Risk factors for redisplacement of pediatric distal forearm and distal radius fractures.

BACKGROUND: Fractures of the distal forearm and distal radius represent the most common types of fracture in the pediatric population, with the majority treated by closed reduction and cast. Redisplacement has been known to occur in up to 39% of cases. There have been numerous risk factors and radiologic indices put forward as methods of predicting redisplacement, but this topic remains a matter of debate. This retrospective study aims to further assess the significance of the many factors in redisplacement after treatment with closed reduction. METHODS: This retrospective study included 155

Incidence of extensor pollicis longus tendon rupture after nondisplaced distal radius fractures.

PURPOSE: The incidence of extensor pollicis longus (EPL) tendon rupture in the setting of nondisplaced distal radius fractures is unknown. Extensor pollicis longus rupture is a known complication after distal radius fractures and is believed to occur more frequently after minimally displaced and nondisplaced distal radius fractures. Our study sought to define the incidence of EPL tendon rupture after nondisplaced distal radius fractures presenting to a level 1 trauma center. METHODS: Using our billing database, we identified distal radius fractures presenting to our institution between 2006 a…

[Arthroscopic assessment of articular fractures of distal radius osteosyntheses by percutaneous pins].

OBJECTIVES: We studied the radiological results of percutaneous pinning in articular fractures of the distal radius. We did not, thereby, have a method of direct ligamentous and articular assessment. We realized an anatomical arthroscopic assessment at six weeks of osteosynthesis of wrists. The purpose was to study the correlation between the presence of anatomical lesions and the functional result. METHODS: Thirty-five patients presenting an articular fracture with posterior displacement were included; 30 women and five men. The mean age was 62 years. All the patients were right -handed and

Distal radius fractures with diaphyseal involvement: fixation with fixed angle volar plate.

BACKGROUND: The purpose of this study was to evaluate the outcome of fractures of the distal radius with metaphyseal and diaphyseal involvement treated with fixed angle volar plates. MATERIALS AND METHODS: Twenty-one patients with fracture of the radius involving the diaphyseal, metaphyseal and epiphyseal parts were treated with fixed angle plate fixation through an extended volar Henry's approach. Circle wire loops, screws and intrafocal wire fixations were associated in 12 cases. Coexisting ulnar fractures were fixed with plates or K-wires in 8 cases. All patients were prospectively followe

Dynamic and static external fixation for distal radius fractures--a systematic review.

INTRODUCTION: External fixation of distal radius fractures may be static (wrist-bridging) or dynamic (wrist-bridging with mobile hinge or non-bridging). The aim of this systematic review is to investigate the effectiveness of different methods of external fixation for unstable distal radius fractures. METHODS: A Medline database search was performed with strict eligibility criteria to obtain the highest quality evidence from meta-analyses, RCTs and comparative studies. Eligible studies were critically appraised using levels of evidence and RCTs were further appraised using a validated scoring…

[Clinico-radiologic evaluation of distal radius fractures treated with a percutaneous technique].

In 1913 Lambotte began treating distal radius fractures surgically using percutaneous fixation with a Kirschner wire; in 1984 Clancey modified the percutaneous technique by using crossed wires with good results; however, various therapeutic approaches are needed to treat the fractures in elderly or osteopenic patients. The purpose of this study is to present the results of the treatment of distal metaphyseal radius fractures types 3, 4 and 5 according to Fernandez in 21 patients who underwent closed reduction and percutaneous fixation with the Clancey technique, with a forearm-palmar cast, fro…

Skeletally mature patients with bilateral distal radius fractures have more associated injuries.

Bilateral distal radius fractures are rare injuries and only a handful of case reports exist. Understanding the demographic variables and associated injuries in patients with these fractures may improve awareness and treatment of concomitant injuries. We determined the differences in mode of trauma and associated injuries between skeletally mature and skeletally immature patients with bilateral distal radius fractures. We retrospectively reviewed the records of 93 patients with bilateral distal radius fractures. We compared demographic data, fracture patterns, mode of injury, treatment modalit…

[Analysis of acute distal radius fractures with the articular sagittal index (ASI)].

Displacements of articular fragments are not always visible on initial radiographs of acute distal radius fractures. Despite the wide use of CT scan, it may be useful to know a simple index that could suggest an articular displaced fracture from the initial standard radiographs. The authors describe an articular sagittal index (ASI) of the distal radius and provide reference values from a study of 100 healthy volunteers. An increase of the ASI index suggests a displaced articular distal radius fracture with possible impaction.

[Mal-united fractures of the distal radius--biomechanics and operative treatment options].

