BACKGROUND: Acute pain due to rib fractures causes significant in-hospital morbidity and impacts patients' quality of life after discharge. Intraoperative transthoracic cryoneurolysis of the intercostal nerves can improve postoperative pain; however, non-surgical patients are provided limited analgesia options. Here, we describe our experience with a bedside cryoanalgesia technique for management of acute rib fracture pain.
METHODS: Five patients at a single level I trauma center completed bedside intercostal nerve cryoneurolysis (INC) using a handheld cryotherapy device and ultrasound guidan…

BACKGROUND: Sternum and rib fractures represent a significant global health concern, contributing to morbidity and disability on a worldwide scale. However, there is a notable lack of recent epidemiological data detailing the global and regional burden of these fractures.
METHODS: We undertook a systematic analysis of the prevalence of sternum and rib fractures at the global, regional, and national levels in 2019, while also examining time trends spanning from 1990 to 2019. To achieve this, we extracted data from the Global Burden of Disease Study 2019, enabling us to determine incidence, pre…

This case report is unique due to the rare combination of a history of no trauma and a history of scoliosis correction surgery with instrumentation performed 1 year before presentation. A 23-year-old male patient presented to the emergency department in a stable condition and he looked well. He was able to walk without any limitation, but complained of left upper back pain for 2 days after a sneezing episode. The patient denied history of chest pain, shortness of breath, cough, palpitation, headache, dizziness, nausea, vomiting, and abdominal pain. The chest X-ray showed left first rib fractur

INTRODUCTION: Concurrent psychiatric diagnoses adversely impact outcomes in surgical patients, but their relationship to patients with rib fracture after trauma is less understood. We hypothesized that psychiatric comorbidity would be associated with increases in hospital length of stay (LOS) and mortality risk after rib fracture.
MATERIALS AND METHODS: The 2017 National Inpatient Sample was queried for adult patients who were admitted with rib fracture after trauma. Mental health disorders were categorized into 34 psychiatric diagnosis groups (PDGs) using clinical classifications software re

RATIONALE: Degenerative spondylolisthesis is defined as forward slippage of a vertebra with respect to the underlying vertebra and is associated with the induction of lumbar canal stenosis. The use of anterior column support for degenerative lumbar conditions has been well documented. Direct lateral interbody fusion (DLIF) gains access via a lateral approach through the retroperitoneal fat and psoas muscle. It avoids many of the access-related complications yet comes with its own risks and limitations. The location of the iliac wing precludes exposure of the L5-S1 disc space and may make L4-5

This case highlights the probable association of significantly displaced posterior first-rib fracture and jagged edges of the fracture line following blunt chest trauma with delayed ipsilateral subclavian artery rupture. Early angiography and first-rib repair should promptly be considered under such circumstances.

OBJECT: Classic biomechanical models have used thoracic spines disarticulated from the rib cage, but the biomechanical influence of the rib cage on fracture biomechanics has not been investigated. The well-accepted construct for stabilizing midthoracic fractures is posterior instrumentation 3 levels above and 2 levels below the injury. Short-segment fixation failure in thoracolumbar burst fractures has led to kyphosis and implant failure when anterior column support is lacking. Whether shorter constructs are viable in the midthoracic spine is a point of controversy. The objective of this study

Trauma is the third most common cause of death in the West. In the US, approximately 90,000 deaths annually are traumatic in nature and over 75% of casualties from blunt trauma are due to chest injuries. Cardiac injuries from rib fractures following blunt trauma are extremely rare. We report the unusual case of a patient who fell from a height and presented with haemopericardium and haemothorax as a result of left ventricular and lingular lacerations and was successfully operated upon.

The reported patient was a 37-year-old male, who got a blunt chest trauma by a motor vehicle accident. Chest X-ray and computed tomography of the chest revealed bilateral multiple rib-fractures, pneumo-hemothorax, lung contusions, but no evidence of mediastinal hematoma. Since respiratory distress with a flail chest was observed, the patient was placed on an artificial ventilation, and thoracocentesis were also done on both sides, while the hemodynamics was fairly maintained with blood transfusion. On the 10th hospital day, however, he suddenly fell into a deep shock with the left hemothorax b

The treatment of repeated pathologic fractures of the mandible has long taxed the ingenuity of surgeons. The complexity of the problem increases with the recognition of a metabolic defect that renders the facial bones hypoplastic. In the case presented, a tentative diagnosis of familial facial osteodystrophy was made at the National Institutes of Health. Since that time, the patient has undergone autogenous onlay grafting, which has completely resorbed. Because of continuous resorption, a number of pathologic fractures have occurred. The most recent fracture was treated by means of a freeze-dr

BACKGROUND: Surgical rib fixation (SRF) is being used increasingly in trauma centers for stabilization of chest wall injuries, in line with new and evolving surgical techniques. Our institution has developed a pathway for the management of chest wall injuries and SRF, which includes a follow-up low-volume, noncontrast computed tomography (CT) scan at 12 months.
METHODS: This study was a single-center retrospective study conducted on 25 consecutive patients who underwent SRF between February 2019 and February 2020. All CT measurements were done by a CT radiographer under the supervision of a b

BACKGROUND: The accurate diagnosis of rib fractures is clinically important because these injuries are associated with complications. Despite its advantages, computed tomography is not infallible, and rib fractures may remain undetected on initial examinations.
OBJECTIVE: This study aims to perform a CT-based comparative analysis of occult and overt rib fractures.
METHODS: This longitudinal observational study included hospitalized patients presenting with blunt chest trauma. The patients were classified as having either overt rib fractures or occult rib fractures. Demographic and clinical d…

Surgical stabilization of rib fractures (SSRF) has garnered increased attention recently for treatment of blunt chest trauma (BCT). This study evaluated temporal trends in rates and timing of SSRF in isolated BCT patients, hypothesizing increased rates of early SSRF and SSRF overall. The 2017-2023 Trauma Quality Improvement Program (TQIP) database was queried for patients with ≥2 rib fractures and abbreviated injury scale <2 in non-chest regions to delineate an isolated BCT cohort. Patients were stratified by timing of SSRF from arrival: early (≤72 h), intermediate (>72-≤ 120 h), and late (>12…

BACKGROUND: Surgical stabilization of rib fractures (SSRF) is increasingly performed. Nationwide data comparing its outcomes with nonoperative management (NOM) and defining the best timing for SSRF are scarce.
METHODS: We analyzed data from the American College of Surgeons Trauma Quality Improvement Program, 2017-2021. Adults with three or more blunt rib fractures and no major extrathoracic injury were included. Surgical fixation was compared with risk-weighted NOM using inverse probability of treatment weighting. Primary outcome was in-hospital mortality. Secondary outcomes were hospital and…

BACKGROUND: In intensive care unit management, the serratus anterior plane block is sometimes meant to be inefficient for treating rib fractures, despite its proven efficacy in thoracic surgery analgesia. This cadaveric study aimed to investigate the anatomical distribution of local anesthetic by evaluate its dissemination on fractured and non-fractured sides. The primary outcome was the distribution pattern of dye as visualised by CT imaging and confirmed by anatomical dissection.
METHODS: Cadaveric study. Single-centre study conducted in the Department of Anatomy Two formalin-fixed female c…
