, studies have been carried out using different pneumococcal vaccines conjugated to diphtheria, tetanus or meningococcal proteins. Despite the incomplete nature of the data, it can already be confirmed that these vaccines are immunogenic from the very first months of life onwards. Numerous problems nevertheless remain …

Pneumococcal vaccine in the prevention of community-acquired pneumonia: a skeptical view of cost-effectiveness.

Streptococcus pneumoniae (pneumococcus) is an important pathogen that causes pneumonia, bacteremia, and meningitis in adults. In recent years, pneumococcal isolates resistant to penicillin have become increasingly prevalent in the U.S. For these reasons, wide use of the pneumococcal vaccine has been advocated. Pneumococcal vaccines have been proven to be effective in preventing the invasive complications of S pneumoniae infection. The efficacy of pneumococcal vaccines in preventing non-bacteremic pneumonia in high-risk subjects, however, has not been proven. The current pneumococcal vaccine is…

[Failure of anti-pneumococcal vaccination and prophylactic antibiotic therapy in 2 splenectomized subjects].

Asplenic patients are at high risk for the development of fulminant pneumococcal bacteremia. The mortality rate in those infected patients remains high. Therefore pneumococcal polysaccharide vaccine has been recommended, but despite penicillin prophylaxis pneumococcal sepsis can occur in vaccinated patients. We have observed two cases (one fatal) of overwhelming postsplenectomy pneumococcal sepsis in vaccinated patients who received long-term antibiotic prophylaxis. Both exhibited a poor antibody response to polyvalent vaccine.

Decline of serum antibody in splenectomized children after vaccination with pneumococcal capsular polysaccharides.

Asplenic persons are at increased risk of overwhelming sepsis caused by Streptococcus pneumoniae. Vaccination with polyvalent pneumococcal polysaccharide has been shown to stimulate a nearly normal antibody response in these individuals, indicating that active vaccination might prevent pneumococcal disease in this population. To obtain information on the duration of protective levels of pneumococcal antibody, 33 asplenic children were vaccinated and antibody levels were measured at intervals for up to 4 1/2 years after vaccination. Significant antibody decline was observed in children who had

The effect of polyvalent pneumococcal polysaccharide vaccine on nasopharyngeal and nasal carriage of Streptococcus pneumoniae.

To investigate the possible effect of a polyvalent pneumococcal vaccine on the nasopharyngeal and nasal carriage of Streptococcus pneumoniae, we studied 313 children under 8 years of age who had received either 14-valent pneumococcal capsular polysaccharide vaccine (Pn) or Haemophilus influenzae type b capsular polysaccharide vaccine (Hib) after having recovered from acute otitis media. Nasopharyngeal and nasal swabs were obtained during a symptomless phase, 6 to 12 months after the vaccination, and cultured by routine bacteriological methods. All S. pneumoniae strains were serotyped and among

Hospital-based vaccination of older adult inpatients: comparison of two delivery models for influenza, COVID-19 and pneumococcal vaccines.

BACKGROUND: Vaccination coverage for influenza, SARS-CoV-2 and pneumococcal disease among older Australians remains suboptimal. Hospital admission is an underutilised opportunity for the vaccination of older adults. METHODS: We conducted a 12-week prospective, single-centre evaluation at a tertiary hospital in Australia (August–November 2024). Inpatients aged ≥ 65 years admitted under cardiology, geriatrics, geriatric rehabilitation, respiratory and infectious diseases were screened for eligibility for National Immunisation Program (NIP)-recommended vaccines (influenza, SARS-CoV-2, conjugate p

A multicenter study on the epidemiology of complicated parapneumonic effusion in the era of currently available pneumococcal conjugate vaccines.

BACKGROUND: Parapneumonic effusion (PPE) is a common complication of pneumonia. Streptococcus pneumoniae is the most common cause of bacterial pneumonia. A reduction in pneumonia hospitalizations has been observed since the introduction of the 7-valent pneumococcal conjugate vaccine (PCV7). Despite this apparent benefit, an increase in the incidence of PPE was recorded in some countries following PCV7 implementation. As the 13-valent pneumococcal conjugate vaccine (PCV13) was expected to provide a wider protection against PPE, the aim of the present study was to evaluate the impact of PCV13 in

Long-term Impact of Pneumococcal Conjugate Vaccines on Invasive Disease and Pneumonia Hospitalizations in Indigenous and Non-Indigenous Australians.

