Long-term health-related quality of life after complex and/or complicated esophageal atresia in adults and children registered in a German patient support group.

BACKGROUND: Health-related quality of life (HRQoL) after esophageal atresia (EA) repair is postulated to be good. However, little is known about the long-term results after repair of complex and/or complicated EA regarding HRQoL. We investigated long-term HRQoL after delayed anastomosis, esophageal replacement, major revisions, or multiple dilatations in patients registered in a support group. METHODS: Patients registered in the German patient support group database (KEKS) were enrolled and allocated to subgroups according to surgical treatment and age. HRQoL was evaluated using validated que

AIM: To explore what it means to be a close relative of a person dependent on home enteral tube feeding (HETF) and how they can manage this situation. METHODS: Twelve close relatives were interviewed twice, using open-ended questions. Five were relatives of patients supported by home care

Follow-up of pathological gamblers after treatment.

Pathological gamblers entering a comprehensive treatment program were thoroughly assessed before treatment and 6 months after completion of treatment. The reports of gamblers and collateral informants were highly consistent. Follow-up interviews were conducted with 57 of the 66 patients entering treatment. Total abstinence was reported by 56% (N = 32) of the patients located for follow-up, and significant improvement on a wide range of measures was documented. The authors feel that the results demonstrate that pathological gambling is a treatable mental disorder.

Is breast the best?

With the recent resurgence of breast-feeding, the medical profession has been facing numerous questions regarding the advantages, limitations and practicality of this mode of infant feeding. The recent identification in human milk of "growth modulators" has probably augmented its nutritional superiority. Several psychosocial advantages of breast-feeding have also become well recognized. The increasing knowledge on the immunologic quality of human milk has contributed a great deal to the explanation of the long noticed lower morbidity and mortality in breastfed infants. Several benefits have al

[Alcohol dependence syndrome and before-discharge intervention method (BDIM)--Report 2. Statistical analysis of BDIM].

The present author has developed the structured Before-Discharge Intervention Method (BDIM), a modified version of the intervention methods by Johnson and Picard in an effort to achieve the patient's awareness of his/her problem drinking. The BDIM first involves the patient's family members and helps them recover from co-dependence on the patient's alcoholism and secondly, instructs them to write structured letters to the patient. Each letter should include a few episodes of recent and past drinking problems, and family members' concern and love for the patient, and family members' hope for hi

A web-based self-help intervention for partners of cancer patients based on Acceptance and Commitment Therapy: a protocol of a randomized controlled trial.

BACKGROUND: There is a growing recognition that cancer not only affects the lives of the patients, but also the lives of their partners. Partners of cancer patients are highly involved in the illness trajectory by providing informal care and they often experience distress. However, supporting interventions for this group are scarce and existing interventions bear several limitations. On the basis of the need for theory- and evidence-based supportive interventions for partners of cancer patients, the web-based self-help intervention Hold on, for each other has been developed. This intervention

Association of psychiatric and substance use disorder comorbidity with cocaine dependence severity and treatment utilization in cocaine-dependent individuals.

The relations among psychiatric and substance dependence disorders and treatment utilization are of interest both for their clinical management and for health services. We examined these relations using six self-reported indices of cocaine dependence severity and three self-reported measures of treatment utilization and self-help group participation for cocaine dependence. The sample consisted of dyads: namely, a cocaine-dependent adult proband (N=449) and a cocaine-dependent sibling (N=449). Psychiatric and substance use disorders were assessed with the Semi-structured Assessment for Drug Dep

A health profile of spousal Alzheimer's caregivers. Depression and physical health characteristics.

1. For caregivers of persons with Alzheimer's disease (AD), each day holds unexpected challenges and losses of personal control. Both physical and emotional health are at risk for caregivers of persons with AD. 2. Depression is a problem for caregivers, whether the spouse with AD remains in the home or has been transferred to a nursing home. 3. It is important for clinicians to advise caregivers that depression is a treatable disorder. When a caregiver complains of sleep problems, a red flag for depression should go up.
