We report a case of multimetastatic malignant melanoma that was diagnosed after histopathologic examination of the excised paratesticular mass. Our patient initially visited the neurosurgery clinic due to low back pain. After preliminary evaluation, he was referred to our clinic due to a right scrotal mass. We excised the paratesticular mass and testis. The histopathologic findings were consistent with malignant melanoma. He was referred to the oncology department for adjuvant treatment. The patient died during the third month of chemotherapy. Our case was unique because the malignant melanoma

To determine the relative frequency of different diseases and of functional gastrointestinal disorders among patients referred by general practitioners to a gastroenterology clinic, 2000 patients referred over a five-year period were studied. 980 had organic diseases, of which peptic ulcer, oesophagitis, and inflammatory bowel disease accounted for about half. 888 patients had functional disorders of the gastrointestinal tract, without any disease. Among these, various syndromes could be distinguished; abdominal pain with altered bowel habit (irritable bowel syndrome, spastic colon type) accou

Breast pain has no association with breast cancer yet is a frequent reason for referral from Primary to Secondary Care, often on an urgent (2-week wait) referral. The referral often causes significant patient anxiety, further heightened by screening mammograms and/or ultrasound scans in the absence of an associated red flag symptom or finding by the patient or general practitioner. This paper reports the pilot implementation of a specialist Primary Care Breast Pain Clinic in Mid-Nottinghamshire where patients were seen, examined without any imaging and assessed for their risk of familial breas

BACKGROUND: Psoriatic arthritis (PsA) is a chronic inflammatory arthritis involving axial and peripheral joints and tendons that affects a subset of patients with psoriasis (PsO). PsA can be a debilitating disease and warrants prompt rheumatologic evaluation and management. The diagnosis of PsA can be challenging for the practising dermatologist as there is often an overlap in the symptoms of PsA and non-inflammatory musculoskeletal conditions such as osteoarthritis, tendonitis and myofascial pain.
AIMS: The primary aim of the study is to examine psoriasis patients seen at our tertiary instit

OBJECTIVES: To describe and compare the characteristics of patients fulfilling the ASAS criteria for axial Spondyloarthritis (SpA) versus peripheral SpA .
METHODS: Baseline dataset from the ESPeranza cohort was used. In this programme, patients were referred to rheumatologist in case of (1) age <45 years, (2) symptoms duration 3-24 months and (3) inflammatory back pain (IBP) or asymmetrical arthritis or spinal/joint pain plus ≥1 SpA features. The programme was developed between April 2008 and June 2011. Data from 377 patients fulfilling the ASAS classification criteria for SpA were used. Desc

This case report describes an atypical presentation of anterior cutaneous nerve entrapment syndrome (ACNES). A male patient with refractory … de neuropathic pain (RNP) may arise from entrapment of the anterior cutaneous nerve, manifesting as referred pain in the lower back. Even in the absence of abdominal symptoms, a positive Carnett's sign should prompt the consideration of abdominal wall nerve entrapment and associated RNP in the differential diagnosis of refractory low back pain.

BACKGROUND: The following case describes management of severe persistent vulvar pain from postherpetic neuralgia through a ganglion impar block.
CASE REPORT: A 21-year-old female patient initially presented with genital lesions consistent with herpes simplex virus-1 (HSV-1) and antibody testing suggestive of primary HSV-1 infection. After her genital lesions resolved, she continued to have severe persistent vulvar pain and was diagnosed with postherpetic neuralgia, approximately one year after initial presentation. After unsuccessful medical management with her gynecologist and primary care p

Trigeminal neuralgia (TN) is characterized by recurrent paroxysms of unilateral facial pain that typically is severe, lancinating, and activated with cutaneous stimulation. There are two types of TN, classical TN and atypical TN. The pain nature of classical TN are the same as those described above, whereas atypical TN is characterized by constant, burning pain. We describe the case of a 49-year-old male presenting with right-sided facial pain. The patient was diagnosed with temporomandibular joint disorder at a dental clinic and was on medical treatment, but his symptoms worsened gradually. H

The relationship of personality variables and patient recruitment to pain coping strategies and psychological distress was assessed in a Dutch sample of 111 chronic tension headache patients. Using the Coping Strategy Questionnaire (CSQ), high scores on the factor of helplessness proved to be associated with psychological distress. In particular, patients who manifested neuroticism and hostility as personality traits and who were referred for treatment by physicians achieved higher scores on the factor of helplessness. Patients who reported a lower level of pain intensity manifested a higher p