Despite all progress in the treatment of distal radius fractures, malunion is still one of the most common complications. It may be true that not all non-anatomically aligned fractures of the distal radius are causing disabilities. However, most patients with a distal radial malunion complain about loss of wrist motion and forearm rotation, loss of grip strength, and pain. These complaints should lead to a sophisticated clinical examination and a careful evaluation of the radiographs of the wrist, and - if needed - to further investigations. Treatment options are procedures solely aimed to dim…

Current concepts in volar fixed-angle fixation of unstable distal radius fractures.

UNLABELLED: We present new developments in the volar treatment of unstable distal radius fractures in adults. New perspectives on the anatomy of the wrist, the watershed line on the volar radius and the usefulness of the pronator fossa are presented and these help to avoid flexor and extensor tendon disturbance when using a volar approach. Other new insights on the bony anatomy of the distal end of the radius are discussed, which are important in improving the quality of fracture fixation, including the benefits of constructing a precise fixed-angle scaffold underneath the articular surface in…

Fixed angle fixation of distal radius fractures through a minimally invasive approach.

Treating unstable distal radius fractures in osteoporotic patients remains a challenge for the surgeon. Fixed angle plate fixation requires ample surgical dissection but has been shown to improve stability, allow early functional use of the hand and facilitate rehabilitation. We herein describe a treatment method that provides the benefits of fixed angle fixation while utilizing a minimally invasive approach. Stability is achieved by the use of a new implant that is placed through a small dorsal incision and minimizes extensor tendon disruption. This method finds application in the unstable ex…

[Fractures of the distal radius. Minimally invasive pin fixation: indications and results].

Therapy of distal radius fractures in the elderly is increasingly subject to higher demands on functional results, especially in terms of patient comfort, load-bearing, tissue atrophy, and osteoporosis. From 1 January 1998 until 30 June 1999, 215 patients with distal radius fractures were treated at our hospital. One hundred eighteen of them had follow-up examinations after an average of 33 months. Using objective criteria (X-ray and functional), results were very good in 68.9% of the cases and good in 28.6%. Under subjective criteria, too, most of the patients had very little or no complaints…

[New principles in the surgical treatment of distal radius fractures -- locking implants].

The treatment goal for fractures of the distal end of the radius is full functional recovery of the wrist. Prerequisites are restoration of the anatomy and early functional after treatment in accordance to the treatment guidelines for other articular and juxta-articular fractures. A better understanding of the anatomy and new biomechanical models lead to the development of new implants and operative techniques. The LCP-concept with locking screws had a considerable impact on recent advances in the operative treatment of distal radius fractures.

Simultaneous distal radius and hip fractures in elderly patients--implications to rehabilitation.

PURPOSE: The reasons for fractures in the elderly patients are multifactorial. Osteoporosis is considered to be the main pathology. Other reasons are the increase in fall frequency and the protective response to trauma. The most common sites at which these fractures occur are the hip, vertebra and distal radius. A combination of these is uncommon. METHODS: All patients who were treated between January 1990 and December 2000 with a combination of distal radius and hip fractures and were older than 65-years were retrospectively evaluated. The following parameters were evaluated: age; sex; pre f

[Primary arthroscopic treatment of TFCC tears in fractures of the distal radius].

The primary simultaneous diagnosis and therapy of TFCC tears in distal radius fractures is still the exception. We present our results of arthroscopic treatment of these injuries. From January 1998 until September 1999, we treated 21 patients with a type C fracture (AO classification) of the distal radius and one patient with a scaphoid fracture and TFCC tears. The Palmer 1A (n = 5) and 1 C (n = 4) tears have been arthroscopically shaved as well as the meniscal tear (n = 1). The Palmer 1B tears were refixed either in an outside-inside technique (n = 6) or in a new all-inside technique (n = 6).…

Tilted lateral radiographs in the evaluation of intra-articular distal radius fractures.

The anatomy of the distal radius confounds the assessment of fracture displacement of the lunate facet. Since reduction of the articular surface is paramount to obtaining good clinical results, this study was designed to evaluate whether a radiograph taken 22 degrees from true lateral (forearm held at a 22 degrees angle from the horizontal film cassette) would enhance the accuracy of measuring displacement. Fifteen lunate facet fractures of varying depression were produced in 7 fresh cadaveric wrists. Posteroanterior (PA), standard lateral, and 22 degrees tilted lateral radiographs were obtain

Long-term results of radioscapholunate fusion following fractures of the distal radius.

Fifteen patients with radioscapholunate (RSL) fusion for traumatic lesions of the radiocarpal junction, whose short-term results have been previously reported, were reassessed after an average follow-up time of 8 years. Five patients had undergone wrist fusion because of non-union or early progressive arthritis. Of the ten wrists with retained mobility, eight continued to function satisfactorily. Two wrists were painful for reasons other than secondary midcarpal arthritis. Patient satisfaction was comparable in both groups with the wrist score better for wrists with residual motion. The surviv
