Carotid brainstem reflex myoclonus after hypoxic brain damage.

A patient comatose after acute anoxia developed bilaterally synchronous, periodic myoclonic jerks most prominently in the bilateral upper limbs. Although the myoclonus seemed to occur spontaneously, electrophysiological studies showed that the myoclonic jerks correlated in timing and size with arterial pulses, and was suppressed by massage over the carotid sinus. It is proposed that the present myoclonus is a variant of brainstem reflex myoclonus in which arterial pulses served as intrinsic trigger stimuli via the carotid sinus and the medullary reticular formation.

Controversies surrounding reflex sympathetic dystrophy: a review article.

The topic of reflex sympathetic dystrophy (RSD) has generated an increasingly significant volume of medical literature and controversy over the last decade. A search of PubMed, the online site of the National Library of Medicine, for papers on RSD reveals nearly 2200 articles on the topic (using algodystrophy as the search word, wherein RSD references are also included, and more older and European articles are also listed). From 1991 through 1998 inclusive there is an average of nearly 100 articles per year on the topic, which represents more than a third of all the articles referenced since 1…

Monitoring of head injury by myotatic reflex evaluation.

OBJECTIVES: (1) To establish the feasibility of myotatic reflex measurement in patients with head injury. (2) To test the hypothesis that cerebral dysfunction after head injury causes myotatic reflex abnormalities through disordered descending control. These objectives arise from a proposal to use reflex measurements in monitoring patients with head injury. METHODS: The phasic stretch reflex of biceps brachii was elicited by a servo-positioned tendon hammer. Antagonist inhibition was evoked by vibration to the triceps. Using surface EMG, the amplitude of the unconditioned biceps reflex and pe…

Safety and clinical performance of acoustic reflex tests.

OBJECTIVE: Safety and effectiveness of acoustic reflex tests are important issues because these tests are widely applied to screen for retrocochlear pathology. Previous studies have reported moderately high sensitivity and specificity for detection of acoustic neuroma. However, there have been reports of possible iatrogenic hearing loss resulting from acoustic reflex threshold (ART) and decay (ARD) tests. This study assessed safety and clinical performance of ART tests for detection of acoustic neuroma. DESIGN: We report a case in which ARD testing resulted in a significant bilateral permanen…

Sensorimotor transformation in cat nociceptive withdrawal reflex system.

The withdrawal reflex system of higher vertebrates has been extensively used as a model for spinal sensorimotor integration, nociceptive processing and plasticity. In the rat, the nociceptive withdrawal reflex system appears to have a modular organization. Each reflex module controls a single muscle or a few synergistic muscles, and its cutaneous receptive field corresponds to the skin area withdrawn upon contraction of the effector muscle(s) when the limb is in the standing position. This organization principle is at odds with the 'flexion reflex' concept postulated from cat studies. To asses…

Reflex sympathetic dystrophy associated with amyotrophic lateral sclerosis.

Reflex sympathetic dystrophy (RSD) is a syndrome characterised by severe distal pain and vasomotor changes. It is believed to be caused by sympathetic nervous system overactivity. Trauma is the most frequent precipitant event. An association with amyotrophic lateral sclerosis (ALS) has been reported only once. We report three patients with ALS in whom the occurrence of RSD, in one of them at a very early clinical stage, seemed to have precipitated a more rapid clinical evolution. New sprouting re-innervating fibres have abnormal ion channels which might increase the risk of RSD. On the other h…

Soleus H-reflex to S1 nerve root stimulation.

H-reflexes in normals were elicited by percutaneous electrical and magnetic stimulation of proximal nerve roots at the cauda equina. H-M interval to S1 nerve root stimulation at the level of the S1 foramen was 6.8 +/- 0.33 ms, with side to side difference of 0.16 +/- 0.13 ms. Compression/ischemia of the sciatic nerve in the mid-thigh abolished the H-reflex to stimulation of the tibial nerve at the popliteal fossa when the H-reflex to S1 nerve root stimulation was preserved. The length of the S1 nerve root in human cadavers was measured to be 17.5 +/- 03 cm, providing an estimated dorsal root c…

Blink reflex in migraine and tension-type headache.

The blink reflex was studied in 19 patients with migraine, 10 patients with tension-type headache, and 30 healthy controls. Significantly lower values of R2 and R2' amplitude and size were found in the migraine group, compared with the healthy control group. The differences were independent of the stimulation side (headache or nonheadache) and highly significant (P<0.001). The abnormalities of R2 and R2' amplitude and size were found only during the headache phase of migraine, being normal between migraine attacks. R1 latency and amplitude were normal in all patients. The blink reflex was norm…

Masseter reflex potentials in olivo-ponto-cerebellar atrophy.

