[Function tests for the Eustachian tube. Current knowledge].

The Eustachian tube regulates the homeostasis of the middle ear. Problems with its function are predominantly found in childhood. As a consequence, otitis and hearing impairment occur. The most important muscle is the tensor tympani muscle. The complexity of the functional anatomy and physiology are reasons why no function test alone is capable of fulfilling all diagnostic needs. The predictive value of the various Eustachian tube function tests for the outcome of a tympanoplasty is not yet clear. An overview of clinically and scientifically relevant tests for the Eustachian tube function is p

[Additive treatment for central vestibular vertigo].

We have observed in our own practice that numerous patients with primary symptoms of vertigo exhibit cervical segmental muscular imbalance and increased tension in the masticatory musculature. This is frequently associated with functional blockades, especially in the joints of the head and upper cervical spine. Particularly important are special receptors of the small vertebral joints and muscle insertions at the cervicocranial transition. Evidence indicates that there are neuroanatomic structures between these receptors and the central vestibular and cochlear core area of the brain stem, whic

[Antipruritic therapy in atopic eczema].

In patients with atopic eczema, the quality of life is seriously affected by tormenting pruritus. As the pathogenesis of pruritus is not understood, symptomatic measures are the mainstay of treatment along with avoiding aggravating factors. They include emollients, topical and systemic active ingredients and physical therapy (e. g. phototherapy) adjusted to the acuity the disease. Behavioral therapy offers additional support. New treatment modalities, partly still in experimental stage, such as immunomodulators, immunosuppressive agents and opiate antagonists are reserved for nonresponding pat

Promoting emergency contraception.

The use of available oral contraceptive agents for emergency contraception has been judged safe and effective by the U. S. Food and Drug Administration and other agencies. Yet implementation by physicians has been limited, and only a small percentage of women take advantage of this option each year. Lack of a specially packaged--and marketed--product for this indication has been a major barrier.

The nature and content of telephone prescribing habits in a community practice.

Prescribing medicine over the telephone was studied during a two-month period in a two-physician fee-for-service private family practice. Of 1,264 telephone calls, 392 (31.0%) resulted in a medication prescription. Of these calls, 176 (44.9%) were for refills of previously prescribed medicines. The most common drugs for new telephone prescriptions were decongestants/antihistamines/antitussives, antibiotics, and pain medications. Medicines were more likely to be prescribed via telephone for the diagnoses of upper respiratory infections, headache, low-back pain, or bronchitis. A patient was more

Physicians' attitudes and practices in the evaluation and treatment of irritable bowel syndrome.

OBJECTIVE: Irritable bowel syndrome (IBS) is a common disorder characterized by abdominal discomfort and disordered bowel habits. Despite the high prevalence of IBS, little is known about how physicians perceive this condition. The aims of our study were to measure physicians' understanding of IBS, to assess their attitudes towards patients with IBS, and to determine whether there are differences in the way Internal Medicine physicians (IM), Family Practice physicians (FP), and Gastroenterology physicians (GI) evaluate and treat IBS patients. MATERIAL AND METHODS: A survey was sent to 3000 ph

Primary care physicians and their HIV prevention practices.

A national random-sample survey of 4011 primary care physicians was conducted to determine the extent to which they are providing HIV prevention and clinical services, and to learn what characteristics and attitudes might impede the provision of such services. Physicians were asked about their history-taking practices for new adult and adolescent patients, including asking about the use of illicit drugs (injection and noninjection), the number of sexual partners, use of condoms and contraceptives, past episodes of sexually transmitted diseases (STDs), sexual orientation, and sexual contact wit

Physicians as second victims after a malpractice claim: An important issue in need of attention.

OBJECTIVE: Being sued for malpractice is extremely stressful and potentially traumatizing. We aim to identify claims' consequences on the physicians' well-being and medical practice. MATERIAL AND METHODS: We administered a confidential telephonic survey to those physicians with a claim closed during 2014, among those insured by the main professional liability insurance company in the region. The questionnaire addressed several topics: symptoms and well-being changes, needs, impairments and practice changes. We used descriptive statistics as well as Chi-square and T-Student tests. RESULTS: A

[Dutch College of General Practitioners' practice guideline on symptoms of the hand and wrist: a summary].

The first edition of the practice guideline for general practitioners (GPs) on hand and wrist symptoms was published in January 2010 by the Dutch College of General Practitioners. This practice guideline provides GPs with pointers for the diagnosis and treatment of hand and wrist symptoms. Carpal tunnel syndrome (CTS) can be diagnosed on its typical clinical presentation alone. The treatment of mild symptoms of CTS, trigger finger and De Quervain's tenosynovitis is conservative, or sometimes a corticosteroid injection: more severe or persistent symptoms require referral to hospital. [The practice guideline 'Vaginal discharge' (first revision) from the Dutch College of General Practitioners; a response from the perspective of general practice].

The revised practice guideline by the Dutch College of General Practitioners (NHG) on vaginal discharge is intended to offer general practitioners a handle for management of vaginal complaints. Most general practitioners experience uncertainties in daily practice with respect to the diagnosis of vaginal discharge. In most cases history taking and physical examination fails to provide sufficient valuable information to arrive at a definite diagnosis. No further (microscopic) investigation is only permissible in a very obvious infection with Candida albicans. A recent onset of complaints of itch

From theory to practice: physicians' knowledge, attitudes, and practices regarding amyotrophic lateral sclerosis in Egypt - a cross-sectional study.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a progressive neurodegenerative disorder with heterogeneous presentations that often mimic other conditions, leading to frequent misdiagnosis and delayed recognition. Physician awareness and diagnostic confidence are critical for early detection, yet little is known about these factors in the Middle East and North Africa (MENA) region, where ALS services and specialized centers remain limited. METHODS: We conducted a nationwide cross-sectional survey of 1,320 physicians in Egypt. A validated online questionnaire assessed demographics, knowled

[Therapy for infertile couples].

Therapy for infertile couples comprises gender-specific as well as couple-specific aspects. Diagnostic and therapeutic procedures focus on the causes of the infertility such as refertilization or epididymovasostomy on the one hand and assisted fertilization with extracted spermatozoa on the other. Using an interdisciplinary approach including urology, gynecology, reproductive medicine and human genetics, the treating physicians are able to fulfil the desire of infertile couples for a healthy child in many cases.

Competencies and clinical guidelines for managing acne with isotretinoin in general practice: a scoping review.

BACKGROUND: Acne is a common, chronic, and burdensome disease. There is evidence of delayed and inequitable patient access to isotretinoin. Overuse of antibiotics in patients with acne raises antimicrobial stewardship concerns. AIM: To identify clinical practice guideline (CPG) and consensus statement recommendations regarding the clinical competencies required for prescribing oral isotretinoin for acne. DESIGN AND SETTING: This was a scoping review of acne CPGs and consensus statements, globally. METHOD: The Arksey and O'Malley framework informed design in conjunction with Joanna Briggs In

National survey of physicians in Swedish child health centres finds insecurity and unawareness regarding management of undescended testicles.

AIM: To investigate underlying factors for previously reported shortcomings in child health centres' (CHC) referral process of boys with undescended testicles. METHODS: A total of 386 physicians working at Swedish CHCs were surveyed regarding their knowledge about undescended testicles and their clinical management. Multivariate regression analyses were performed to identify risk factors of non-adherence to guidelines and self-reported lack of clinical skills. RESULTS: The overall knowledge of the health benefits of undescended testicle surgery was high (89%), while two-thirds were unaware o
