Pre-pregnancy cardiac status impacts risk of postpartum mood and anxiety disorders.

OBJECTIVE: Our study aimed to assess whether maternal cardiac disease is an independent risk factor for postpartum mood and anxiety disorders (PPMADs) and whether higher cardiac risk scores are associated with higher risk of PPMAD. METHODS: This was a retrospective matched cohort study of patients who delivered between June 2019 and February 2024 and completed PPMAD screening at a tertiary care institution. The cardiac cohort was managed by a multidisciplinary cardio-obstetrics team, and the non-cardiac cohort was identified through matching on important demographic factors. The primary outco

[Psychopharmacotherapy in pregnancy and lactation. 2: Lactation].

Whilst the incidence of psychiatric disorders decreases during pregnancy, the risk during the postpartum period increases significantly, often leading to the necessity of psychopharmacological intervention during the puerperium, and subsequently during lactation and breast-feeding. The necessity for lithium prophylaxis in manic-depressive women after childbirth has been identified, and it is recommended that weaning rather than omission of psychopharmacological treatment is preferable during the puerperium.

Cerebrovascular disease and stroke in women.

There are many special conditions which may predispose women to have a higher risk of stroke. These conditions include pregnancy and the puerperium, exogenous estrogen use, cerebral vasculitis, mitral valve prolapse and migraine. Recognition that certain stroke syndromes are more likely to affect women may aid the clinician in determining the optimal management of cerebrovascular disease and stroke in women.

Postpartum psychosis and postpartum thyroiditis.

The term postpartum psychosis refers to a group of severe and heterogeneous disorders with psychotic symptoms that occur most frequently in the context of a mood disorder during the postpartum period. We report a case of 'postpartum psychosis' possibly associated with postpartum thyroiditis in a 29 year-old woman. The appearance of psychotic symptoms was chronologically related to the onset of postpartum thyroiditis and resolution of psychosis synchronized with the achievement of biochemical euthyroidism. The patient had typical symptoms of 'classic postpartum psychosis' (a historical term not

Comparison between dairy cow disease incidence in data registered by farmers and in data from a disease-recording system based on veterinary reporting.

Sweden has a national disease-recording system based on veterinary reporting. From this system, all cattle-disease records are transferred to the dairy industry cattle database (DDD) where they are used for several purposes including research and dairy-health statistics. Our objective was to evaluate the completeness of this data source by comparing it with disease data registered by dairy farmers. The proportion of veterinary-treated disease events was estimated, by diagnosis. Disease incidence in the DDD was compared, by diagnosis and age, with disease data registered by the farmers. Compari

Protocol for economic evaluation alongside a cluster-randomised controlled trial of a psychoeducational intervention for the primary prevention of postnatal mental health problems in first-time mothers.

INTRODUCTION: Postnatal mental health problems, which are an international public health priority, are a suitable target for preventive approaches. The financial burden of these disorders is borne across sectors in society, including health, early childhood, education, justice and the workforce. This paper describes the planned economic evaluation of What Were We Thinking, a psychoeducational intervention for the prevention of postnatal mental health problems in first-time mothers. METHODS AND ANALYSIS: The evaluation will be conducted alongside a cluster-randomised controlled trial of its cl

Postpartum Anorectal and Pelvic Floor Disorders: Evaluation, Treatment, and Prevention.

PURPOSE OF REVIEW: Postpartum anorectal and pelvic floor disorders (PFD) are common, though under-recognized. There is limited knowledge regarding their diagnosis, treatment, and prevention. Here we provide a critical review of this topic and highlight knowledge gaps and treatment options for these problems. RECENT FINDINGS: Recent advances include dynamic 3D and 4D ultrasound of the pelvic floor to reveal pathology, anal sphincter defects, and pelvic organ prolapse. Treatments for fecal incontinence include anal inserts, vaginal inserts, translumbosacral neuromodulation therapy (not yet stud

Health practitioners' recognition and management of postpartum obsessive-compulsive thoughts of infant harm.

The postpartum period has been associated with elevated rates of onset of obsessive-compulsive disorder (OCD) among women, with a prevalence of 2-9%. Postpartum OCD is often characterized by recurrent, unwanted, and highly distressing thoughts, images, or impulses of deliberate infant harm. This study investigated health practitioners' recognition of, and clinical management strategies for, postpartum obsessive-compulsive symptoms (OCS). Ninety-four perinatal health practitioners from a range of disciplines and professional backgrounds completed a survey comprised of a hypothetical case vignet

Neonatal benzodiazepines exposure during breastfeeding.

OBJECTIVE: To assess central nervous system depression and other adverse effects in infants exposed to benzodiazepines through breast milk. STUDY DESIGN: A prospectively recruited, retrospectively assessed cohort study of mothers who contacted the Motherisk program regarding the safety of benzodiazepines and were invited to participate in a follow-up program regarding the effects of these medications on their infants during lactation. RESULTS: A total of 124 consenting women participated. Adverse outcomes, specifically sedation, was identified in only 1.6% (2 of 124) of infants and was not a

Measuring hypomania in the postpartum: a comparison of the Highs Scale and the Altman Mania Rating Scale.

We examine the prevalence of hypomania on day 3 postpartum using two self-report mania scales: The Highs Scale and Altman Mania Rating Scale (AMRS). 279 women were recruited from postnatal wards and completed the questionnaires on day 3 postpartum. The scales show good correlation, however, 11% of women meet the suggested threshold for caseness on the Highs Scale and 44% on the AMRS. Hypomanic symptoms are commonly experienced in the early postpartum. Although there is some evidence that the Highs Scale might be conservative, the AMRS likely overestimates hypomania in the postpartum. The defin

Determining the reliability of ultrasound measurements and the validity of the formulae for ultrasound estimation of postvoid residual bladder volume in postpartum women.

AIMS: To determine the reliability and validity of ultrasonic assessment of the postvoid residual bladder volume (PVRBV) in postpartum women. METHODS: Ultrasound estimated PVRBVs were performed on 99 consecutive women with postpartum urinary retention, to validate 12 published bladder volume-estimation formulae. This ultrasound-predicted volume was compared with the immediately collected catheterized volume. Comparison of individual formula with the catheterized volumes was performed by using the Intraclass Correlation Coefficient (concordance), and difference plots (bias, linearity of the di
