IMPORTANCE: The Instability Severity Index (ISI) score was developed to evaluate a patient's risk of recurrent shoulder instability following arthroscopic Bankart repair. While patients with an ISI score of >6 were originally recommended to undergo an open procedure (ie, Latarjet) to minimise the risk of recurrence, recent literature has called into question the utility of the ISI score.
OBJECTIVE: The purpose of this systematic review was to evaluate the efficacy of the ISI score as a tool to predict postoperative recurrence among patients undergoing arthroscopic Bankart procedures.

»: Shoulder instability is a complex problem with a high rate of recurrence in athletes. Treatment of a first-time subluxation or dislocation event is controversial and depends on patient-specific factors as well as the identified pathology.
»: Athletic trainers and physical therapists are an integral part of the treatment team of an in-season athlete who has experienced a shoulder instability event. Through comprehensive physiological assessments, these providers can effectively suggest modifications to the patient's training regimen as well as an appropriate rehabilitation program.

BACKGROUND: The purpose of this study was to perform a systematic review and meta-analysis of the current evidence in the literature to determine how arthroscopic Bankart repair (ABR) and remplissage compare with ABR alone and the open Latarjet procedure for anterior shoulder instability in patients with concomitant Hill-Sachs lesions.
METHODS: A literature search was performed based on the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) guidelines. Studies comparing ABR and remplissage vs. ABR alone or the Latarjet procedure for anterior shoulder instability in pa

In an in vitro cadaver study, the mechanical properties of capsulolabral repair were significantly improved with the authors' double-row technique over single-row repair. Whether this time zero improvement in mechanical properties will translate into improved clinical outcomes remains to be seen, and there are concerns, including risks of increased costs and complications, with the clinical application of the technique.

Glenohumeral arthritis may develop after primary or recurrent shoulder dislocation or after surgery for stabilization. Postoperatively, the incidence is reported to be from 12% to 62%, depending on different risk factors. There is no certain correlation between the surgical technique and the rate of arthrosis. Capsulorraphy arthropathy is biomechanically and clinically a well defined entity. The risk of developing severe arthrosis of the shoulder following dislocation of the shoulder is between 10 and 20 times greater in comparison to the normal population. Risk factors are the age during the

Posterior instability is less common than anterior shoulder instability and diagnosis and treatment are more difficult and complex. This review article gives an overview on diagnostics, classification and the influence on therapy. Furthermore different arthroscopic and open procedures are presented and the surgical techniques are described in detail.

Operative repair of shoulder instability may fail because of multiple causes: a constitutional predisposition, a new trauma, incorrect diagnosis, inadequate operative techniques or inappropriate rehabilitation can be involved. The key to successful revision surgery is a thorough analysis of errors of the primary repair and the revision also has to deal with the decisive pathological factors. The management of revision surgery after failed surgery for patients with instability has to focus on the decisive pathological factors and has to take a higher complication rate and lower success rate tha

Glenohumeral joint instability and dislocations are common diagnoses seen by physicians. There are many different pathologic etiologies for these conditions. A thorough understanding of the history, physical examination, pathoanatomy, and classification systems is required to make an accurate diagnosis. With the appropriate diagnosis, the clinician can choose the correct treatment and improve the patient's outcome.

MR imaging has proved to be of great value in the assessment of musculoskeletal imaging. Its superiority in evaluating knee and spine pathology is well established. Recent studies also have established an important role for MR imaging in the assessment of shoulder disorders. Multiplanar capabilities coupled with excellent contrast resolution that allows simultaneous delineation of osseous and soft-tissue structures make MR imaging a useful method for evaluating the complex anatomy of the shoulder.

Understanding the loads and stresses on different tissues within the shoulder complex is crucial for preventing joint injury and developing shoulder implants. Finite element (FE) models of the shoulder joint can be helpful in describing these forces and the biomechanics of the joint. Currently, there are no validated FE models of the intact shoulder available in the public domain. This study aimed to develop and validate a shoulder FE model, then make the model available to the orthopaedic research community. Publicly available medical images of the Visible Human Project male subject's right s…

Recurrent shoulder dislocation is often induced as a result of a traumatic situation that results in structural disease; however, a small subset of individuals suffer symptomatic recurrent shoulder instability without trauma. The glenohumeral articulation geometry offers a lot of flexibility at the sacrifice of inherent stability. The commonly dislocated joint is the shoulder joint as it has intrinsic instability that can lead to repeated dislocations. The procedure is designed to minimize the probability of problems. Physiotherapy is essential for regaining patients' mobility and enhancing th…

Neck/shoulder pain has previously been linked to repetitive work and muscle fatigue. We have shown that asymptomatic people performing repetitive upper limb tasks display signs of shoulder fatigue and of whole-body compensatory strategies. However, the role played by the proprioceptive system in the production of these compensatory strategies has not been studied. A group of asymptomatic adults (n = 18) performed a repetitive pointing task at shoulder height to fatigue. Before and after fatigue, they performed two position sense tasks, eyes closed: a single-joint task where they abducted their

Accurate shoulder joint angle estimation is crucial for analyzing joint kinematics and kinetics across a spectrum of movement applications including in athletic performance evaluation, injury prevention, and rehabilitation. However, accurate IMU-based shoulder angle estimation is challenging and the specific influence of key error factors on shoulder angle estimation is unclear. We thus propose an analytical model based on quaternions and rotation vectors that decouples and quantifies the effects of two key error factors, namely sensor-to-segment misalignment and sensor orientation estimation …

CONTEXT: In-season glenohumeral instability is a common clinical dilemma faced by physicians who care for athletes. Both nonoperative and operative management of athletes with in-season glenohumeral instability have been well described. Functional bracing remains less understood as a treatment modality. This review aims to provide an update on the most recent literature regarding the use of functional bracing for shoulder instability.

Shoulder injuries are common in contact athletes and vary in severity because of the required complex interplay of shoulder stability and range of motion for proper function. Pathology varies based on sport but most commonly includes shoulder instability, acromioclavicular injuries, traumatic rotator cuff tears, and brachial plexus injuries. Acute management ranges from reduction of shoulder dislocations to physical examination to determine the severity of injury. Appropriate radiographs should be obtained to evaluate for alignment and fracture, with magnetic resonance imaging commonly being n

INTRODUCTION AND IMPORTANCE: Rare presence of intra-articular osteoid osteoma may be difficult to diagnose due to the lack of typical radiographic features and clinical appearance similar to other articular pathologies. Additionally traditional treatment choices for osteoid osteoma may not suit the given environment of the shoulder joint area.
CASE PRESENTATION: We presented a 50-year-old male with a prolonged history of anterior shoulder pain and shoulder stiffness after physical activity. Intra-articular joint pathology was suspected after initial clinical and radiographic assessment. Magne…
