Cerebral blood flow studies in the diagnosis and management of intrauterine growth restriction.

PURPOSE OF REVIEW: To provide evidence that fetal brain vasodilatation can be related with postnatal cerebral structural and functional complications. RECENT FINDINGS: Most early-onset intrauterine growth restricted (IUGR) fetuses show signs of brain vasodilatation. As most of them are born prematurely, a high prevalence of short-term and long-term neurological complications is expected. However, the clinical significance of brain vasodilatation might be different. In the early stage of fetal deterioration, the risk of structural brain damage is low, but at advanced stages there is an increas

Aortic stiffness is increased in polymyalgia rheumatica and improves after steroid treatment.

BACKGROUND: Inflammatory rheumatic diseases have been associated with increased cardiovascular risk and arterial stiffness. Polymyalgia rheumatica (PMR), a disease which affects primarily older people, is characterised by a systemic inflammatory response but little is known about aortic involvement in PMR. A study was undertaken to investigate whether aortic stiffness is increased in PMR and whether it improves after steroid treatment. METHODS: Thirty-nine patients with PMR (age 72 ± 8 years, 44% men, blood pressure (BP) 134/75 ± 16/9 mm Hg) and 39 age-, sex- and BP-matched control subjects u

[Assessment of major artery stiffness: new perspectives of noninvasive diagnosis of coronary atherosclerosis].

AIM: Arterial stiffness is a predictor of atherosclerosis. This study was conducted to develop a method of coronary atherosclerosis severity assessment by means of brachial-ankle pulse wave velocity (baPWV). MATERIAL AND METHODS: We measured baPWV in 119 males (age 51.67 +/- 7.25) who received coronary angiographic examination (CAG). The baPWV was measured by Vasera VS-1000 (Fukuda Denshi). RESULTS: The patients were divided into two groups by severity of stenosis (group 1--less than 50% stenosis, group 2 > 50%). The baPWV value was significantly greater in group 2 (n = 98, baPWV 13.15 +/- 2

Arterial stiffness and QT interval prolongation in a general population: the Hisayama study.

Few population studies have addressed the association of QT interval prolongation with clinical or subclinical arterial disease. The primary objective here was to examine the relationship between the pulse wave velocity (PWV) and the heart rate-corrected QT interval duration (QTc). This is a cross-sectional study, based on a survey of a general population of Japanese. We examined 2,666 community-dwelling individuals without history of cardiovascular disease, aged 40 or over. The PWV was measured between the brachial and ankle regions (baPWV). QTc was estimated using Bazett's equation. The age-

Assessment of endothelial function in the radial artery using inhaled albuterol.

Endothelial dysfunction is an early indicator of developing atherosclerosis and is a strong predictor of future heart attack and stroke. At present, evaluation of endothelial function (EF) (specifically, EF mediated by nitric oxide, NO) is too technically difficult to form part of a routine clinical examination. Non-invasive methods that measure NO-dependent EF in arteries make use of a 4-5 minute blood pressure cuff occlusion of the arm in order to induce reactive hyperemia (RH) upon cuff release. The increased blood flow that results from the RH stimulates the endothelial cells to release NO

Changes in pulmonary venous Doppler parameters in fetal cardiac defects.

OBJECTIVES: Recent observations in pediatric cardiological studies have prompted discussion on the possible role of intrauterine pulmonary venous changes in neonatal and postoperative outcome of infants with congenital heart defects. This study analyzes changes of Doppler blood flow velocity waveforms in the pulmonary veins of fetuses with different cardiac defects. METHODS: Eighty fetuses (mean gestational age, 27 weeks) with prenatally diagnosed cardiac defects were classified into one of five groups: obstructed left atrium, other left ventricular outflow tract obstruction, right ventricula

Arterial distensibility as determined by carotid-femoral pulse wave velocity in patients with Behçet's disease.

Behçet's disease (BD) is a chronic, multisystem disorder characterized by genital and oral aphthae, skin lesions, uveitis, and tendency to thrombosis. Pulse wave velocity (PWV) is an important factor in determining cardiovascular mortality and morbidity. It is an index of arterial wall stiffness and inversely related to the arterial distensibility. In this study we investigated the arterial distensibility in BD by PWV. We studied 14 patients with BD (18-44 years old, 10 men) and 28 healthy subjects (18-39 years old, 21 men) without known cardiovascular disease. Arterial distensibility was asse

A mathematical description of blood spiral flow in vessels: application to a numerical study of flow in arterial bending.

Local arterial haemodynamics has been associated with the pathophysiology of several cardiovascular diseases. The stable spiral blood-flows that were observed in vivo in several vessels, may play a dual role in vascular haemodynamics, beneficial since it induces stability, reducing turbulence in the arterial tree, and accounts for normal organ perfusion, but detrimental in view of the imparted tangential velocities that are involved in plaque formation and development. Being a spiral flow considered representative of the local blood dynamics in certain vessels, a method is proposed to quantify

Doppler ultrasound studies in pelvic inflammatory disease.

Ten women with tubo-ovarian abscess caused by pelvic inflammatory disease (PID) were investigated by transvaginal Doppler ultrasound during the acute and healing phases of the infection. The pulsatility index (PI) of the uterine arteries was measured and compared with the values obtained from 19 healthy women. Each control patient was investigated three times during a single menstrual cycle. In PID patients, the PI values were significantly lower than in controls in the same phase of the menstrual cycle. When C-reactive protein was > 50, the PI values were lowest and reverted to normal values

Changes in middle cerebral artery blood velocity in uremic patients after hemodialysis.

