Moderate intra-abdominal hypertension is associated with an increased lactate-pyruvate ratio in the rectus abdominis muscle tissue: a pilot study during laparoscopic surgery.

BACKGROUND: The development of intra-abdominal hypertension [IAH] in critically ill patients admitted to the ICU is an independent predictor of mortality. In an attempt to find an early, clinically relevant metabolic signal of modest IAH, we investigated abdominal wall metabolite concentrations in a small group of patients undergoing laparoscopic surgery. We hypothesized that elevated intra-abdominal pressure [IAP] due to pneumoperitoneum leads to an increased lactate/pyruvate [L/P] ratio in the rectus abdominis muscle [RAM], indicating anaerobic metabolism. METHOD: Six patients scheduled for…

Hand-assisted laparoscopic splenectomy for idiopathic thrombocytopenic purpura during pregnancy.

A successful case of a hand-assisted laparoscopic splenectomy with low-pressure pneumoperitoneum for autoimmune thrombocytopenic purpura in a patient at 23 weeks' gestation is reported. Preoperative splenic arterial embolization was performed on the same day as the operation using painless contour embolic material and super-absorbent polymer microspheres. The abdominal wall retraction method first was applied to avoid the effects of pneumoperitoneum on systemic hemodynamic alterations. However, a sufficient surgical view could not be obtained, as the intra-abdominal organs were elevated becaus…

When Air Is Not an Emergency: Using Intraperitoneal Sevoflurane to Rule Out Perforation.

Idiopathic pneumoperitoneum (IP) is a rare diagnosis of exclusion typically discovered during imaging for unrelated concerns. Most pneumoperitoneum cases are secondary to visceral perforation, necessitating urgent surgical intervention. However, IP is a distinct, non-surgical condition that can present asymptomatically, complicating clinical decision-making. A 76-year-old female with multiple comorbidities presented to the emergency department after her Holter monitor detected intermittent, third-degree heart block. She was asymptomatic from a gastrointestinal standpoint, with a benign abdomin…

A negative pressure-based visualization technique for abdominal Veress needle insertion.

PURPOSE: Abdominal Veress needle insertion is commonly performed to generate a pneumoperitoneum during laparoscopy. Various safety tests are conducted to confirm accurate needle tip positioning into the abdominal cavity. However, these occasionally yield unclear results and do not help directly visualize the peritoneum puncture. We validated a negative pressure-based technique that helps instantly visualize the moment of the Veress needle entry into the abdominal cavity. METHODS: This study included 761 patients who underwent laparoscopic hernioplasty between 2003 and 2021 that entailed pneum…

Ultrasonographic evaluation of intraoperative optic nerve sheath diameter changes in patients undergoing laparoscopic surgery.

BACKGROUND: Laparoscopic surgery offers several advantages such as reduced postoperative pain and faster recovery; however, the establishment of pneumoperitoneum has the potential to transiently increase intracranial pressure (ICP). Ultrasonographic measurement of the optic nerve sheath diameter (ONSD) provides a reliable noninvasive indicator of ICP changes. This study aimed to evaluate ONSD measurements, which may reflect intraoperative ICP changes during laparoscopic upper abdominal surgery performed in the reverse Trendelenburg position, and to assess the specific effects of pneumoperitone…

Low-Pressure Versus Normal-Pressure Laparoscopic Cholecystectomy and its Effect on Intra-Operative Parameters and Post-Operative Pain: An Observational Study.

BACKGROUND: Pneumoperitoneum, created by insufflating carbon dioxide (CO2), is essential for surgical visualization, but conventional intra-abdominal pressures (12-15 mmHg) can negatively impact cardiac and respiratory functions. To mitigate these effects, low-pressure pneumoperitoneum (7-10 mmHg) has been suggested. The present study aimed to compare the outcomes of laparoscopic cholecystectomy at low intra-abdominal pressure with conventional standard pressure laparoscopic cholecystectomy. METHODOLOGY: A prospective observational study was conducted in a tertiary care hospital in Eastern In…

Randomized controlled trial comparing low pressure (8 mmHg) versus high pressure (14 mmHg) CO2 insufflation on postoperative pain in patients undergoing laparoscopic cholecystectomy: Protocol.

