A lumbosacral transitional vertebra in the dog predisposes to cauda equina syndrome.

The association between the occurrence of a lumbosacral transitional vertebra (LTV) and the cauda equina syndrome (CES) in dogs was investigated. In 4000 control dogs without signs of CES, 3.5% had an LTV, while in 92 dogs with CES, 16.3% had an LTV. The lesion causing CES always occurred between the last true lumbar vertebra and the LTV. Dogs with an LTV were eight times more likely to develop CES than dogs without an LTV. German Shepherd dogs were eight times more likely to develop CES compared with other breeds. Male dogs were twice as likely to develop CES than females. Dogs with an LTV de

[Diagnosis and treatment of cauda equina syndrome secondary to lumbar disc herniation].

OBJECTIVE: To discuss the diagnosis and treatment of cauda equina syndrome. METHODS: Thirty-four cases of cauda equina syndrome from 1993 to 2005 were analysed retrospectively. Its clinical representation, pathology character and clinical outcome were observed. RESULTS: At follow-ups of 3 to 102 months after the treatment (averagely 43 months), the muscle strength and function of the constrictor began to resume to different degrees. Muscle strength resumed gradually one week after the operation, resumed faster 3 months after the operation, and resumed completely more one year later. Function

Recovery of long-term denervated human muscles induced by electrical stimulation.

We investigated the restorative potential of intensive electrical stimulation in a patient with long-standing quadriceps denervation. Stimulation started 18 months after injury. After 26 months, the thighs were visibly less wasted. Muscle cross-sectional areas, measured by computerized tomography, increased from 36.0 cm(2) to 57.9 cm(2) (right) and from 36.1 cm(2) to 52.4 cm(2) (left). Knee torque had become sufficient to maintain standing without upper extremity support. Biopsies revealed evidence of both growth and regeneration of myofibers. The results suggest that electrical stimulation ma

[Pure subacute pandysautonomia: an assessment of treatment with intravenous polyvalent immunoglobulins].

INTRODUCTION: Acute or sub-acute pure dysautonomia is uncommon. We report a case of sub-acute pure pandysautonomia with favorable outcome after intravenous immunoglobulin therapy. CASE REPORT: A 29-year-old right-handed student, with an uneventful medical history presented, for one month, bilateral loss of visual acuity and digestive disorders, associating diarrhea, vomiting and anorexia. Physical examination revealed bilateral intrinsec oculomotor nerve palsy, a dryness syndrome and severe orthostatic hypotension. Ophthalmologic examination showed bilateral diffuse parasympathic impairment a

Electromyography of the anal sphincter: which muscle to examine?

Electromyographic examination of the deeper external anal sphincter (EAS) muscle is far more uncomfortable than of the subcutaneous muscle, so we tested the need for its examination. We compared the findings in 85 paired examinations of ipsilateral subcutaneous and deeper EAS muscles in 67 patients with a cauda equina lesion. When the other muscle was normal, the deeper or subcutaneous muscle was pathological in 18% or 13% of pairs, respectively. In addition, the subcutaneous EAS muscle was usually more severely affected than the deeper EAS muscle. Thus, examination of the deeper EAS muscle di

Medical realities of cauda equina syndrome secondary to lumbar disc herniation.

STUDY DESIGN: An analysis of 44 cauda equina syndrome cases. OBJECTIVES: To determine the neurologic outcome of cauda equina syndrome cases, in light of the significant medical implications of this disorder. SUMMARY OF BACKGROUND DATA: Cauda equina syndrome from lumbar disc herniation accounts for up to 1% of all disc herniations. Most of the literature supports surgery within 24 hours as a means of improving the outcome. METHODS: A retrospective chi 2 analysis was performed of 44 patients surgically treated for lumbar disc herniation who initially sought treatment for cauda equina syndrome

Cauda equina syndrome secondary to lumbar disc herniation: a meta-analysis of surgical outcomes.

STUDY DESIGN: A meta-analysis of surgical outcomes of cauda equina syndrome secondary to lumbar disc herniation. OBJECTIVES: To determine the relationship between time to decompression after onset of cauda equina syndrome and clinical outcome, and to identify preoperative variables that were associated with outcomes. SUMMARY OF BACKGROUND DATA: The timing of surgical decompression for cauda equina syndrome is controversial. Although most surgeons recommend emergent decompression, results in certain studies show that delayed surgery may provide a satisfactory outcome. METHODS: A meta-analysi

[Characteristic cerebrospinal fluid findings and clinical aspects of lymphocytic meningoradiculitis].

Clinical data and Cerebrospinal fluid (CSF) findings of 20 patients suffering from a meningoradiculitis were analyzed. Most patients had a tick bite or an erythema migrans before neurological symptoms occurred. All suffered from severe radicular pain. Predominant neurological symptoms were peripheral palsies of the facial nerve followed by motor paresis of the trunk and the extremities of mostly asymmetric distribution. Predominant CSF alterations were a long lasting inflammatory cellular reaction and an impairment of the blood-CSF barrier. In contrast to the CSF of 17 patients suffering from

Neurologic Injury After Sacral Neuromodulation.

