[A Case of Recurrent Variceal Hemorrhage Secondary to Portosinusoidal Vascular Disease].

Portal hypertension is a syndrome characterized by an elevation in pressure within the portal venous system, with severe consequences. Hepatic cirrhosis is its most common cause, but there are less frequent causes, including the recently described hepatic vascular disease, portosinusoidal vascular disease (PSVD), which affects portal venules and hepatic sinusoids. We present the case of a 68-year-old man with a history of portal hypertension and recurrent variceal bleeding. Despite medical treatment and endoscopic procedures, the hemorrhages persisted. A transjugular intrahepatic portosystemic

[Injuries to the main vessels and their reconstruction in pancreatoduodenal resection].

The article discusses the results of surgical correction of injuries inflicted to the vessels of the portal system in three patients during pancreatoduodenal resection. Growth of the pancreatic tumor into the main vessels may be revealed only during the operation, which requires vascular plastics. Compression of the portal vein causes marked changes of the central and regional hemodynamics.

Effects of end-to-side portacaval shunt and distal splenorenal shunt on systemic and pulmonary haemodynamics in patients with cirrhosis.

BACKGROUND: Patients with cirrhosis exhibit splanchnic, peripheral and pulmonary vasodilation, which are thought to play a role in increasing portal pressure, promoting sodium retention and determining arterial hypoxaemia. The present study investigated whether these abnormalities are influenced by portal hypertension or by portal systemic shunting. METHODS: Sixty-one patients with cirrhosis who had haemodynamic measurements before and after end-to-side portacaval shunt (n = 30) or distal splenorenal shunt (n = 31) were evaluated. RESULTS: End-to-side portacaval shunts were more effective th…

Fenestration of the maxillary septal bulla using different surgical approaches and endoscopy systems in horses: An ex vivo study.

Sinusitis is common in horses. Sinoscopy is used for visualization of sinus cavities and can be used for diagnostic and therapeutic purposes. In this surgical procedure, a larger chamber is created and communication with other sinus compartments facilitated. To compare the applicability of different surgical approaches and rigid endoscopy systems for sinus inspection and fenestration of the maxillary septal bulla, and to examine the impact of different lens angles on the effectiveness of the procedure. Six chemically preserved cadaveric equine heads were used. Frontal and caudal maxillary port

The authors studied changes of hepatic circulation, hepatoportal hemodynamics, and homeostasis of peripheral and portal venous blood in resp … changes occur after removal of the spleen, which creates real preconditions for the development of portal system thrombosis. All the above-said provides the basis for reducing considerably the range of indications for splenectomy and recommending it only in cases with marked clinical manifestations of hypersplenism hemolysis and increased tendency to bleeding.

The effect of splenic vein diameter on the diagnosis of portal vein thrombosis.

BACKGROUND: It was still controversial that whether the increase of splenic vein (SV) diameter increased the risk of portal vein thrombosis (PVT), which was a severe disease with high mortality, in the clinic. PURPOSES: This study, using computational fluid dynamics method, aimed to investigate how the increase of SV diameter affects the portal vein hemodynamics under different anatomical and geometric features of the portal venous system, thus how it induced to PVT. METHODS: The ideal models of the portal system, including different anatomical structures according to the location of left ga…

Acute Thrombosis of Left Portal Vein during Right Portal Vein Embolization Extended to Segment 4.

Portal vein thrombosis (PVT) is an uncommon, but potentially devastating complication of portal vein embolization (PVE). Its occurrence relates to both local and systemic risk factors. In the setting of PVE, precipitating factors include injury to the vessel wall and reduced portal flow. Contributory factors include portal hypertension, hypercoagulopathy, inflammatory processes, malignancy, pregnancy, oral contraceptive use, and asplenia. The goal of therapy is to prevent thrombus progression and lyse existing clot. Hepatectomy is impossible if adequate recanalization has not occurred and/or o

Caution for living donor liver transplantation with congenital portosystemic shunt: a case report.

BACKGROUND: Congenital portosystemic shunt is an infrequent abnormal connection between the portal vascular system and the systemic circulation. Portosystemic shunts are common findings in patients with cirrhosis, causing gastroesophageal varices, hepatic encephalopathy, and others. However, there is no consensus or literature describing how to manage asymptomatic patients with portosystemic shunts and normal liver. CASE PRESENTATION: The patient was a 39-year-old female who underwent donor right hepatectomy for living donor liver transplantation. The patient was healthy by nature, however, d

Current prognosis in necrotizing enterocolitis with portal vein gas.

