Priapism is regarded as an uncommon disease in white populations, although no previous research has been done into its incidence in a certain geographical area. All cases of priapism in Finland during the years 1975-90 are reviewed here. Its incidence varied from 0.34 to 0.52/100,000 males per year, being very stable during the whole period of 16 years, when cases due to intracorporeal injections for treatment of impotence are excluded. UNLABELLED: Penile scintigraphy with [99mTc]pertechnetate/99mTc-RBCs was performed in patients with sickle cell disease patients who had priapism to assess the role of this imaging procedure in directing the clinical management of these patients.

PURPOSE: To evaluate selective embolization for management of posttraumatic priapism in preadolescent boys and color Doppler sonography for diagnosis of the causative lesion and for planning embolization.

Metastases to the penis due to a primary carcinoma of the prostate are rare. In approximately half of the patients, malignant priapism is the main symptom. This study reports on a case of malignant priapism, caused by a direct and metastatic infiltration of the corpora cavernosa by a prostatic adenocarcinoma. Sonography gave hints, the magnetic resonance imaging verified the infiltration and aspiration cytology verified the carcinoma. Hemodynamics, evaluated by Doppler sonography, and intracavernosal blood gas analysis demonstrated a mixed high-low priapism without need of therapy.

Metastases to the penis from renal cell carcinoma producing priapism are very rare, as only 18 cases have been described since 1964. We present an additional case of priapism due to massive metastatic involvement of the corpora cavernosa in a patient with huge renal cell carcinoma, 28 cm in diameter, discussing the possible pathogenetic mechanism of such a rare condition.

A case of priapism complicating essential thrombocythaemia in a 45-year old Nigerian gardener is described. His platelet counts ranged from 1.2-1.6 x 10(12)/L, haematocrit from 0.25-0.33 L/L and leucocytes from 23.4-30.6 x 10(9)/L. There was a splenomegaly of 5cm. He had bilateral cavernostomy but is yet to regain penile erection 3(1)-2 months after surgery.

A 99mTc penile scan was performed in 9 patients with priapism. The technique of the procedure is described. Penile scan is an easily obtainable modality that can differentiate between high- and low-flow forms of priapism. We believe that these scintigraphic findings facilitate management of priapism.

We report a case of priapism in a patient with philadelphia positive chronic granulocytic leukemia. Combined medical and surgical therapy, including chemotherapy with hydroxyurea and multiple punctures of the corpora cavernosa respectively, effected a rapid detumescence and resolution of the priapism and control of the primary disease, although subsequent ability of erection was not fully restored.

In 5 out of 84 patients suffering from impotence a prolonged erection was observed after intracavernous papaverine injection. Treatment consisted of puncture of the cavernous body, blood evacuation and intracavernous adrenaline injection. In 4 patients erectile function could be preserved and in the fifth patient a penile prosthesis has been implanted.

A case of a 64-year-old man with stenosis of the lumbar canal and symptoms of compression of the cauda equina is described. This symptomatology increased after walking and was associated with spontaneous priapism. All symptoms were relieved after surgical decompression.

The authors report a case of priapism following cavernography with secondary fibrosis of the corpora cavernosa demonstrated histologically. They stress the importance of a rigorous cavernography technique which must take account of the respective tissue toxicity of the various contrast media used.

A case report of mixed prostatic carcinoma metastasizing to the penis is presented. A review of the literature revealed only 55 cases of prostatic carcinoma metastasizing to the penis report to date. To the best of our knowledge, no such unusual case has been reported before.

To investigate the mechanism of drug-induced priapism, we gave the antipsychotic agent chlorpromazine and the antidepressant trazodone to 14 dogs by intravenous and intracorporeal injection. Bilateral intracorporeal pressure, blood flow within the internal pudendal artery, and systemic blood pressure were monitored. Venous outflow restriction was evaluated by continuous saline infusion of the corpus cavernosum with the infrarenal aorta clamped.