BACKGROUND AND PURPOSE: Strokes are a frequent complication in uremic patients on dialysis. We wanted to evaluate the effect of this treatment on cerebral hemodynamic parameters, particularly those of patients with carotid stenosis, who are at higher risk for atherothrombotic ischemic events. METHODS: We used transcranial Doppler ultrasonography to evaluate blood velocity of the middle cerebral artery in 18 uremic patients before and after hemodialysis. Carotid stenosis was evaluated by echo-Doppler investigation. Six patients were also studied before and after recombinant human erythropoieti

Erectile dysfunction in hypertensive men: sleep-related erections, penile blood flow and musculovascular events.

To explore how hypertension affects penile erection, we studied erectile hemodynamics during nocturnal penile tumescence in 3 groups of middle-aged men: hypertensive patients with and without erectile dysfunction, and normotensive controls without erectile problems. The hypertensive patients were not taking antihypertensive medication. Evaluations included standard monitoring of penile circumference change as well as noninvasive monitoring of penile segmental pulsatile blood flow and activity in the bulbocavernosus-ischiocavernosus muscles. Variables differed in how they discriminated among gr

A review of infections in patients with left ventricular assist devices: prevention, diagnosis and management.

Since the advent of ventricular assist devices with smaller configurations and continuous-flow technology, survival rates for patients with end-stage heart failure have dramatically improved. While the burden of infectious complications is decreased in patients on continuous-flow ventricular assist devices compared to bulkier pulsatile-flow devices, infection remains one of the most common causes of morbidity and mortality. The majority of infections occur at the driveline exit site, beginning with a disruption or trauma to the barrier between the skin and driveline and sometimes spreading dee

The influence of combined regional anesthetic blockade, compared with local anesthesia, on hemodynamic parameters and pain criteria in patients receiving an arteriovenous fistula for hemodialysis.

BACKGROUND: In the creation of arteriovenous fistulas (AVF) for hemodialysis, regional brachial plexus block (BPB) has a sympatholytic effect, increasing blood flow and diameters in the vessels. METHODS: A prospective, randomized, clinical-surgical pilot study was carried out, comprising 40 patients, randomly allocated into two groups (BPB or local anesthesia-LA). The diameter (veins and arteries), and the flow, peak systolic velocity (PSV), pulsatility index (PI), and pulsatility index ratio (PIR) of the arteries were evaluated before and after anesthesia. Flow was measured in the AVF at the

[Evaluation and Optimization of Microvascular Arterial Anastomoses by Transit Time Flow Measurement].

UNLABELLED: INDRODUCTION: The regular application of transit time flow measurement in microvascular anastomoses during heart surgery has lead to improvements of the outcome of coronary artery bypass grafts. Our study was meant to discover whether this measurement method was also applicable for evaluation and optimization of microvascular arterial anastomoses of radial forearm flaps. METHODS: In this prospective examination a combining ultrasound imaging and transit time flow measurement device (VeriQ, MediStim) was used during surgery to assess anastomotic quality of 15 radial forearm flaps.

Color Doppler imaging of the ocular ischemic syndrome.

PURPOSE: This study describes hemodynamic characteristics of the ophthalmic, central retinal, and posterior ciliary arteries in 16 eyes of 11 patients with the ocular ischemic syndrome. Understanding the hemodynamic characteristics of the retrobulbar circulation may elucidate the natural history and pathophysiology of the ocular ischemic syndrome and perhaps form the basis for rational treatment of this condition. METHODS: Color Doppler imaging, a procedure that permits rapid noninvasive imaging of the ophthalmic, central retinal, and posterior ciliary arteries, was used to quantitate peak sy

MR phase imaging and cerebrospinal fluid flow in the head and spine.

Motion of the cerebrospinal fluid (CSF) in and around the brain and spinal cord was examined in healthy subjects and in a number of patients with abnormalities of the CSF circulation. The pulsatile motion of the CSF was determined by spin echo phase (velocity) imaging, sometimes in combination with gradient echo phase contrast cine. Differences in flow patterns across CSF spaces were observed: flow reversal in the cerebellomedullary cistern and lumbar area relative to cervical CSF, and in the posterior versus the anterior subarachnoid space in the spinal canal. Flow communication was demonstra

Hemodynamics with mechanical circulatory support devices using a cardiogenic shock model.

Mechanical circulatory support (MCS) devices, including veno-arterial extracorporeal membrane oxygenation (VA-ECMO) and Impella, have been widely used for patients with cardiogenic shock (CS). However, hemodynamics with each device and combination therapy is not thoroughly understood. We aimed to elucidate the hemodynamics with MCS using a pulsatile flow model. Hemodynamics with Impella CP, VA-ECMO, and a combination of Impella CP and VA-ECMO were assessed based on the pressure and flow under support with each device and the pressure-volume loop of the ventricle model. The Impella CP device wi…

Feasibility of Vascular Parameter Estimation for Assessing Hypertensive Pregnancy Disorders.

Hypertensive pregnancy disorders (HPDs), such as pre-eclampsia, are leading sources of both maternal and fetal morbidity in pregnancy. Noninvasive imaging, such as ultrasound (US) and magnetic resonance imaging (MRI), is an important tool for predicting and monitoring these high risk pregnancies. While imaging can measure hemodynamic parameters, such as uterine artery pulsatility and resistivity indices (PI and RI), the interpretation of such metrics for disease assessment relies on ad hoc standards, which provide limited insight to the physical mechanisms underlying the emergence of hypertens