Visceral pain remains a major clinical challenge, often resistant to conventional analgesics. Sigma-1 receptor (σ1R) antagonists enhance opioid antinociception in somatic and neuropathic pain, yet their role in visceral nociception remains unclear. Here we investigated whether σ1R blockade potentiates the antinociceptive effects of µ-opioid agonists in a well-established model of capsaicin-induced visceral pain in mice. To this end, female wild-type (WT) and σ1R-knockout (σ1R-KO) mice received intracolonic capsaicin (0.1%). Visceral pain-related behaviors and referred mechanical hyperalgesia w

BACKGROUND: Loin pain haematuria syndrome is characterised by episodes of loin pain and microscopic or macroscopic haematuria, without a urological origin.
CASE DESCRIPTION: We describe a 39-year-old woman who was referred to us because of microscopic haematuria and proteinuria without an apparent cause, which had been present for 20 years. For 9 months she had also had continuous loin pain, aggravated by exertion. Additional examination showed erythrocytes in the renal tubules and a thin glomerular basement membrane. We made the diagnosis of "loin pain haematuria syndrome based on thin basem

We present the fictional case of a 29-year-old man with sickle cell disease referred to psychiatry for evaluation of depression during an acute pain episode. Consultation-liaison psychiatrists with expertise in sickle cell disease provide guidance for this commonly encountered clinical case based on their experience and a review of the available literature. Key teaching points include the high prevalence of mood and cognitive disorders in this population, as well as pertinent issues related to chronic pain, opioids, and stigma.

We report the case of a 67-year-old male who was referred to our hospital due to severe abdominal pain and fever. His past medical history included a total gastrectomy surgery for stomach cancer. Abdominal examination showed intense pain on deep palpation in the epigastric region. Laboratory test revealed elevated C-reactive protein (235 mg/l) and amylase (1,789 U/l). Computerized tomography reported a pancreatic collection with the presence of air bubbles inside and areas of necrotic pancreatic parenchyma.

A 26-year-old woman was referred to physical therapy with lower back pain extending into the right buttock. Palpation of the right buttock revealed a firm, immobile mass of considerable size, which the patient identified as her locus of symptoms. Due to the noted palpable mass, her pain, her inability to sit normally, and the longevity of symptoms, the physical therapist ordered radiographs, which identified multiple osteochondromas. J Orthop Sports Phys Ther 2017;47(6):442. doi:10.2519/jospt.2017.6877.

Scrotal scintigraphy has become a relatively uncommon examination in many nuclear medicine departments. However, scrotal perfusion is visualized during three-phase bone imaging of the pelvis and hips. The authors present a compilation of three cases of hip pain secondary to unsuspected scrotal pathology documented on routine three-phase bone scintigraphy, discussing this topic for the first time in the literature. A short review of the pathophysiology of referred hip pain, differential diagnosis, and imaging alternatives emphasize the unique contribution of nuclear imaging.

Subacute thyroiditis is an inflammatory condition of the thyroid gland in which pain may be referred to the jaws. The clinical features of the condition are outlined and emphasized through an illustrative case report. Clinicians should include this disease in the differential diagnosis of facial pain. It is important that, in an area so fraught with emotional overlay, a precise and careful history be taken from all patients and that these data be married to a concise classification of potential causes.

Chronic obstructive pulmonary disease (COPD) and cardiovascular disease (CVD) frequently coexist, increasing the prevalence of both entities and impacting on symptoms and prognosis. CVD should be suspected in patients with COPD who have high/very high risk scores on validated scales, frequent exacerbations, precordial pain, disproportionate dyspnea, or palpitations. They should be referred to cardiology if they have palpitations of unknown cause or angina pain. COPD should be suspected in patients with CVD if they have recurrent bronchitis, cough and expectoration, or disproportionate dyspnea.

Bilateral bipartite lunate is a very rare congenital anomaly of the lunate. A 36-year-old military European male was referred to our service diagnosed with a lunate fracture. Symptoms began 3 months before our encounter, after falling on his outstretched left hand. The patient was misdiagnosed with a lunate fracture, therefore treated with a cast and then transitioned to a removable splint over 2 months in total; When the patient presented to our facility, on physical examination, he referred pain over the dorso-ulnar side of the wrist, especially the ulnar snuff. Tenderness to palpation over …

BACKGROUND: Little information exists on highly anxious patients referred for periodontal therapy.
METHODS: All patients referred for periodontal therapy between November 1, 2002 and April 1, 2004 were included in this study. Patients who perceived themselves as extremely anxious were interviewed further, and their anxiety levels were observed during the various stages of periodontal therapy.
RESULTS: Twenty-one (19 females and two males, average age, 49.76 years) of 176 patients (12.1%) reported extreme anticipatory anxiety. The main reasons for their anxiety were fear of pain and previous
