A 50-year-old man with repeated episodes of right epigastric pain was seen in our hospital. An alternative method for bilateral spermatic vein ligation is presented. With a retroperitoneoscopic approach, the vascular structures are clearly identified, and damage to the intraperitoneal organs, spermatic arteries and lymphatics is minimized. We believe that the potential complications of transperitoneal laparoscopic techniques may be decreased with a retroperitoneoscopic approach.

A patient presenting with thigh and flank masses and back pain for 3 months proved to have actinomycosis involving the retroperitoneum and quadriceps muscle. Retroperitoneal involvement without intraperitoneal disease is rare. Computed tomography (CT), however, showed disease transgressing adjacent anatomic compartments with direct extension through the body wall and involvement of adjacent bony structures which is characteristic of actinomycosis.

A case of retroperitoneal xanthogranuloma is reported. A 51-year-old man was referred to our hospital for the evaluation and treatment of right flank pain and hydronephrosis. There are generated conditions for development of massive anaerobe infection which is in condition of communication off retroperitoneal colon, directly endangering life of the patient. From familiar anatomical characteristics of retroperitoneal part of colon and on account of massive anaerobe infect in retroperitoneum, all primary anastomoses and sutures are insecure and shouldn't be used. Application of exteriorization in cases of destruction of retroperitoneal colon is the securiest method.

Benign lymph node hyperplasia or Castleman's disease is an unusual entity, which rarely calcifies and is characteristically found near the hila of the lungs or in the mediastinum. In the case we have reported, a calcified mass in the left upper abdominal quadrant proved to be a rare case of abdominal Castleman's disease.

A case of extravesical malacoplakia, the first case in Japan, is described in detail. The patient was a 61-year-old woman with a right flank mass. Radiologically, the mass was thought to be of the renal origin. Surgically, however, the tumor was found attached not only to the cortical surface but extended to the retroperitoneum and psoas muscle. Pathological examination confirmed the lesion to be malacoplakia characterized by the presence of von Hansemann cells and Michaelis-Gutmann bodies.