BACKGROUND: Laparoscopic cholecystectomy is the gold standard for treating symptomatic gallstone disease. Pneumoperitoneum is essential for adequate visualization during the procedure and is typically maintained at a standard pressure of 12-14 mmHg. However, high insufflation pressure may contribute to increased postoperative pain. This study aims to assess whether low-pressure pneumoperitoneum (8 mmHg) reduces postoperative pain compared to standard pressure (14 mmHg), without compromising operative safety or efficacy. We hypothesize that low pressure pneumoperitoneum at 8 mmHg reduces postop…

Bladder pressure monitoring and CO2 gas-related adverse events during per-oral endoscopic myotomy.

Per-oral endoscopic myotomy (POEM) is a minimally invasive treatment for esophageal achalasia. However, POEM has the potential risk of inducing carbon dioxide (CO2) gas-related adverse events, such as pneumoperitoneum, pneumomediastinum, and pneumothorax. The aim of this study was to evaluate the usability of bladder pressure monitoring as an index of CO2 gas-related pneumoperitoneum. The monitoring of bladder pressure and lung compliance and the incidence of iatrogenic pneumoperitoneum were retrospectively studied in 20 patients who underwent POEM between June 2013 and March 2015. The bladder…

Arterial to end-tidal carbon dioxide pressure difference during laparoscopic surgery in pregnancy.

BACKGROUND: There is controversy about whether capnography is adequate to monitor pulmonary ventilation to reduce the risk of significant respiratory acidosis in pregnant patients undergoing laparoscopic surgery. In this prospective study, changes in arterial to end-tidal carbon dioxide pressure difference (PaCO2--PetCO2), induced by carbon dioxide pneumoperitoneum, were determined in pregnant patients undergoing laparoscopic cholecystectomy. METHODS: Eight pregnant women underwent general anesthesia at 17-30 weeks of gestation. Carbon dioxide pnueumoperitoneum was initiated after obtaining a…

Effect of PEEP titration on the incidence of elevated driving pressure in laparoscopic bariatric surgery: a randomized superiority trial.

BACKGROUND: Suboptimal positive end-expiratory pressure (PEEP) settings may lead to cyclic lung overstress, especially in obese patients undergoing general anesthesia. We hypothesized that recruitment maneuver (RM) followed by driving pressure-guided PEEP titration - before and during pneumoperitoneum - would reduce the incidence of elevated driving pressure (> 15 cmH₂O) during pneumoperitoneum. METHODS: In this single-center, randomized, superiority trial, obese patients undergoing laparoscopic bariatric surgery were allocated into three groups: (1) recruitment maneuver followed by driving p…

Transfundal insertion of a Veress needle in laparoscopy of obese subjects: a practical alternative.

Because induction of artificial pneumoperitoneum through the infra-umbilical route is associated with complications in laparoscopic procedures, especially in obese patients, we performed a prospective randomized study comparing the conventional infra-umbilical route with a transfundal route, in which the Veress needle is inserted into the peritoneal cavity through the uterine fundus. One hundred obese subjects (body mass index >/=25 kg/m(2)) scheduled for laparoscopic sterilization were randomized into two groups. In the infra-umbilical group pneumoperitoneum was achieved at a ratio (punctures…

MRI measurement of the effects of moderate and deep neuromuscular blockade on the abdominal working space during laparoscopic surgery, a clinical study.

BACKGROUND: Conflicting data exist regarding the effects of deep neuromuscular blockade (NMB) on abdominal dimensions during laparoscopic procedures. We performed a clinical study to establish the influence of moderate and deep neuromuscular blockade (NMB) on the abdominal working space, measured by Magnetic Resonance Imaging (MRI), during laparoscopic donor nephrectomy with standard pressure (12 mmHg) pneumoperitoneum under sevoflurane anaesthesia. METHODS: Ten patients were intraoperatively scanned three times in the lateral decubitus position, with pneumoperitoneum maintained by a mobile i…
