Intrathecal baclofen in current neuromodulatory practice: established indications and emerging applications.

Intrathecal baclofen (ITB) has evolved into a standard treatment for severe spasticity of both spinal and cerebral origin. The accumulated promising data from reported series of patients receiving ITB therapy together with the fact that spastic hypertonia commonly coexists with other neurological disorders have constituted a solid basis for offering this kind of treatment to patients suffering from other movement disorders. These include motor disorders such as dystonia, amyotrophic lateral sclerosis, status dystonicus, Hallervorden-Spatz disease, Freidreich's ataxia, "stiff-man" syndrome, but

Belonephobia--a fear of needles.

Belonephobia is an unreasonable and altered response due to the fear of needles. It affects up to 10% of the population and has implications for treatment and follow up, especially in the paediatric setting (see Case study). A three step behavioural approach involving recognition and relaxation, control and preparation, and graded exposure, can be effective in overcoming belonephobia. This will assist with non-urgent minor procedures being undertaken.

Relaxation treatment of adolescent headache sufferers: results from a school-based replication series.

BACKGROUND: In recent reviews of psychological and drug treatment, relaxation training approaches have been found to be efficacious for children and adolescents suffering from recurrent tension-type headache (TTH), while biofeedback procedures provide effective help for migraine headache sufferers, primarily treated in tertiary clinics. OBJECTIVE: In a school-based replication series, the effectiveness and efficiency of relaxation training provided within school settings were examined, in addition to the effects on various headache features and the maintenance of treatment gains at a 6 to 10-

Is there an association between water baths during labor and the development of chorioamnionitis or endometritis?

OBJECTIVE: Our purpose was to determine whether water baths during labor are associated with the development of chorioamnionitis or endometritis. STUDY DESIGN: One hundred ten cases of chorioamnionitis or endometritis and 97 controls were identified among singleton term gestations from the University of California at San Francisco perinatal database. Medical record review and patient interviews provided information on membrane rupture, history of bathing during labor, and relaxation and pain relief perceived by patients who experienced water bathing during labor. RESULTS: There was no statis

A school-based, nurse-administered relaxation training for children with chronic tension-type headache.

Compared the efficacy of a school-based, nurse-administered relaxation training intervention to a no-treatment control condition for children (10-15 years old) with chronic tension-type headache and the outcome at posttreatment and a 6-month follow-up. The study was conducted in a controlled between-group design including 26 schoolchildren who were randomly assigned to the two treatment conditions. Results showed that headache activity in the children treated with relaxation training was significantly more reduced than among those in the no-treatment control group at posttreatment as well as t

[Clinical features of spastic dysphonia].

Clinical, neurological, endoscopic, psychological findings, questionnaire data on vegetative sphere, diaphragm x-ray, articulation test and Viene test system evidence obtained on 25 patients with phonic spasm confirm organic neurological nature of spastic dysphonia as focal muscular dystonia. This condition can be accompanied with tremor, rotatory, winking and writers' spasms, oromandibular dystonia. As indicated by positive treatment outcomes, combined treatment of phonic spasm with GABA-ergic drugs of clonazepam (antelepsin) and baclofen, orthophonic voice correction, physiotherapy is pathog

Fibromyalgia in children; diagnosis and treatment.

Fifteen children (16 years and younger, 10 females, 5 males, mean age 13 years) with juvenile primary fibromyalgia syndrome (JPFS) were seen in a private rheumatology practice over two years. This represented 45 percent of the total number of pediatric rheumatology patients. Symptoms included polymyalgias, polyarthralgias, nonrestorative sleep, difficulty concentrating in school and fatigue. Examination revealed typical tender points, absence of joint swelling, synovitis or nodules and absence of neurological findings. Dolorimetry was abnormal and standard laboratory tests were normal. Most of

[The "Ashkelon" hypertension detection and follow-up program].

The program described aims at enhancing awareness of high blood pressure and other risk factors for cardiovascular disease (CVD) and promoting a healthy lifestyle. The study population consisted of 11,000 workers at worksites and visitors to community centers, aged 25-65 years. The program was carried out in cooperation with primary care clinics and non-pharmacological control of high blood pressure was by a community center for prevention of CVD. In 1980, 56.0% of hypertensives were unaware of their condition, while by 1988 this was reduced to 6.1%. Body weight reduction resulted in lowering

Effect of Benson relaxation response technique on the quality of life among patients with systemic lupus erythematous: quasi-experimental study.