OBJECTIVE: To investigate anecdotal reports that portal vein gas in necrotizing enterocolitis is no longer associated with a poor prognosis. PATIENTS AND METHODS: Twelve cases of neonatal necrotizing enterocolitis with portal vein gas from 1988 to 1994 were identified retrospectively from the radiology dictation system in a large university hospital. RESULTS: Two distinct groups of patients with portal vein gas in necrotizing enterocolitis were identified. In 3 of the 12 children, portal vein gas was identified on abdominal films after the diagnosis of necrotizing enterocolitis. These infant

[False image of portal thrombosis in MRI: pitfalls in portal hypertension. Apropos of Budd-Chiari syndrome].

This observation illustrates how difficult it is to interpret the intravascular signal observed with MRI in cases of portal hypertension. A signal occupying the entire lumen of the portal trunk and branches can be seen, which is constantly observed in the axial as well as coronal planes in the 3 series studied. It is hypointense relative to the liver on T1-weighted images, hyperintense on T2-weighted images: this appearance therefore perfectly mimicks portal thrombosis. Ultrasound combined with pulsed Doppler demonstrates the stagnant blood within the patent portal system. This kind of artifac…

A Rare Case of Idiopathic Noncirrhotic Portal Hypertension in a Young Patient.

The most common cause of portal hypertension (PH) is liver cirrhosis. When symptoms of PH develop in noncirrhotic patients secondary to hepatic or systemic disorders, it is termed as noncirrhotic portal hypertension (NCPH) while idiopathic noncirrhotic portal hypertension (INCPH) is the term for PH present without any identifiable underlying cause. INCPH is a diagnosis of exclusion when all other causes of liver cirrhosis have been ruled out. A liver biopsy is required to diagnose INCPH. Presuming that PH is secondary to cirrhosis when no liver biopsy is performed is not always true. The incid

Cyclic GMP in Liver Cirrhosis-Role in Pathophysiology of Portal Hypertension and Therapeutic Implications.

The NO-cGMP signal transduction pathway plays a crucial role in tone regulation in hepatic sinusoids and peripheral blood vessels. In a cirrhotic liver, the key enzymes endothelial NO synthase (eNOS), soluble guanylate cyclase (sGC), and phosphodiesterase-5 (PDE-5) are overexpressed, leading to decreased cyclic guanosine-monophosphate (cGMP). This results in constriction of hepatic sinusoids, contributing about 30% of portal pressure. In contrast, in peripheral arteries, dilation prevails with excess cGMP due to low PDE-5. Both effects eventually lead to circulatory dysfunction in progressed l

The hyperdynamic circulation in cirrhosis: an overview.

The hyperdynamic circulation begins in the portal venous bed as a consequence of portal hypertension due to the increased resistance to flow from altered hepatic vascular morphology of chronic liver disease. Dilatation of the portal vein is associated with increased blood flow, as well as the opening up or formation of veno-venous shunts and splenomegaly. At the same time, portal hypertension leads to subclinical sodium retention resulting in expansion of all body fluid compartments, including the systemic and central blood volumes. This blood volume expansion is associated with vasorelaxation

A new injection portal for brachially inserted central venous catheter. A multicenter study.

Totally implantable portal systems are widely used for long-term central venous access. A new venous portal system inserted via the brachial veins (P.A.S. Port system, Pharmacia Deltec Inc, U.S.A.) was studied in five centres. From January 1988 through May 1989 61 systems were implanted. Fifty-two patients had malignant diseases. Nine cases had non-malignant disorders. The portals were implanted subcutaneously in the fore-arm and catheterization was done percutaneously (46) or by cutdown-technique (15) under local anesthesia. Catheter tip position was controlled by fluoroscopy or x-ray. The ba…

Methods to evaluate portal hypertension.

There are a variety of clinical investigations available to evaluate the development of portal hypertension and its complications. Although portal pressure or hepatic venous pressure gradient measurements are the most appropriate methods, clinical findings and other hemodynamic measurements should also be evaluated in patients with portal hypertension. Noninvasive techniques such as variceal pressure and echo Doppler flowmetry have not yet been entirely validated, and new techniques are being investigated. Finally, experimental hemodynamic studies of different models of portal hypertension giv

Systemic and hepatic hemodynamics in hepatosplenic Manson's schistosomiasis with and without propranolol.

Systemic and hepatic hemodynamics were prospectively studied in 11 patients with Manson's schistosomiasis and portal hypertension, as well as alterations resulting from the use of propranolol. It was decided that patients whose portal pressure was reduced by 30% with the use of the drug would not undergo surgery and that treatment would consist of the chronic use of propranolol, associated with sclerosis of esophageal varices. This objective was not met by any of the patients whose portal pressure was measured and the study was interrupted. Results show that patients with Manson's schistosomia
