[Obstructive sleep apnea-hypopnea syndrome in children: Clinical diagnosis].

The French Society of Research and Sleep Medicine (SFRMS) organized a meeting on obstructive sleep apnea syndrome (OSAS) in children. A mult … sis and the severity of the disease, and therefore for classifying which children will benefit form polysomnography and for proposing an adapted follow-up after OSAS therapy. Even if they are not able to ascertain the diagnosis of OSAS in children, clinical history, questionnaires, and physical examination are very important. Finally, we propose a classification …

Questionnaire and nocturnal oxymetry in children with adenotonsillar hypertrophy.

OBJECTIVES: Draw up an evaluative approach to the diagnostic contribution of nocturnal oximetry associated with a parental questionnaire in … a negative questionnaire can predict that oximetry will not be useful and if necessary advise for a polysomnography for an exclusion diagnosis. In contrast, a positive questionnaire followed by a positive oximetry argues in favor of the polysomnography not being useful.

Determining the site of airway collapse in obstructive sleep apnea with airway pressure monitoring.

Twenty patients with obstructive sleep apnea (OSA) underwent complete polysomnography and simultaneous upper airway pressure monitoring with a custom-made, soft silicone-covered catheter measuring 2.3 mm in diameter. The catheter had four solid-state microtip pressure sensors positioned in the posterior nasopharynx, immediately caudal to the tip of the uvula, at the level of the hyoid bone, and in the midesophagus. The level(s) of airway collapse was determined by changes in the pressure patterns between transducers. In 14 of the 20 patients, airway collapse was confined or initiated at the or…

[Role of nocturnal oximetry in screening for sleep apnea syndrome in pulmonary medicine. Study of 329 patients].

Nocturnal oximetry can show nocturnal oxygen desaturation. This examination was proposed as an investigation for the early detection of the sleep apnoea syndrome (SAS). We have compared the results of nocturnal oximetry and polysomnography in 329 consecutive patients seen in the department of thoracic medicine for the early detection of the SAS between June 1990 and June 1995. The diagnosis of SAS was confirmed at the time of polysomnography using an hypopnoea/apnoea index (IAH) greater or equal to 15 per hour. Two parameters of oximetry were well correlated with IAH less than 15 per hour: if …

Sleep health is a multidimensional component of cardiovascular health, and modulating sleep as a cardiometabolic risk factor requires accura … or and electroencephalography-based device evaluations, with emphasis on device performance against polysomnography. Consistent with a narrative review, sources were selected through domain expertise rather than systematic screening. Wearable devices measure peripheral proxies to determine if the wearer is awake or asleep. The simplest method, actigraphy, uses mo …

Impact of obstructive sleep apnea on cardiometabolic health in a random sample of older adults in rural South Africa: building the case for the treatment of sleep disorders in underresourced settings.

STUDY OBJECTIVES: The association between obstructive sleep apnea (OSA) and increased cardiometabolic risk (CMR) has been well documented in higher-income countries. However, OSA and its association with CMR have not yet been investigated, based on objective measures, in southern Africa. We measured polysomnography-derived sleep characteristics, OSA prevalence, and its association with cardiometabolic diseases in a rural, low-income, African-ancestry sample of older adult participants in South Africa. METHODS: Seventy-five participants completed the study. Body mass index, hypertension, diabe…

Diagnostic Criteria, Differential Diagnosis, and Treatment of Minor Motor Activity and Less Well-Known Movement Disorders of Sleep.

PURPOSE OF REVIEW: Sleep-related movement disorders (SRMD) include several different motor activities during sleep. Few of them are well known and well classified, whereas others are minor motor disorders of sleep which are neither thoroughly characterized and classified nor have been extensively investigated to clarify their pathogenesis and clinical relevance. This review will focus on those minor sleep-related movement disorders. RECENT FINDINGS: Before diagnosing periodic limb movement (PLM) disorder in patients with PLM during polysomnography, other disorders associated with PLM need to …

Sleep-disordered breathing in spinal cord-injured patients: a short-term longitudinal study.

UNLABELLED: Spinal cord-injured subjects were assessed during the acute admission for sleep-disordered breathing. Polysomnography demonstrated a high incidence of sleep apnoea that persisted during the acute phase. There was no correlation with respiratory function tests or measures of ventilatory control. Screening of this population is worthwhile although the clinical significance is uncertain. BACKGROUND AND OBJECTIVE: Previous studies have demonstrated an increased incidence of sleep apnoea in spinal cord-injured patients. Many of these studies were performed in long-term, stable spinal c…

Methodological Limitations and the Necessity of Objective Biomarkers in Linking Oral Frailty to Subjective Sleep Quality.

Although Lin et al. highlighted the importance of oral frailty and sleep quality as key research areas, their methodologies still rely too heavily on subjective reporting. Incorporating objective indicators such as polysomnography and tongue pressure measurement is essential to verifying these findings.

A Novel Snore Recording Application Demonstrating High Correlation With Two-channel Portable Monitoring for Diagnosing Sleep Apnea Syndrome.

BACKGROUND/AIM: Obstructive sleep apnea syndrome is a common disorder that is diagnosed via polysomnography. However, polysomnography is complex and expensive. Therefore, the aim of this study was to develop a novel snore recording application to evaluate sleep apnea status. PATIENTS AND METHODS: First, we monitored 67 participants using a two-channel portable monitor and a smartphone recording application. Acoustic data and SpO2 or airflow data from 25 participants whose apnea-hypopnea index (AHI) was ≥5 were used to develop an algorithm for evaluating sleep apnea. RESULTS: The coefficient …

The prevalence of obstructive sleep apnea in symptomatic patients with syndromic craniosynostosis.

The reported prevalence of obstructive sleep apnea (OSA) in patients with syndromic craniosynostosis (SCS) varies due to inconsistent definitions of OSA, lack of uniform diagnostic testing, and different mixes of syndromic diagnoses. The purpose of this study was to determine the prevalence of OSA in symptomatic patients with SCS, and to determine whether this differs by phenotypic diagnosis. A retrospective cohort study of children with SCS was conducted. The primary outcome was presence of OSA diagnosed by polysomnography. The prevalence of OSA was calculated and stratified by diagnosis to c…

Is uvulopalatopharyngoplasty still an option for the treatment of obstructive sleep apnea?

Uvulopalatopharyngoplasty (UPPP) has been widely described as a surgical option for the treatment of obstructive sleep apnea syndrome. Several studies have demonstrated variable success rates, and different criteria have been used to define success of surgery. The aim of the present study was to assess the efficiency of UPPP by polysomnography and to correlate it to the clinical findings. Also, we correlated to the various studies dealing with this topic, considering the different criteria adopted in each of this. Sixty-four patients were included in the study. The efficiency of surgery for UP…

Update in obstructive sleep apnea syndrome in children.

The prevalence of OSAS in children is 0.7-3%, with peak incidence in pre-schoolers. It is characterised by partial or complete upper airway … ep and mouth breathing. Nocturnal pulse oximetry, nocturnal noise audio/videotape recording and nap polysomnography are useful tools for screening suspected cases of OSAS in children, and the gold-standard for diagnosis is overnight polysomnography in the sleep laboratory. On the contrary of SAOS adults, children usually present: less arousals associated to apnea …
