Inducible nonhuman primate models of retinal degeneration for testing end-stage therapies.

The anatomical differences between the retinas of humans and most animal models pose a challenge for testing novel therapies. Nonhuman primate (NHP) retina is anatomically closest to the human retina. However, there is a lack of relevant NHP models of retinal degeneration (RD) suitable for preclinical studies. To address this unmet need, we generated three distinct inducible cynomolgus macaque models of RD. We developed two genetically targeted strategies using optogenetics and CRISPR-Cas9 to ablate rods and mimic rod-cone dystrophy. In addition, we created an acute model by physical separatio

The Value of Electroretinography in Identifying Candidate Genes for Inherited Retinal Dystrophies: A Diagnostic Guide.

Inherited retinal dystrophies (IRDs) are a group of heterogeneous diseases caused by genetic mutations that specifically affect the function of the rod, cone, or bipolar cells in the retina. Electroretinography (ERG) is a diagnostic tool that measures the electrical activity of the retina in response to light stimuli, and it can help to determine the function of these cells. A normal ERG response consists of two waves, the a-wave and the b-wave, which reflect the activity of the photoreceptor cells and the bipolar and Muller cells, respectively. Despite the growing availability of next-generat

Impaired glutamylation of RPGRORF15 underlies the cone-dominated phenotype associated with truncating distal ORF15 variants.

Pathogenic variants in the Retinitis pigmentosa GTPase regulator (RPGR) gene lead to a clinically severe form of X-linked retinal dystrophy. However, it remains unclear why some variants cause a predominant rod, while others result in a cone-dominated phenotype. Post-translational glutamylation of the photoreceptor-specific RPGRORF15 isoform by the TTLL5 enzyme is essential for its optimal function in photoreceptors, and loss of TTLL5 leads to retinal dystrophy with a cone phenotype. Here we show that RPGR retinal disease, studied in a single cohort of 116 male patients, leads to a clear progr

Wavelet-Based Pattern ERG Biomarkers Outperform Temporal Amplitude Measures for Functional Stratification in Optic Nerve Disease.

PURPOSE: To extend wavelet analysis of pattern electroretinography (PERG) from macular cone to retinal ganglion cell (RGC) dysfunction in optic nerve disease (OND) by validating Symlet-2 (sym2) discrete wavelet transform (DWT) features. METHODS: From the open access PERG-Institute of Applied Ophthalmobiology (IOBA) dataset, 58 recordings from OND subjects and 262 recordings from healthy volunteers (HVs) were analyzed. Five pre-selected sym2 coefficients (D5-2, D6-2, D6-3, A6-3, A6-4) were quantified. Their correlations with canonical amplitudes (|P50-N35|, |N95-P50|) and group separation (ran

Selective amplification of ipRGC signals accounts for interictal photophobia in migraine.

Second only to headache, photophobia is the most debilitating symptom reported by people with migraine. While the melanopsin-containing intrinsically photosensitive retinal ganglion cells (ipRGCs) are thought to play a role, how cone and melanopsin signals are integrated in this pathway to produce visual discomfort is poorly understood. We studied 60 people: 20 without headache and 20 each with interictal photophobia from migraine with or without visual aura. Participants viewed pulses of spectral change that selectively targeted melanopsin, the cones, or both and rated the degree of visual di

Melanopsin supports irradiance-driven changes in maintained activity in the superior colliculus of the mouse.

Melanopsin phototransduction allows intrinsically photosensitive retinal ganglion cells (ipRGCs) to maintain firing under sustained illumination and to encode irradiance. ipRGCs project to different parts of the visual system, including the superficial superior colliculus (sSC), but to date there is no description of melanopsin contributions to the activity of that nucleus. We sought to fill that gap using extracellular recordings to describe light response in the sSC. We failed to observe light responses in the sSC of mice lacking rod and cone function, in which melanopsin provides the only p

Electroretinographic (ERG) responses in pediatric patients using vigabatrin.

The antiepileptic drug vigabatrin is known to cause retinal and visual dysfunction, particularly visual field defects, in some patients. Electroretinography (ERG) is used in an attempt to identify adverse effects of vigabatrin (VGB) in patients who are not candidates for conventional perimetry. We report data from 114 pediatric patients taking VGB referred for clinical evaluation; median age at test was 22.9 (2.4 to 266.1) months, and median duration of VGB use was 9.7 (0.3 to 140.7) months. Twenty-seven of them were tested longitudinally (3 to 12 ERG tests). ERG responses to full-field stimul

Clinical heterogeneity between two Japanese siblings with congenital achromatopsia.

Congenital achromatopsia is a stationary retinal disorder with autosomal recessive inheritance. It is characterized by significant attenuation of cone-photoreceptor function. Symptoms include photophobia, nystagmus, and poor visual acuity from birth. Unlike in cone or cone-rod dystrophies, the retinal fundus usually appears normal. Here we describe two siblings with congenital achromatopsia, who exhibit different ophthalmic phenotypes. History was taken, and ophthalmic examinations were performed in a 7-year-old girl and her 5-year-old brother, who were referred to our department because of po

Human immunodeficiency virus-associated vision loss: electroretinogram attenuation.

