Indirect rupture of the globe was found to be a rare injury, more common in males (P less than .01), and having a tendency to involve the left eye. Severe chemosis, a hyphema, and hypotony were usually present, but the intraocular pressure was elevated in three eyes. Fourteen patients operated upon for subcutaneous bladder perforation are reported. The majority of the patients (11/14) had intraperitoneal rupture of the bladder and multiple associated injuries (10/14), most often pelvic fractures (9/14). Traumatic rupture of the spleen is a well known problem, complicating many types of abdominal injury. In contrast, a spontaneous rupture is a rare occurrence, mainly associated with a large spleen resulting from infections, the most common in this country being infectious mononucleosis. It is also a rare complication of leukaemias and lymphomas and the case presented here demonstrates a peculiar diagnostic problem.

The traditional management of amebic liver abscess which has ruptured into the peritoneal cavity has been conservative and 70% to 90% fatal. Laparotomy with complete toilet of the abscess and the peritoneal cavity has shown encouraging results. A comparative study of the management of 36 cases of intraperitoneal rupture of amebic liver abscess was carried out. Twenty-one cases were treated by laparotomy with 85.5% survival. A report of six cases of retroperitoneal rupture of the duodenum is presented. A high index of suspicion is necessary to enable early diagnosis to be made and appropriate treatment carried out. Early treatment will prevent the extremely rare, but at present uniformly fatal complication of gas gangrene. Caecal rupture due to colonic ileus is rare and has a mortality rate of 43 per cent. Three new cases are presented and the 18 previously reported cases are reviewed. The disease has always occurred in association with another illness, has usually afflicted patients over the age of 55 and has only resulted when the caecum was at least 9 cm in diameter. The technique of 'blow-hole' caecostomy, a method for decompressing the distended caecum, is described.

The authors review the case histories in twenty patients with traumatic rupture of the diaphragm seen and surgically treated during the last eight years at the Central Hospital of the Mexican Red Cross. Twenty-seven cases of ruptured chordae tendineae have been discovered during surgery for mitral regurgitation (9,3 %) : the highest incidence of ruptured chordae tendineae has been found among pure mitral insufficiency (36 %).
