Childhood trauma is associated with increased brain responses to emotionally negative as compared with positive faces in patients with psychotic disorders.

BACKGROUND: Childhood trauma increases risk of a range of mental disorders including psychosis. Whereas the mechanisms are unclear, previous evidence has implicated atypical processing of emotions among the core cognitive models, in particular suggesting altered attentional allocation towards negative stimuli and increased negativity bias. Here, we tested the association between childhood trauma and brain activation during emotional face processing in patients diagnosed with psychosis continuum disorders. In particular, we tested if childhood trauma was associated with the differentiation in b

Self-Reported Reasons for Smoking: Predicting Abstinence and Implications for Smoking Cessation Treatments Among Those With a Psychotic Disorder.

OBJECTIVES: People living with a psychotic illness have higher rates of cigarette smoking and face unique barriers to quitting compared to the general population. We examined whether self-reported reasons for smoking are useful predictors of successful quit attempts among people with psychosis. METHODS: As part of a randomized controlled trial addressing smoking and cardiovascular disease risk behaviors among people with psychosis, self-reported reasons for smoking were assessed at baseline (n = 235), 15 weeks (n = 151), and 12 months (n = 139). Three factors from the Reasons for Smoking Ques

Psychotic symptoms complicate the clinical differentiation of Parkinson's disease with major depressive disorder from dementia with Lewy bodies.

Dementia with Lewy bodies (DLB) is diagnosed clinically according to the diagnostic criteria in the Third Report of the DLB Consortium. However, psychotic symptoms, such as visual hallucinations, delusions, and stupor, may complicate the clinical diagnosis of DLB. The present study reports on a patient with Parkinson's disease that was difficult to distinguish from DLB because of the presence of various psychotic symptoms. In making a diagnosis of DLB, it is important to assess essential psychiatric features and to observe patients for any changes in these features.

Bender Gestalt Recall as a measure of short-term visual memory in children and adolescents with psychotic and other severe disorders.

To investigate the short-term visual memory ability of children and adolescents with severe psychiatric disorders, 82 child and adolescent inpatients and day hospital patients in a state psychiatric hospital were administered the Bender Gestalt Test as part of a psychological assessment and then asked to reproduce the designs from memory. No significant differences were found between groups on either the Bender Gestalt Recall, or the WISC-III IQs and the Digit Span and Symbol Search subtests for Psychotic Disorders (Schizophrenia, Schizoaffective Disorder, Psychosis Not Otherwise Specified), A…

Effect of trifluoperazine, a potential drug for tuberculosis with psychotic disorders, on the growth of clinical isolates of drug resistant Mycobacterium tuberculosis.

The effect of the antipsychotic drug trifluoperazine (TFP) on the in-vitro growth of 50 clinical isolates of Mycobacterium tuberculosis was tested. Of these isolates, 29 were susceptible to all five of the antitubercular drugs isoniazid, rifampicin, streptomycin, ethambutol and pyrazinamide, and 21 were resistant to one or more of the five drugs. The minimum inhibitory concentration (MIC) of TFP was 4 microg/ml for 40% of both the susceptible (12/29) and resistant (8/21) isolates and 8 microg/ml for 55% (16/29) and 48% (10/21) of the susceptible and resistant isolates respectively. Which clinical factors and biochemical parameters differentiate major depressive disorder with versus without lifetime psychotic symptoms?

OBJECTIVE: Major depressive disorder (MDD) is a prevalent disabling condition, with psychotic features complicating its course and management. Purpose of the study is to identify clinical and biochemical factors differentiating psychotic from non-psychotic MDD. METHODS: This is a retrospective single-centre study conducted on patients hospitalised between 2002 and 2022 at Fondazione IRCCS Policlinico (Milan, Italy) with a diagnosis of MDD. A large set of clinical and biochemical variables was collected on the first day of hospitalisation. Patients were divided according to the presence or abs

The role of social support in transdiagnostic symptom networks in affective and psychotic disorders: a cross-sectional and longitudinal network analysis.

Symptoms and psychosocial factors interact dynamically across affective and psychotic disorders, yet longitudinal evidence remains limited. Building on the network theory of psychopathology, this study conceptualizes mental disorders as systems shaped by symptom-environment interactions. Using data from a two-site German cohort (N = 2743), we examined 36 psychopathological symptoms and 11 psychosocial factors using cross-sectional and longitudinal analyses. At baseline, transdiagnostic symptom networks were estimated and compared between patients with affective or psychotic disorders (N = 1569…

A rare case of self-inflicted throat incision in a patient with psychotic depressive disorder: A case report and literature review.

Suicide is a complex phenomenon triggered by multiple factors. Cut throat injuries as a method of suicide are rare compared with other methods. This method is most commonly observed in young adult males in developing countries and is often associated with economic hardship. In injuries resulting from suicide attempts, hesitation marks may be present, and these marks assist forensic medicine in determining whether an injury is suicidal. We deemed it appropriate to present the general characteristics of suicidal cut throat injuries and the unique nature of our case in this article. A 71-year-old

Practices and attitudes of adult psychiatrists regarding methamphetamine-associated psychotic disorder: an internet based survey conducted in Turkey.

