Acne: epidemiology, risk factors, pathogenesis, management and cutting-edge research.

Acne vulgaris is a widespread chronic inflammatory skin condition particularly affecting the pilosebaceous unit, with its pathogenesis intricately linked to excessive sebum production, abnormal follicular keratinization, overcolonization by Cutibacterium acnes (C. acnes), and dysregulated host immune responses. Beyond its dermatological manifestations, the disorder is frequently associated with psychological comorbidities such as anxiety and depression, significantly impacting patients' quality of life. A genetic predisposition is evident, with associated polymorphisms involving pathways relat

Novel transethosome coencapsulated combination for acne treatment: in-vitro efficacy and ex-vivo biodistribution studies.

BACKGROUND: Acne vulgaris is a skin condition affecting approximately 85% of young adults. It is influenced by androgens and primarily occurs in the pilosebaceous unit, where inflammation and obstruction happen. Hyperseborrhea and hyperkeratinization lead to increased levels of fatty acids and necrotic keratinocytes, promoting the proliferation of C. acnes phylotype IA1. METHODS: Here, the in-vitro efficacy of a novel combination on the main processes involved in acne was studied. Furthermore, the combination was coencapsulated in transethosomes to target the pilosebaceous unit, and the biodi

Acne microbiome: From phyla to phylotypes.

Acne vulgaris is a chronic inflammatory skin disease with a complex pathogenesis. Traditionally, the primary pathophysiologic factors in acne have been thought to be: (1) altered sebum production, (2) inflammation, (3) excess keratinization and (4) colonization with the commensal Cutibacterium acnes. However, the role of C. acnes has been unclear, since virtually all adults have C. acnes on their skin yet not all develop acne. In recent years, understanding of the role of C. acnes has expanded. It is still acknowledged to have an important place in acne pathogenesis, but evidence suggests that

Sarecycline inhibits protein translation in Cutibacterium acnes 70S ribosome using a two-site mechanism.

Acne vulgaris is a chronic disfiguring skin disease affecting ∼1 billion people worldwide, often having persistent negative effects on physical and mental health. The Gram-positive anaerobe, Cutibacterium acnes is implicated in acne pathogenesis and is, therefore, a main target for antibiotic-based acne therapy. We determined a 2.8-Å resolution structure of the 70S ribosome of Cutibacterium acnes by cryogenic electron microscopy and discovered that sarecycline, a narrow-spectrum antibiotic against Cutibacterium acnes, may inhibit two active sites of this bacterium's ribosome in contrast to the

High-frequency devices effect in vitro: promissing approach in the treatment of acne vulgaris?

BACKGROUND: Acne vulgaris is an inflammatory skin disorder leading to an impairment of quality of life and is therefore not only a cosmetic issue. Its pathogenesis is multifactorial - of particular importance is the colonization with the bacterium Propionibacterium acnes. A wide range of different treatment options exists including topical and systemic treatments depending on severity. High Frequency (HF) therapy, historically developed in the 19th century, claims antimicrobial effects on acne skin, but solid data on its efficacy and mechanism of action is lacking. OBJECTIVES: The main object…

Different Cutibacterium acnes Phylotypes Release Distinct Extracellular Vesicles.

Bacterial extracellular vesicles (EVs) perform various biological functions, including those that are critical to microbes. Determination of EVs composition allows for a deep understanding of their role in the bacterial community and communication among them. Cutibacterium acnes, formerly Propionibacteriumacnes, are commensal bacteria responsible for various infections, e.g., prosthesis, sarcoidosis, soft-tissue infections, and the most known but still controversial-acnes lesion. In C. acnes, three major phylotypes represented variable disease associations. Herein, for the first time, we prese…

Antibiotics and Antimicrobial Resistance in Acne: Epidemiological Trends and Clinical Practice Considerations.

Antimicrobial resistance is an increasing public health problem worldwide. The interest of a focus on antimicrobial resistance in acne lies on the facts that acne vulgaris (acne) is the most common skin disease worldwide, that the bacterium Cutibacterium acnes (C. acnes, formerly Propionibacterium acnes) plays a key role in the pathogenesis of acne, while at the same time being part of the skin flora, and that antibiotics are commonly recommended for acne treatment. The overuse of topical and/or systemic antibiotics, the long treatment courses used for acne, and the availability of over-the-co…

[Phylotyping and Determining the Antimicrobial Susceptibility of Cutibacterium acnes Isolated from Patients with Acne Vulgaris].

Cutibacterium acnes (formerly known as Propionibacterium acnes), obligate anaerobic gram-positive diphtheroid, is a member of normal skin microbiota and frequently isolated from acne lesions and also in various infections as an opportunist pathogen. Within the last decade, distinct phylogroups of C.acnes have been discovered, and specific strains associated with human disease were defined. Increasing resistance to antimicrobials used in the treatment of C.acnes infections has been reported. Resistance rates vary among isolates from different geographic locations. However, knowledge about the a…

pH-Dependent Antibacterial Activity of Glycolic Acid: Implications for Anti-Acne Formulations.

Glycolic acid is the smallest alpha hydroxy acid and widely used for skincare applications, including to treat acne vulgaris. Oftentimes, high concentrations of glycolic acid (~20-50 vol%) are incorporated into chemical peels to reduce acne-related inflammation while there is an outstanding need to determine to what extent glycolic acid can potently inhibit Cutibacterium acnes (formerly known as Propionibacterium acnes), which is a Gram-positive bacterium implicated in acne pathogenesis. Herein, we report that glycolic acid exhibits pH-dependent antibacterial activity against C. acnes and mech…

Selection of therapy for acne vulgaris: balancing concerns about antibiotic resistance.

The widespread use of long-term antibiotics to treat acne and rosacea has resulted in increased Propionibacterium acnes resistance to antibiotics. Although resistance may decrease treatment effectiveness in some cases, it also is clear that antimicrobial activity is not the only role antibiotics play in acne management. The anti-inflammatory activity of antibiotics contributes to antipropionibacterial efficacy. In addition, the use of benzoyl peroxide (BPO) in combination with antibiotics to reduce resistant populations of propionibacteria is widely advocated, as P acnes has not developed resi…

Acne: inflammation.

The inflammatory stage of acne vulgaris is usually of greatest concern to the patient. A number of morphologically different inflammatory lesions may form that can be painful and unsightly. In 30% of patients, such lesions lead to scarring(1). Inflammatory acne and acne scarring can have significant psychological effects on the patient, including depression, anxiety, and poor self-image(2). Although inflammatory acne has been well characterized clinically, the mechanisms by which inflammatory lesions arise are still poorly understood. The human skin commensal bacterium, Propionibacterium acnes…

Peptostreptococcus magnus does not cause hematogenous infections in experimental arthroplasties.

Rabbits with cemented endoprostheses in the knee joints were given 1-ml intravenous injections of suspensions containing Peptostreptococcus magnus (3 x 10(8) colony-forming units/ml) on three consecutive days seven weeks postoperatively. The animals were killed after another seven weeks. The bacterial challenge did not result in infection. This is in contrast to a previous study using the same experimental system with another anaerobic bacterial species, Propionibacterium acnes, which caused infection in approximately 50% of the animals. Both these bacteria have previously been reported to cau…
