Crusted scabies with scalp involvement in an institutionalized elderly.

Scabies is a common disease and typically described as a skin condition with sparing of face and scalp in adults. However, crusted scabies is not conventional scabies. It can also affect the scalp. Herein, we report an unusual case of crusted scabies with scalp infestation and suggest that for cases with suspicious lesions on scalp, immunocompromised patients or relapse cases, crusted scabies, special age groups, such as infant, young children, and the elderly, topical treatment of the scalp should be included.

Vertical or horizontal scalp mobility: What to assess.

BACKGROUND: Scalp is comprised of five layers namely skin, connective tissue, aponeurosis, loose areolar tissue, and periosteum, each having distinct characteristics. The scalp mobility plays an important role in scalp wound closure. Thus, the estimation of scalp mobility is necessary to obtain a tension-free closure and fine scar in hair restoration surgery. AIMS: To know the difference of available number of FUs while using two different scalp mobility methods, that is, horizontal vs vertical methods. PATIENTS/METHODS: The study was conducted in a private hair restoration clinic in the pat…

Aggressive cutaneous squamous cell carcinoma of the scalp.

Cutaneous squamous cell carcinoma (cSCC) is the second most common nonmelanoma skin cancer, and its incidence is increasing globally. In Korea, there were 12,516 diagnosed cases of cSCC between 1999 and 2014. Surgical treatment, for which several options are available, is the standard of care for cSCC and securing a sufficient surgical resection margin is always important. cSCC of the scalp sometimes exhibits unusually aggressive behavior. In this article, we report a case of cSCC of the scalp with invasion into the skull and dura mater.

How to Optimize Trichoscopy for Evaluation of Scalp Vessels.

INTRODUCTION: Trichoscopy greatly facilitates clinical diagnosis in patients with hair loss and may decrease the necessity for histopathological examination. Structures which may be revealed by trichoscopy include hair shafts, hair follicle openings, the perifollicular epidermis, and cutaneous microvessels. Cutaneous microvessels revealed in trichoscopy may vary in type and number depending on the scalp area, type of the disease, and its activity. Firm direct pressure (diascopy) might result in their blanching, but as of yet, there are no studies on the types of vessels that do or do not blanc…

Erosive pustular dermatosis of the scalp: causes and treatments.

Erosive pustular dermatosis of the scalp is a rare condition which primarily affects older women after local trauma and has historically been treated with topical steroids. As it is a rare entity and resembles other dermatologic conditions, it may easily be misdiagnosed. Identifying the causes and evaluating the efficacy of treatments of erosive pustular dermatosis of the scalp (EPDS) is of great importance to both avoid misdiagnosis and ensure optimal treatment of this rare condition. There are numerous causes. In addition to surgeries and physical injuries, topical and procedural treatments …

Case Report: A rapidly growing cyst on the scalp.

Trichilemmal carcinoma is a rare tumour derived from the outer root sheath of hair follicles .  It can be difficult to distinguish both clinically and histologically from other skin lesions, particularly squamous cell carcinoma.  We present the case of a 62-year-old female with a 20-year history of three 1-cm cysts on her scalp.  Over a six-month period, a cyst overlying the occiput had become painful and grown in size.  The general practitioner and subsequently local emergency department suspected infection.  The lesion was incised, and the patient was treated with oral antibiotics.  At the t…

Scalp Cooling: Implementation of a Program at a Multisite Organization.

BACKGROUND: Chemotherapy-induced alopecia is a well-known side effect of some types of cancer treatments. With U.S. Food and Drug Administration clearance of two scalp cooling machines, patients with cancer now have the opportunity to minimize this hair loss. However, multiple barriers can exist for organizations when establishing a scalp cooling program. OBJECTIVES: This article describes the experience of a large multisite organization that implemented a machine-based scalp cooling program. METHODS: Nursing staff led an interprofessional team that addressed components of the program. As a …

Pyoderma Gangrenosum of the Scalp: A Rare Clinical Variant.

Pyoderma gangrenosum (PG) is a rare, neutrophil-predominant dermatosis that usually presents as a papule or pustule and progresses into a painful ulcer. Clinical and histopathological features are nonspecific, making PG a challenging condition to diagnose. Lesions may occur anywhere on the body; however, the lower extremity is the most common location. Solitary lesions in atypical locations such as the scalp are uncommon, making this clinical variant especially difficult to recognize and diagnose. Although the clinical features and subsequent management of scalp PG might be different from othe…

Congenital Arteriovenous Malformation of the Scalp Involving the Orbit.

BACKGROUND: Arteriovenous malformations (AVMs) of the scalp are rare and infrequently encountered by the neurosurgeon. CASE DESCRIPTION: We report a unique case of a 42-year-old patient who presented with a progressive worsening of visual acuity in the right eye (lower quadrantanopia) and palpebral ptosis. Physical examination revealed a right exophthalmos and a right frontoparietal scalp soft swelling when the patient was in the supine position. Neurologic work-up showed a scalp AVM extending into the orbit and connected to an intraorbital cavernous angioma. The patient was treated with a fr…

Glioblastoma multiforme metastases to the masticator muscles and the scalp.

Glioblastoma multiforme (GBM) is the most common malignant primary brain tumor. Metastases outside the nervous system are a rare phenomenon. We present a pictographically striking case of GBM with metastases in the masticatory muscles of the infratemporal fossa and the scalp, in the context of a dense intracranial dissemination. Extracranial metastases of GBM have been reported anecdotally, while, to our knowledge, masticatory muscle metastases have been previously reported in only one case.

A Teenager With Sickle Cell Disease and Scalp Swelling.

An intracranial bleed with a midline shift is a potentially life-threatening clinical condition. We present the unusual case of a 13-year-old boy with sickle cell disease who had numerous emergency department visits for a scalp hematoma and was subsequently determined to have subdural and epidural hematomas with midline shift, associated with a skull bone infarction. We review the pathophysiology of this unusual condition and emphasize the importance of including it in the differential diagnosis of any child with sickle cell anemia presenting with a nontraumatic scalp hematoma.
