Idiopathic retroperitoneal fibrosis (IRF) is a disease difficult to diagnose. It is considered by different authors of autoimmune origin given its similarity to other connective tissue diseases. We present the case of a female patient diagnosed by a postmortem study, of IRF with multiorgan involvement (lymph nodes, serose tissue, liver, spleen, adrenal glands, thyroid and kidneys) and atypical presentation, not finding other similar cases in the national literature.

To evaluate the features on magnetic resonance (MR) images of malignant and nonmalignant retroperitoneal fibrosis (RPF) and to assess the usefulness of MR imaging in differentiating the two conditions, MR studies of nine patients with malignant RPF were retrospectively examined and compared with those of eight patients with nonmalignant RPF. Eleven patients (8 male) with idiopathic retroperitoneal fibrosis (IRPF) were reviewed. The mean age of the patients was 44 years. Five patients (group 1) who had moderate obstruction were treated only by steroids. Six patients (group 2) who presented with severe obstructive uropathy (and/or serious metabolic disturbances) were treated with a combination of surgery and steroid administration. Ureteral obstruction resulting from idiopathic retroperitoneal fibrosis (IRF) generally requires a surgical approach sometimes associated with the administration of a corticosteroid. Corticosteroids alone have also been used successfully for recurrences of IRF after the surgical procedure; however, their success is not uniform. Four patients with retroperitoneal fibrosis (Ormond's disease) were examined by computer tomography; the appearances are described and the differential diagnosis discussed. Computer tomography clearly defines the proliferative-inflammatory process and together with conventional radiology, angiography and lymphangiography it permits a presumptive diagnosis of retroperitoneal fibrosis. The non-invasive nature of computer tomography makes it the method of choice for follow-up of this disease.

A case of Actinomycosis israeli causing retroperitoneal fibrosis and ureteral obstruction is presented. This rare occurrence followed direct extension of the infectious process from intra-abdominal abscesses to the retroperitoneum, resulting in dense fibrosis. Diagnosis is most commonly established by histologic identification of sulfur granules obtained at abdominal exploration. The pathogenesis and treatment of genitourinary as well as retroperitoneal actinomycosis is discussed.