PURPOSE: To report a 35-year-old man with human immunodeficiency virus (HIV) and bilateral progressive decrease in vision thought to be caused by HIV optic neuropathy but associated with a severe attenuation of the electroretinogram. METHODS: Case report, review of laboratory studies, visual fields, and electroretinogram. RESULTS: Visual function deteriorated in an asymmetric fashion over 9 months of follow-up and continued to deteriorate, even when the patient had no detectable viral load. No evidence of cytomegalovirus retinitis or HIV retinopathy was present. An electroretinogram showed s

Exonic splice variant discovery using in vitro models of inherited retinal disease.

Correct identification of the molecular consequences of pathogenic genetic variants is essential to the development of allele-specific therapies. However, such molecular effects may remain ambiguous following genetic sequence analysis alone. Here, we identify exonic codon-altering variants that are also predicted to disrupt normal RNA splicing in the context of inherited retinal disease. NR2E3 c.932G>A (p.Arg311Gln) is a variant commonly associated with enhanced S cone syndrome. Previous studies using mutagenized cDNA constructs have shown that the arginine to glutamine substitution at positio

The Oxidative-Mitochondrial-Inflammatory Axis in Retinitis Pigmentosa: Extracellular mtDNA as Biomarker and Therapeutic Read-Out.

Retinitis pigmentosa (RP) is the most common inherited retinal dystrophy (prevalence ~1:4000) and a leading Mendelian cause of working-age blindness. Despite marked genetic heterogeneity, its progression converges on a common secondary cascade of outer-retinal hyperoxia, increased reactive oxygen species (ROS), and mitochondrial dysfunction that drives cone degeneration and central vision loss. Because this oxidative cascade is largely genotype-independent and pharmacologically tractable, oxidative stress is a cross-cutting therapeutic target. Within it, mitochondrial DNA (mtDNA) is a key elem

Roles of the nonvisual opsins in the mammalian retina.

In addition to the rods and cones that subserve vision, several classes of mammalian retinal ganglion cells are also directly photoreceptive. These "intrinsically photosensitive" retinal ganglion cells ("ipRGCs") utilize the nonvisual photopigments melanopsin (Opn4), neuropsin (Opn5), and encephalopsin (Opn3), which are expressed in largely nonoverlapping ganglion cell populations. Remarkably, all three nonvisual opsins appear to influence the circadian clock, but in different ways. Opn4-expressing cells project from the retina to brain regions regulating circadian rhythms, such as the suprach

Luminance invariant encoding in mouse primary visual cortex.

The visual system adapts to maintain sensitivity and selectivity over a large range of luminance intensities. One way that the retina maintains sensitivity across night and day is by switching between rod and cone photoreceptors, which alters the receptive fields and interneuronal correlations of retinal ganglion cells (RGCs). While these adaptations allow the retina to transmit visual information to the brain across environmental conditions, the code used for that transmission varies. To determine how downstream targets encode visual scenes across light levels, we measured the effects of lumi

Inner Retina Changes in ABCA4-Related Retinopathy: Structure-Function Relationship.

PURPOSE: The aim of this retrospective study was to evaluate the inner retinal changes in patients with ABCA4-related retinal dystrophy (ABCA4-RD), and correlate these changes with the photopic negative response (PhNR), a cone ERG component reflecting inner retinal activity, previously reported as altered in ABCA4-RD. METHODS: Nineteen ABCA4-RD patients (21 eyes) and 20 age-matched controls (20 eyes) were included in the study. Non-Image-Forming Functions of Intrinsically Photosensitive Retinal Ganglion Cells.

Life on this planet is heavily influenced by light, the most critical external environmental factor. Mammals perceive environmental light mainly through three types of photoreceptors in the retina-rods, cones, and intrinsically photosensitive retinal ganglion cells (ipRGCs). The latest discovered ipRGCs are particularly sensitive to short-wavelength light and have a unique phototransduction mechanism, compared with rods and cones. Piles of evidence suggest that ipRGCs mediate a series of light-regulated physiological functions such as circadian rhythms, sleep, metabolic homeostasis, mood, deve

Redefining the role of Ca2+-permeable channels in photoreceptor degeneration using diltiazem.

Hereditary degeneration of photoreceptors has been linked to over-activation of Ca2+-permeable channels, excessive Ca2+-influx, and downstream activation of Ca2+-dependent calpain-type proteases. Unfortunately, after more than 20 years of pertinent research, unequivocal evidence proving significant and reproducible photoreceptor protection with Ca2+-channel blockers is still lacking. Here, we show that both D- and L-cis enantiomers of the anti-hypertensive drug diltiazem were very effective at blocking photoreceptor Ca2+-influx, most probably by blocking the pore of Ca2+-permeable channels. A circuit motif for color in the human foveal retina.

The neural pathways that start human color vision begin in the complex synaptic network of the foveal retina where signals originating in long (L), middle (M), and short (S) wavelength-sensitive cone photoreceptor types are compared through antagonistic interactions, referred to as opponency. In nonhuman primates, two cone opponent pathways are well established: an L vs. M cone circuit linked to the midget ganglion cell type, often called the red-green pathway, and an S vs. L + M cone circuit linked to the small bistratified ganglion cell type, often called the blue-yellow pathway. These pathw
