Pattern of vitreo-retinal diseases at the national referral hospital in Bhutan: a retrospective, hospital-based study.

BACKGROUND: Knowing the pattern and presentation of the diseases is critical for management strategies. To inform eye-care policy we quantified the pattern of vitreo-retinal (VR) diseases presenting at the national referral hospital in Bhutan. METHODS: We reviewed all new patients over three years from the retinal clinic of the Jigme Dorji Wangchuck National Referral Hospital. Demographic data, presenting complaints and duration, treatment history, associated systemic diseases, diagnostic procedures performed, and final diagnoses were quantified. Comparisons of the expected and observed frequ

Novel Therapeutic Approaches for the Treatment of Retinal Degenerative Diseases: Focus on CRISPR/Cas-Based Gene Editing.

Inherited retinal diseases encompass a highly heterogenous group of disorders caused by a wide range of genetic variants and with diverse clinical symptoms that converge in the common trait of retinal degeneration. Indeed, mutations in over 270 genes have been associated with some form of retinal degenerative phenotype. Given the immune privileged status of the eye, cell replacement and gene augmentation therapies have been envisioned. While some of these approaches, such as delivery of genes through recombinant adeno-associated viral vectors, have been successfully tested in clinical trials, …

Application of CRISPR/Cas9 technologies combined with iPSCs in the study and treatment of retinal degenerative diseases.

Retinal degeneration diseases, such as age-related macular degeneration and retinitis pigmentosa, affect millions of people worldwide and are major causes of irreversible blindness. Effective treatments for retinal degeneration, including drug therapy, gene augmentation or transplantation approaches, have been widely investigated. Nevertheless, more research should be dedicated to therapeutic methods to improve future clinical treatments. Recently, with the rapid development of genome-editing technology, gene therapy has become a potentially effective treatment for retinal degeneration disease

The role of the multifocal visual evoked potential (mfVEP) latency in understanding optic nerve and retinal diseases.

PURPOSE: To examine the extent to which changes in the latency of the multifocal visual evoked potential (mfVEP) overlap in patients with glaucoma, recovered optic neuritis/multiple sclerosis (ON/MS), and retinal disease. METHODS: Monocular mfVEPs were obtained for both eyes of all subjects. Latencies and amplitudes of individual mfVEP responses were measured using custom software and expressed relative to a normative group (n = 100). Recordings were obtained from patients with ON/MS (n = 12), glaucoma (n = 50), and retinal disease (n = 15), as well as control subjects (n = 50). All subjects

Real-World Engagement with Free Genotyping by Patients with Inherited Retinal Disease in a Single Private Retinal Centre in Australia.

PURPOSE: Genotyping patients with inherited retinal diseases (IRD) is important for diagnosis, genetic counselling, and future treatments. However, in Australia less than 10% of patients with IRD have undergone genetic testing. Recognised barriers to testing include cost, access, and trust in the treating clinician. We aimed to assess uptake and outcomes to an offer of sponsored (free) genetic testing with home DNA collection, thereby removing typical barriers to genetic testing. METHODS: Participation in a sponsored genetic testing program (Invitae Corporation) was offered to adults from a p…

Retinal Ganglion Cells and Circadian Rhythms in Alzheimer's Disease, Parkinson's Disease, and Beyond.

There is increasing awareness on the role played by circadian rhythm abnormalities in neurodegenerative disorders such as Alzheimer's disease (AD) and Parkinson's disease (PD). The characterization of the circadian dysfunction parallels the mounting evidence that the hallmarks of neurodegeneration also affect the retina and frequently lead to loss of retinal ganglion cells (RGCs) and to different degrees of optic neuropathy. In the RGC population, there is the subgroup of cells intrinsically photosensitive and expressing the photopigment melanopsin [melanopsin-containing retinal ganglion cells

Should ApoE4 gene editing therapy be approached with caution when AD is accompanied by retinal neurodegenerative diseases?

Apolipoprotein E4 (ApoE4) is the strongest genetic risk factor for Alzheimer's disease (AD), and ApoE4-directed gene editing and gene therapy have emerged as promising therapeutic strategies for AD, although they remain investigational and are not yet an established mainstream treatment. Interestingly, however, ApoE4 is reported to be associated with a reduced risk of retinal degenerative diseases such as glaucoma, age-related macular degeneration (AMD), and diabetic retinopathy (DR). Therefore, should ApoE4 gene editing therapy be approached with caution when AD occurs concurrently with glauc

Assessment of In-Frame Indel Variants in an Unsolved Cohort of Inherited Retinal Diseases Using Machine Learning.

The standard for in silico pathogenicity prediction of in-frame insertions and deletions (indels) is less established compared to other types of variations. We aimed to systematically assess the performance of in silico machine learning (ML) tools on a patient cohort with inherited retinal diseases (IRDs). The performance of four ML tools (CADD, FATHMM-indel, VEST4, and MetaRNN-indel) was compared. Among them, MetaRNN-indel showed the best overall results. MetaRNN-indel was then applied to 1013 unsolved IRD patients, identifying two likely pathogenic causal variants in two unrelated IRD patien…

Toward Efficient Identification of Retinal Diseases: A Lightweight Convolutional Neural Network-Based Approach Using Optical Coherence Tomography.

Retinal diseases are a major cause of both temporary and permanent vision loss, making early detection and treatment essential to prevent irreversible damage, as the retina's ability to transmit light signals to the brain can be compromised. While deep learning has shown strong potential in diagnosing such conditions using optical coherence tomography (OCT) images, many existing models are highly complex and computationally intensive, making them impractical for deployment on edge devices in clinical settings. To address this issue, we propose a diagnostic framework based on a lightweight deep…

A review of the 661W cell line as a tool to facilitate treatment development for retinal diseases.

Retinal diseases encompass a diverse group of disorders that affect the structure and function of the retina, leading to visual impairment and, in some cases, irreversible vision loss. The investigation of retinal diseases is crucial for understanding their underlying mechanisms, identifying potential therapeutic targets, and developing effective treatments. The use of in vitro cell models has become instrumental in advancing our knowledge of these disorders, but given that these conditions usually affect retinal neuronal cell types, access to appropriate cell models can be potentially challen…

Artificial intelligence-driven metabolomics of retinal nerve fibre layer to profile risks of mortality and cardiometabolic diseases.

Retinal nerve fibre layer (RNFL) is a non-invasive structural biomarker of cardiometabolic health, yet its biological underpinnings remain unknown. Here, we integrate advanced retinal optical biopsy and artificial intelligence (AI) algorithms with two complementary metabolomic assays across ethnically diverse cohorts to elucidate the metabolic basis underlying RNFL degeneration and its link to cardiometabolic disease (CMD) in Western cohort and Eastern cohort (Guangzhou Diabetic Eye Study, GDES). We identify 26 metabolic biomarkers significantly associated with RNFL thickness, most of which (r

EyeCNN: exploring the potential of convolutional neural networks for identification of multiple eye diseases through retinal imagery.

BACKGROUND: The eyes are the most important part of the human body as these are directly connected to the brain and help us perceive the imagery in daily life whereas, eye diseases are mostly ignored and underestimated until it is too late. Diagnosing eye disorders through manual diagnosis by the physician can be very costly and time taking. OBJECTIVE: Thus, to tackle this, a novel method namely EyeCNN is proposed for identifying eye diseases through retinal images using EfficientNet B3. METHODS: A dataset of retinal imagery of three diseases, i.e. Diabetic Retinopathy, Glaucoma, and Catarac
