Opioid prescriptions: are they really necessary after major intra-abdominal surgery? A single-centered prospective study.

BACKGROUND: In an effort to curb opioid use, we instituted a no routine opioids protocol for major colorectal and general surgical procedures requiring inpatient admission. METHODS: This was a prospective cohort study with preintervention (control) and postintervention groups that included abdominal and pelvic surgery requiring inpatient admission. The control groups received opioid prescriptions at discharge in keeping with the surgeon's standard practice whereas the intervention group received counseling on the use of nonopioid analgesics and avoiding opioid use. The goal of the interventio…

Medicaid 'Unwinding:' Much Of The Reduction In Medicaid-Paid Prescriptions Was Offset By Increased Commercial Coverage.

With the expiration of the Medicaid continuous coverage condition on March 31, 2023, states began returning to regular eligibility renewals … od. Leveraging state variation in Medicaid coverage termination start dates, we found that although prescriptions paid for by Medicaid or CHIP fell during unwinding, this decline was mostly offset by an increase in commercial-paid prescriptions. Total prescriptions were unchanged, suggesting that the unwinding did not result in reduced access to medications.

Cross-sectional examination of characteristics of higher-dose buprenorphine prescriptions during the era of illicit fentanyl.

BACKGROUND: In response to greater illicit fentanyl use, buprenorphine daily doses exceeding the FDA's recommended target daily dose (16 mg) and maximum suggested daily dose (24 mg) may provide better outcomes, but little is known about higher dosage prescribing patterns. To better understand buprenorphine prescribing patterns, this manuscript examines the frequency and characteristics of dispensed buprenorphine of ≤ 16mg, > 16-24 mg, and > 24 mg daily dose. METHODS: We used IQVIA data to conduct a cross-sectional study of opioid use disorder-indicated buprenorphine prescriptions dispensed at…

Raising the Jewish nation: prescriptions of modern motherhood in folksgeszunt to Jews in interwar Eastern Europe.

The Society for the Preservation of the Health of the Jewish Population (OZE) was an organisation dedicated to providing medical aid to Eastern European Jews ravaged by war, revolution, poverty and disease during and after World War I. The OZE's top priority was addressing the health needs of Jewish children and teaching mothers how to 'properly' raise their infants, as children were believed to be the backbone and future of the Jewish nation. Analysing the OZE's public-facing newspaper Folksgezunt (People's Health), this paper examines how the OZE used reigning ideas in the Western European a

A retrospective analysis of e-prescriptions for non-communicable diseases on a telehealth platform in Malaysia.

BACKGROUND: The management of non-communicable diseases (NCDs) has benefited from telehealth services. As these services which include teleconsultation services and e-prescriptions are relatively new in Malaysia, the data generated provide an unprecedented opportunity to study medication use patterns for the management of NCDs in the country. We analyze e-prescriptions from a local telehealth service to identify medication use patterns and potential areas to optimize medication use in relation to clinical practice guidelines. METHODS: A cross sectional observational study was conducted by ret…

Systematic analysis of traditional Chinese medicine prescriptions provides new insights into drug combination therapy for pox.

ETHNOPHARMACOLOGICAL RELEVANCE: The decline in cross-protection provided by the smallpox vaccine increases the risk of infection from other poxviruses. While drug combinations are a promising management, they remain underdeveloped for poxviruses. Prior to the development of the smallpox vaccine, China had long relied on herbal medicine to combat pox and accumulated a wealth of knowledge regarding different herb combinations and symptoms related to pox. The information was documented in the form of prescriptions. AIM OF THE STUDY: The extensive data of prescriptions offer the potential for unc…

Improving Clinician Confidence in Insulin Prescriptions at Discharge for Individuals With Newly Diagnosed Type 1 Diabetes.

Background In a questionnaire, we found that pediatric clinicians at Basildon and Thurrock University Hospital (BTUH) have low confidence levels in prescribing multiple daily injections (MDI) for newly diagnosed pediatric patients with type 1 diabetes mellitus. We designed and evaluated locally tailored prescription guidance to improve confidence in MDI discharge prescriptions for pediatric doctors of all grades. Methods We designed a prescription guidance tool by adapting existing local guidelines to improve clinician familiarity with MDI prescriptions and prevent prescription errors. The int…