We recorded masseter reflex potentials to examine the correlation between the masseter reflex and the muscle stretch reflexes of limbs in 19 patients with olivo-ponto-cerebellar atrophy (OPCA). The patients were subdivided into hyper- (n = 5), normo- (n = 7) and hypo- (n = 7) reflexia groups according to the degrees of the conventional deep tendon jerks in the upper limbs. The masseter reflex potentials, elicited by tapping the chin with a reflex hammer, were recorded from the bilateral masseters using a pair of surface electrodes. The latency of the potentials in the hyporeflexia was signific…

Reflex sympathetic dystrophy in childhood: a case report.

A case report is presented of a 15-year-old girl with reflex sympathetic dystrophy (RSD). She was referred to hospital because of left upper limb pain. Her left upper limb was cold, edematous and blue with a limited active range of movement. The serum concentration of noradrenaline was lower on the painful side than on the healthy side, and neurotropin, which has an antinociceptive effect to hyperalgesia, was clearly effective. Early diagnosis and management is essential in the treatment of RSD and administration of neurotropin is a useful and non-invasive treatment without severe adverse effe…

Does peri-operative guanethidine prevent reflex sympathetic dystrophy?

The use of intravenous guanethidine blocks is an accepted treatment for established reflex sympathetic dystrophy (RSD). Some units administer intravenous guanethidine peri-operatively with the intention of protecting their patients from post-operative dystrophy. There have been no studies confirming this protective effect of peri-operative guanethidine. Between 1992 and 1994 we performed a prospective randomized double blind study in 71 patients undergoing fasciectomy for Duputyren's disease. Peri-operative guanethidine did not prevent post-operative RSD in our series.

Reflex sympathetic dystrophy: early treatment and psychological aspects.

We report the results of two prospective studies of early treatment and psychological aspects in a series of 26 patients with sympathetic reflex dystrophy of the hand in which treatment was started within 3 months after diagnosis. Ismelin blocks is an often used therapy in sympathetic reflex dystrophy but a probable better therapy in the first stage of the dystrophy was also investigated. Thirteen patients were treated with Regional Intravenous Ismelin blocks and 13 other patients were treated with a hydroxyl radical scavenger, dimethylsulfoxide (DMSO). After 9 weeks there was a better result …

Midbrain 6-hydroxydopamine lesions modulate blink reflex excitability.

The blink reflex abnormalities present in the 6 hydroxydopamine (6-OHDA) lesioned rat model of parkinsonism mimicked those of the human with Parkinson's disease. In alert rats, we monitored the long and short latency components of the orbicularis oculi electromyographic (OOemg) response evoked by electrical stimulation of the supraorbital branch of the trigeminal nerve (SO). Two paradigms, habituation and double pulse, provided a measure of blink reflex excitability. In normal rats, repeated stimulation of the SO produced habituation of the R2 component of the blink. In the double pulse paradi…

[Stapedial reflex test for gerontal sensorineural hearing loss].

The purpose of this investigation was to find a method that can diagnose sensorineural hearing loss early. Fifty subjects, aged more than 60 years, with hearing level ranged from normal to profound deafness, were examined. The noise-tone difference (NTD) were calculated at 0.5, 1, 2, and 4kHz from these data. The results indicated that NTD at 1kHz was more sensitive than others for diagnosing gerontal sensorineural hearing loss in early stage.

Reflex blepharospasm associated with bilateral basal ganglia lesion.

A patient with a bilateral striatal lesion secondary to anoxia presented reflex blepharospasm associated with parkinsonism and dystonia in the limbs. The blink reflex excitability curve was enhanced and the R-2 response prolonged as in patients with essential blepharospasm. The findings in this patient support the notion that blepharospasm may be secondary to basal ganglia dysfunction through abnormal facilitation of reticular formation neurons controlling facial nucleus motoneuron excitability.

Reflex sympathetic dystrophy after a minor electric shock.

A 37-year-old right-handed female sustained a minor electric shock to her right hand while changing a bulb in a Kodak Reliant Intelligent Microfilmer 2000. She noted a progressive onset of parasthesias of her right hand that became associated with throbbing pain at rest, as well as swelling and profound weakness of her hand during the next two months. Plethysmography demonstrated arterial vasoconstriction of her right hand. Her reflex sympathetic dystrophy was successfully treated with cervicothoracic sympathetic blocks.