OBJECTIVE: Neurologic injury after sacral nerve stimulation (SNS) is rare, but the incidence is unknown. Infection is a potential mechanism for neurologic damage. This report illustrates the presentation, pathophysiology, diagnostic considerations, and treatment of epidural infection causing neurologic deficits after SNS. CASE REPORT: We present a case of a woman with severe fecal incontinence due to Crohn's disease who underwent SNS implantation and subsequently developed a wound infection requiring complete device explantation. A few days later, she presented with leg pain and weakness. Urg

Cauda Equina Syndrome Due To Dural Sac Shift with Engorgement of the Epidural Venous Plexus: Rare Complication After Lumbar Microdiscectomy.

BACKGROUND: Several postoperative complications related to lumbar microdiscectomy have been reported, including cauda equina syndrome. However, to the best of our knowledge, postoperative cauda equina syndrome resulting from dural sac shift with engorgement of the epidural venous plexus is yet to be reported. CASE DESCRIPTION: A 71-year-old male patient was referred to our hospital with a chief complaint of pain and sensory disturbance due to the lumbar disc herniation of L5-S1. Microdiscectomy was performed to treat the lumbar disc herniation, and his sensory disturbance improved. However, f

Does rectal examination have any value in the clinical diagnosis of cauda equina syndrome?

PURPOSE: This study aims to quantify the value of digital rectal examination (DRE) in the clinical diagnosis of cauda equina syndrome. METHODS: A retrospective case note review was performed on all patients referred to a University Teaching Hospital over a one-year period with documented suspicion of cauda equina syndrome. All Patients underwent MRI scanning to either confirm or rule out the diagnosis. RESULTS: Fifty-seven such patients were identified, 13 (23%) of whom had confirmation of cauda equina syndrome on MRI scanning. The DRE did not significantly discriminate for the outcome of MR

[P75 neurotrophin receptor mRNA sequential expression and significance after Cauda equina compression in rats].

OBJECTIVE: To observe the mRNA expression of p75NTR (p75 neurotrophin receptor) and the amount of neuronal cells apoptosis in lumbar-sacral spinal cord at different time points after the acute cauda equina compression in rats and to explore their correlation. METHODS: Sixty adult female Sprague Dawley(SD) rats were randomly divided into the normal control group and the compression groups. The acute cauda equine compression model was established as placing a silicon gel rubber at L(3,4) level of the vertebral canal which represented about 70% to 80% compression to the cross section. The whole

Anterior dural ectasia mimicking a lytic lesion in the posterior vertebral body in ankylosing spondylitis.

Anterior dural ectasia is an extremely rare finding in ankylosing spondylitis (AS). The authors describe a unique case of AS in which the patient presented with cauda equina syndrome as well as an unusual imaging finding of erosion of the posterior aspect of the L-1 (predominantly) and L-2 vertebral bodies due to anterior dural ectasia. Symptomatic patients with long-standing AS should be monitored for the presence of dural ectasia, which can be anterior in location, as is demonstrated in the present case.

Delayed neurological insufficiency caused by transverse sacral fracture after minor trauma in elderly patients.

Sacral fractures in elderly patients with associated lumbosacral lesions can be overlooked easily because of vague symptoms and delayed neurological insufficiency. A 70-year-old female and a 73-year-old female presented with delayed neurological insufficiency caused by transverse sacral fracture after minor trauma. Both patients had suffered from lower extremity symptoms with dysuresia and dyschezia for more than a month. Coexisting lumbosacral pathological lesions may confuse the correct diagnosis for sacral fractures. Decompressive sacral laminectomy was performed, and the patients showed re

Synovial sarcoma of the cauda equina.

Primary synovial sarcoma originating from the cauda equina is extremely rare. Only one case, involving an 11-year-old girl, has been reported. The authors describe the case of a 23-year-old woman with a primary synovial sarcoma of the cauda equina. The patient visited a local hospital and described a 2-month history of low-back pain. She was referred to the authors' hospital for further evaluation. On physical examination, she had a straight-leg raising result of 70° bilaterally. Motor examination revealed Grade 4/5 strength in the bilateral extensor hallux longus muscles. There was normal sen

U-shaped sacral fracture: an easily missed fracture with high morbidity. A report of two cases.

U-shaped sacral fracture is a very rare injury. This injury is easily missed and the diagnosis is often delayed as it is difficult to detect on the anteroposterior view of the pelvic radiograph. It is highly unstable and neurological injury is common. Two cases of U-shaped sacral fractures are reported here in which the diagnosis was delayed resulting in the late development of cauda equina syndrome. In these two cases, full recoveries were achieved following surgical decompression. A high index of suspicion with proper clinical and radiographic assessments will decrease the incidence of misse