OBJECTIVES: To investigate the effect of Benson relaxation response technique (BRRT) on the quality of life (QOL) among patients with systemic lupus erythematous (SLE). METHODOLOGY: A quasi-experimental design was used to conveniently recruit 170 patients with SLE. Participants were divided into two groups, the control and the intervention group for which the BRRT intervention was administered. Utilising an online questionnaire, the QOL was assessed among the two groups, before and 2 months after the intervention, using the Arabic version of the short form 36-item health survey. RESULTS: Aft

Enhancing the Efficacy of Cognitive Bias Modification for Social Anxiety.

Cognitive bias modification for interpretation (CBM-I) is a promising treatment modality for social anxiety disorder, but effect sizes are relatively small across investigations (Hallion & Ruscio, 2011). Additionally, the extent to which CBM-I impacts other cognitive, emotional, and behavioral outcomes is unclear. This study investigated whether two empirically supported treatment components for anxiety disorders, imaginal exposure (IE) and relaxation, augment the effects of CBM-I and increase the extent to which the effects of CBM-I generalize to behavioral and affective outcomes. We randomly

Effects of group relaxation training and thermal biofeedback on blood pressure and related physiological and psychological variables in essential hypertension.

One hundred and one patients, 70 experimental and 31 controls, with a diagnosis of essential hypertension, were examined for the effects of group relaxation training and thermal biofeedback on blood pressure and on other psychophysiologic measures: heart rate, forehead muscle tension, finger temperature, depression, anxiety, plasma aldosterone, plasma renin activity, and plasma and urinary cortisol. Eighty percent of the participants were medicated. Treatment yielded a short-term success rate, defined as a decrease in mean arterial pressure of 5 mm Hg, of 49% in the experimental group. Other s

Anodyne imagery: an alternative to i.v. sedation in interventional radiology.

OBJECTIVE: Pain and anxiety are to be expected in patients undergoing interventional procedures, and they are usually treated by IV conscious sedation. Insufficient treatment of pain and anxiety can cause cardiovascular strain and restlessness, which may jeopardize the success of the procedure. On the other hand, pharmacologic oversedation can provoke respiratory and cardiovascular depression, thereby increasing the procedural risks and delaying the patient's recovery. We therefore evaluated a nonpharmacologic method, which we call anodyne imagery (anodyne: able to soothe or relieve pain; soot

Medical therapy of low back pain.

Low back pain is a common clinical symptom usually associated with mechanical disorders, primarily muscle strain, affecting the lumbosacral spine. The majority of patients with low back pain improve within a 2-month period with conservative medical management. Conservative management includes limited physical activity, injections, nonsteroidal anti-inflammatory drugs, and muscle relaxants. These therapies work best in combination and are very effective in decreasing low back pain.

[Psychosomatic treatment of irritable bowel syndrome].

Irritable bowel syndrome (IBS) may be defined as a psychosomatic disease. Most primary care physicians do not want to undertake psychosomatic treatment, but may find it necessary in refractory patients. Brief psychosomatic treatments, providing patients with betterways to cope with stress, reduce the symptoms. The literature on the psychosomatic treatment for IBS is summarized. Supportive therapy and autogenic training that produce relaxation response appear to be effective. Other special psychological therapies that require specialized training, such as hypnosis, biofeedback, cognitive therap

Effects of relaxation and mnemonics on memory, attention and anxiety in the elderly.

We measured attentional capacity as 25 normal elderly subjects were trained in techniques to improve face-name recall. Techniques consisted of relaxation training and a mnemonic device. Anxiety was measured simultaneously with attentional measures. We hypothesized that our techniques would increase available memory processing capacity by improving attention and reducing anxious rumination. Results indicated that subjects showing the greatest reduction in anxiety and cognitive interference and the greatest increase in attention also showed the most face-name recall following training. The resul

Ventilatory response to CO2. II. Studies in neurotic psychiatric patients and practitioners of transcendental meditation.

The prominence of respiratory symptoms in patients with neurotic psychiatric disorders is noted and the literature on the control of respiration is reviewed to attempt to explain this finding. A previous study demonstrated a positive correlation between the ventilatory response to CO2 (S) and neurotic personality traits in a group of normal subjects. This study attempted to follow up this finding and hypothesized that a group of neurotically disturbed patients would have a higher S value and a group of individuals who practiced a calming technique such as transcendental meditation (TM) would h
