Data are presented on 633 strains of Shigella, and 197 strains of Salmonella, isolated in Bujumbura, capital of Burundi, between 1980 and 1985. The epidemiology of shigellosis is dominated by the resurgence of the Shiga bacillus and by its increasing resistance against all available antibacterial drugs. Salmonellosis is caused by a limited range of serotypes, most of them, with the exception of the typhoid bacillus, becoming multiresistant. Bacteriaemia with S. enteritidis is a frequent complication in patients with AIDS.

Antibacterial sensitivity tests were done for 513 salmonellae and 133 shigellae. Salmonella strains were divided into three groups: 336 Salmonella typhi, 134 salmonellae isolated from cases of gastroenteritis and 43 salmonellae isolated from non-human sources. The specificity and sensitivity of the antibody neutralization test in the diagnosis of dysentery have been studied. This test has proved to be highly specific in experimental and clinical trials. The antibody neutralization test is more effective than the methods of the isolation of shigellae. The accuracy of this test can be increased by the elimination of different antierythrocyte agents from the secretions of persons under examination.

Some characteristics of S. flexneri 2a mutants resistant to various concentrations of polymyxin M were studied. The data indicate that mutations resulting in low (50 microgram/ml) and high (300 microgram/ml) levels of the antibiotic resistance were determined by different genes. Thirty-eight cases of Shigella vaginitis were identified in a retrospective review of records kept over the past 14 years. The cases of vaginitis were due to three subgroups of Shigella. Vaginitis varied in severity and duration, persisting for several months in some instances. In 47% of the cases, there was associated bloody vaginal discharge. Two patients were long-term gastrointestinal carriers of Shigella flexneri for 23 mo and 6 mo, respectively. Neither patient responded to oral antibiotics, despite in vitro sensitivity of the bacteria to the antibiotics administered. Oral oxolinic acid produced immediate cessation of the carrier state in both patients, with resolution of minor but persistent physical complaints.

One hundred seventy-three strains of shigellae (113 Shigella sonnei, 56 S. flexneri, and four others) isolated in Houston during 1974 were tested for susceptibility to commonly used and newer antimicrobial agents.