BACKGROUND: Many variables may affect approaches of psychiatrists to methamphetamine-associated psychotic disorder (MAP) treatment. This study was aimed to reach adult psychiatrists actively practicing in Turkey through an internet-based survey and to determine their practices and attitudes to MAP treatment. METHODS: In this internet-based study, participants were divided into three groups based on their answers: Those who do not follow-up any MAP patient were group 1 (n = 78), partially involved in the treatment process of at least one patient diagnosed with MAP were group 2 (n = 128), compl

Borderline Personality Disorder or First-Episode Psychosis? Challenges in Assessing Psychotic Features in Early Intervention: A Case Report.

INTRODUCTION: Adolescence is a crucial period for the onset of both borderline personality disorder (BPD) and first-episode psychosis. Although transient stress-related paranoid ideation and dissociative symptoms are one of the diagnostic criteria for BPD, determining when these psychotic features progress to early-stage schizophrenia remains unclear. METHODS: Two case studies aimed to explore the challenges in assessing and following up on psychotic features in BPD over time. Two adolescent girls aged 14 and 15 were initially diagnosed with BPD using semi-structured clinical interviews for D

The Relationship between Childhood Trauma Experiences and Psychotic Vulnerability in Obsessive Compulsive Disorder: An Italian Cross-Sectional Study.

People with obsessive compulsive disorder (OCD) are at increased risk of developing psychotic disorders; yet little is known about specific clinical features which might hint at this vulnerability. The present study was aimed at elucidating the pathophysiological mechanism linking OCD to psychosis through the investigation of childhood trauma experiences in adolescents and adults with OCD. One hundred outpatients, aged between 12 and 65 years old, were administered the Yale-Brown Obsessive Compulsive Scale (Y-BOCS) and its Child version (CY-BOCS), as well as the Childhood Trauma Questionnaire …

Hypothyroidism and sinus dysfunction associated with lithium-paliperidone combination therapy for bipolar disorder with psychotic symptoms: a case report.

BACKGROUND AND AIM: Lithium is considered to be the first-line treatment for bipolar disorder, and paliperidone was approved for the treatment of schizophrenia and acute bipolar manic/mixed episodes. However, both agents have been associated with thyroid dysfunction and cardiovascular adverse effects like subclinical hypothyroidism, bradycardia, and sinus arrest, even at therapeutic doses. CASE PRESENTATION: Here, we reported a case of a 17-year-old Han Chinese female who developed symptomatic hypothyroidism, sinus bradycardia, and sinus arrest while being treated with lithium and paliperidon

Placebo Effect in Randomized Trials of Major Depressive Disorder With Psychotic Features: A Systematic Review and Descriptive Meta-Analysis.

BACKGROUND: In the 1980s, the response rate of major depressive disorder with psychotic features (MDD-Psy) to placebo pills was reported to be close to 0%. To our knowledge, this placebo response rate has not been systematically reassessed. We undertook a systematic review of randomized controlled trials (RCTs) that have used a placebo or sham control group for MDD-Psy. METHODS: We searched MEDLINE and identified 9 relevant publications reporting on 10 studies comparing a placebo or sham interventions versus an active intervention. We extracted reported rates of response or of dropout for all

The time course of psychotic symptom side effects of ketamine in the treatment of depressive disorders: a systematic review and meta-analysis.

OBJECTIVE: Ketamine is a potential rapid-acting treatment for depression. Studies have suggested that the side effects are minimal and temporary, but the psychotic symptom side effects have yet to be fully examined. This study investigated whether ketamine infusion in the treatment of mood disorders is associated with increases in positive symptoms and whether these symptom effects endure over time. METHODS: A systematic review and meta-analysis of studies of ketamine in the treatment of depression. Embase and Medline databases were searched for studies including (a) participants with major a

Factors associated with psychotic symptoms among a sample of male prisoners with substance use disorder: A cross-sectional study.

BACKGROUND: Prisoners have a higher prevalence of substance use disorder (SUD) than the nonincarcerated population. Many studies report that an SUD increases the likelihood of psychotic symptoms/disorders. Inmates, therefore, may be at higher risk for psychotic disorders. OBJECTIVE: The main objectives of this study were to (1) estimate the prevalence of psychotic symptoms in a sample of male inmates with high levels of SUD and (2) verify if type of abuse or other sociodemographic/clinical features are risk factors of psychotic symptoms. In light of the high prevalence of childhood trauma (CT…

Association Between Schizophrenia-Related Polygenic Liability and the Occurrence and Level of Mood-Incongruent Psychotic Symptoms in Bipolar Disorder.

IMPORTANCE: Bipolar disorder (BD) overlaps schizophrenia in its clinical presentation and genetic liability. Alternative approaches to patient stratification beyond current diagnostic categories are needed to understand the underlying disease processes and mechanisms. OBJECTIVE: To investigate the association between common-variant liability for schizophrenia, indexed by polygenic risk scores (PRSs), and psychotic presentations of BD. DESIGN, SETTING, AND PARTICIPANTS: This case-control study in the United Kingdom used multinomial logistic regression to estimate differential PRS associations

Lifetime presence of psychotic symptoms in bipolar disorder is associated with less favorable socio-demographic and certain clinical features.

BACKGROUND: The presence of psychotic symptoms in bipolar disorder (BD) is considered a feature of higher severity of illness and, in particular, of manic episodes in bipolar I disorder (BD I). However, the possibility to apply the "with psychotic features" specifier to major depressive episodes in either bipolar II disorder (BD II) or BD I highlights the need for additional research in this area. METHODS: The present study assessed the lifetime presence of psychotic symptoms and related socio-demographic and clinical features in a large sample of BD patients (N=360), with (BDPs, N=207) and w
