[Vertical dimension in the OSAHS in adults: an example of functional alteration].

Obstructive sleep apnea and hypoapnea syndrome (OSAHS) is important to orthodontists for several reasons. The authors, after defining OSAHS and describing it briefly, review the literature dealing with the cranio-facial morphology of apnea patients. They report the results of their cephalometric study, which is based on an architectural analysis of 274 patients. Their facial characteristics, especially vertical dimension, are consistent with those displayed by mouth breathing children. Changes in respiratory functioning modify the mechanical behavior and the structures of the upper airways. Os

Relationship between formation/eruption of maxillary teeth and skeletal pattern of maxilla.

Previous reports have indicated that formation and eruption of the maxillary teeth, especially the molars, are delayed in skeletal Class III patients compared with Class I and Class II patients. However, the relationship between the formation/eruption of maxillary teeth and the skeletal pattern of the maxilla is not yet clear. To examine this relationship, we studied 81 skeletal Class III Japanese patients (mean age, 8.8 years). The sample was divided into 2 groups: a maxillary retrusion (MR) group, characterized by a small SNA angle and a short palatal length, and a control group in which tho

Altered orthognathic surgical sequencing and a modified approach to model surgery.

Advances in orthognathic surgical treatment planning and in techniques for complex, simultaneous maxillary and mandibular repositioning have resulted in improved surgical accuracy. In traditional surgical sequencing, maxillary surgery is performed first; the maxilla is set, with or without an intermediate splint, using external reference points to verify and/or determine appropriate movement. However, errors in model surgery and intermediate splint fabrication can lead to surgical inaccuracy despite good surgical technique. In repositioning the maxilla first, when thin bony walls are present,

Video imaging--a computer facilitated approach to communication and planning in orthognathic surgery.

A case report is presented in order to illustrate the benefits of computerized video imaging in orthognathic surgery. It is suggested that superimposition of the cephalogram on the video image is helpful in treatment planning and in communication with the patient. Computerized video imaging techniques help clinicians and patients discuss the aesthetic impact of orthognathic surgery. The superimposition of the cephalogram and profile image, coupled with algorithmic predictions, allow the clinician to plan the surgery to closely match the desired result. This paper presents the planning and coun

Comparative cephalometric analysis in patients with X-chromosome aneuploidy.

The influence of abnormal sex-chromosome constitution on cranial growth, shape and position of the craniofacial structures was analysed. Reduced cranial growth was found in women with Turner's syndrome (45 XO chromosome constitution) and in men with Klinefelter's syndrome (47 XXY chromosome constitution). Lack of or an extra X chromosome produced opposite effects on cranial-base flexion, jaw displacement and maxillary and mandibular inclination to the anterior cranial base. An extra X chromosome affected deviation in sagittal jaw relation while lack of an X chromosome influenced mandibular sha

Rare total and partial palatal clefts experimentally induced in the rat.

Clefts of the hard palate with soft palate integrity can be produced through amniocentesis in Sprague-Dawley rats. This study attempted to induce exotic partial clefts of the palate and to describe their type, frequency and possible genesis. Palatal closure, i.e., elevation and fusion of the anterior zone, remodelling of the middle zone, and fusion in the posterior zone, occurs in rats between 16.0 and 17.5 days of fetal age. Withdrawing a small amount of amniotic fluid produced a series of total and rare partial clefts. The upper jaws of 95 of 410 fetuses of precisely determined age were exam

Microdeletion del(22)(q12.2) encompassing the facial development-associated gene, MN1 (meningioma 1) in a child with Pierre-Robin sequence (including cleft palate) and neurofibromatosis 2 (NF2): a case report and review of the literature.

BACKGROUND: Pierre-Robin sequence (PRS) is defined by micro- and/or retrognathia, glossoptosis and cleft soft palate, either caused by deformational defect or part of a malformation syndrome. Neurofibromatosis type 2 (NF2) is an autosomal dominant syndrome caused by mutations in the NF2 gene on chromosome 22q12.2. NF2 is characterized by bilateral vestibular schwannomas, spinal cord schwannomas, meningiomas and ependymomas, and juvenile cataracts. To date, NF2 and PRS have not been described together in the same patient. CASE PRESENTATION: We report a female with PRS (micrognathia, cleft pala…

New hope for Treacher-Collins syndrome: a surgical case report.

The objective of this case report is to discuss the possibility of developing a surgical treatment paradigm for patients with some of the noted characteristics of Treacher-Collins syndrome, mainly mandibular ankylosis and/or retrognathia (mandibular retrusion), in a way that would provide lasting, predictable results and minimize the frequently seen mandibular ankylosis. Through various medical imaging techniques, mainly computed tomography (CT) scan, the radiology imaging technologist produces accurate CT scan images of the particular patient's osseous cranial structures. These images can the…

Implant-Prosthetic Rehabilitation of the Edentulous Mandible in a Patient With Class III Malocclusion and a Retrognathic Maxilla.

Over the past few years, innovations in computer-aided design/computer-aided manufacturing (CAD/CAM) processes have allowed for manufacturing bar-retained constructions made of titanium or a cobalt-chrome alloy to secure the position of overdentures. Here, the authors demonstrate the challenges of implant-prosthetic mandibular rehabilitation of a male 71-year-old patient with a reconstructed left-sided clefting congenital deformity and class III malocclusion with anterior crossbite and retrognathic maxilla. This case illustrates that the rehabilitation of an edentulous mandible with 4 intrafor

Le Fort I distraction osteogenesis of the edentulous maxilla.

Rehabilitation of the edentulous patient with atrophic ridges is a problem especially when compounded with a severe prognathic inter-arch relationship. It is difficult to rehabilitate these patients prosthetically without correction of the malrelation of the jaws. The established surgical techniques for correcting combined sagittal and vertical discrepancies of edentulous jaws are often prolonged and complex with attendant morbidity. This article presents a novel, simple method of correction of severe interarch sagittal discrepancy (more than 15 mm) by performing distraction osteogenesis at Le