BACKGROUND: Universal pneumococcal conjugate vaccine (PCV) programs began in Indigenous Australian children in 2001 and all children in 2005, changing to 13-valent PCV (PCV13) in 2011. We used laboratory data for invasive pneumococcal disease (IPD) and coded hospitalizations for noninvasive pneumococcal community-acquired pneumonia (PnCAP) to evaluate long-term impact. METHODS: Annual incidence (per 100 000 population) was calculated for age-specific total IPD, PCV13 non-7-valent PCV (PCV7) serotypes, and PnCAP by Indigenous status. Incidence in the pre-universal PCV7 (2002-2004), early PCV7

All-cause pneumonia in children after the introduction of pneumococcal vaccines in the United Kingdom: A population-based study.

PURPOSE: To explore the impact of pneumococcal conjugate vaccines (PCVs) in preventing childhood pneumonia in the United Kingdom. METHODS: We carried out a population-based study to assess the trend of all-cause pneumonia in children aged under 10 years between 2002 and 2012. Data were obtained from the IMS Disease Analyser, a primary care database in the United Kingdom. Three time periods were defined to estimate monthly incidence: pre-PCV7 (January 2002 to August 2006), post-PCV7 (September 2006 to March 2010), and post-PCV13 (April 2010 to December 2012). Interrupted time series analysis (

Preventing bacterial disease in the HIV-infected of sub-Saharan Africa: the role of cotrimoxazole and the pneumococcal vaccines.

Bacterial infection is a major cause of morbidity and mortality among patients with HIV living in Africa. Broadening the scope of cotrimoxazole (CTX) prophylaxis to cover patients whose CD4 counts are above 200 cells/mm(3) has been suggested as a means of improving the control of infectious disease on the continent. CTX has demonstrated antimalarial benefit in Central and West Africa, but in areas of high bacterial resistance to CTX, the prophylactic role of CTX as an antibacterial agent is less clear. In particular there is little to suggest that prophylactic CTX provides reliable control aga

Protective efficacy of PspA (pneumococcal surface protein A)-based DNA vaccines: contribution of both humoral and cellular immune responses.

Streptococcus pneumoniae is a major public health problem and new strategies for the development of cost-effective alternative vaccines are important. The use of protein antigens such as PspA (pneumococcal surface protein A) is a promising approach to increase coverage at reduced costs. We have previously described the induction of a strong antibody response by a DNA vaccine expressing a C-terminal fragment of PspA. Fusion of this fragment with the cytoplasmic variant of SV40 large T-antigen (CT-Ag) caused reduction in specific interferon-gamma produced by stimulated spleen cells. In this work

Indirect effects of 13-valent pneumococcal conjugate vaccine on pneumococcal carriage in children hospitalised with acute respiratory infection despite heterogeneous vaccine coverage: an observational study in Lao People's Democratic Republic.

INTRODUCTION: Empiric data on indirect (herd) effects of pneumococcal conjugate vaccines (PCVs) in settings with low or heterogeneous PCV coverage are limited. The indirect effects of PCV, which benefits both vaccinated and non-vaccinated individuals, are mediated by reductions in vaccine-type (VT) carriage (a prerequisite for disease). The aim of this study among hospitalised children in Lao People's Democratic Republic (Lao PDR) is to determine the effectiveness of a 13-valent PCV (PCV13) against VT pneumococcal nasopharyngeal carriage (direct effects) and the association between village-lev

The relative immunogenicity of 20-valent and 15-valent pneumococcal conjugate vaccines: A systematic review and network meta-analysis.

Higher-valent pneumococcal conjugate vaccines (PCV) may reduce adult pneumococcal disease, but no head-to-head trials compare PCV20 vs. PCV15 in adults. We synthesized serotype-specific immunogenicity from randomized controlled trials using a network meta-analysis via the common comparator PCV13. PubMed, Scopus, Cochrane Library, and ClinicalTrials.gov were searched for trials enrolling adults (≥18 y) and evaluating PCV20 or PCV15. The primary endpoint was opsonophagocytic activity (OPA) geometric mean titers (GMTs) at 30 d, summarized as ratios of means on the log scale. Heterogeneity/inconsi
