Multidisciplinary Management of an Anterior Skull Base Teratoma in a Neonate.

Teratomas are embryonal neoplasms composed of 2 or 3 germ layers. We present the case of a female neonate who had a rare, mature, skull base teratoma that was managed surgically by fetal surgery, neurosurgery, otolaryngology, and plastic surgery. Imaging revealed an anterior skull base teratoma with extension into the nasopharynx, resulting in gross enlargement of the right nostril and sinonasal cavity. An ex utero intrapartum treatment procedure was performed to establish an airway before placental support was removed. Subsequently, neurosurgery and ENT performed endonasal resection of the te…

Management of Traumatic Anterior Skull Base Fractures and Cerebrospinal Fluid Fistulas.

OBJECTIVES: Blunt trauma and falls from heights are the primary causes of skull base fractures, which comprise ∼20% of all skull fractures. One-third of these fractures lead to a cerebrospinal fluid (CSF) fistula, typically managed conservatively and without surgery. AIMS: The authors analyzed the factors affecting the formation and closure time of CSF fistulas. METHODS: The authors followed up 31 patients with traumatic anterior skull base fractures (ASBF) between November 15, 2020 and October 10, 2023. The clinical characteristics, tomography data, and occurrence of CSF fistulas were docum…

Defining the Appropriate Length of Antimicrobial Therapy for Skull Base Osteomyelitis.

OBJECTIVES: Skull base osteomyelitis (SBO) is a rare infection comprising either the lateral (LSBO), central (CSBO), or both lateral and central (BSBO) skull base. Due to its rarity, the optimal length of antimicrobial therapy (AMT) has not been well defined. Therefore, the purpose of this study was to identify factors that may influence outcomes of SBO and formulate clearer recommendations on AMT duration. METHODS: A multi-institutional retrospective cohort study of SBO patients was conducted. Patient demographics, medical comorbidities, presenting symptoms, laboratory results, radiographic …

Ethmoid Sinus Mucocele Penetrating the Anterior Skull Base: A Case Report.

Sinus mucoceles are nonmalignant cystic tumors lined by non-neoplastic epithelium, typically involving the frontal and ethmoid sinuses. Although it is common for these mucoceles to cause destruction of surrounding bone tissue due to their growth, cerebrospinal fluid leaks resulting from skull base penetration by an ethmoid sinus mucocele have rarely been reported. A 24-year-old male patient presented with right proptosis and right periorbital pain, who underwent bilateral endoscopic sinus surgery 12 years ago. Endoscopic sinus surgery was performed to treat the right ethmoid sinus mucocele, an…

Management of Severe Dysphonia and Dysphagia Following Lateral Skull Base Surgery.

OBJECTIVES: Lateral skull base surgeries pose a risk of injuring the lower cranial nerves, leading to potential postoperative complications such as dysphonia and dysphagia. Conservative treatments have shown limited efficacy in addressing these resultant voice and swallowing dysfunctions, significantly impacting patient quality of life. This study aims to evaluate the safety and effectiveness of a combined surgical approach involving autologous fat injection laryngoplasty (AFIL) and transcervical cricopharyngeal myotomy (TCPM) in patients suffering from severe dysphonia and dysphagia following…

Endoscopic endonasal obliteration of the frontal recess and anterior skull base.

BACKGROUND: Although endoscopic techniques have become more widespread in repair of frontal sinus (FS) defects, certain pathologies still re … oscopic procedures. No morbidity or mortality resulted from the endoscopic procedure. CONCLUSIONS: Skull base defects following a failed cranialization of FS are usually located in or in close proximity to the frontal recess. These defects can be safely and effectively repaired via an EEA.

Endoscopic Endonasal Repair and Reconstruction of Traumatic Anterior Skull Base Defects.

Eighty percent of cerebrospinal fluid leaks (CSF) occur following trauma and complicate 12 to 13% percent of all basilar skull fractures (Prosser, Vender, and Solares, 2011). An endoscopic endonasal approach (EEA) is often the preferred method of repair with greater than 90% success rates (Prosser, Vender, and Solares, 2011). We report a case of a 37-year-old man who presented to our regional level 1 trauma centre with multiple facial injuries. Initial cross-sectional imaging revealed multiple, continuous anterior skull base fractures with associated pneumocephalus. Though initially managed co…

Electromyographic predictors of abducens nerve palsy after endoscopic skull base surgery.

OBJECTIVE: Recovery of abducens nerve palsy (ANP) after endoscopic endonasal skull base surgery (ESBS) has been shown to be potentially predicted by postoperative ophthalmological examination. Triggered electromyography (t-EMG) and free-run electromyography (f-EMG) activity provide an intraoperative assessment of abducens nerve function, but associations with long-term ANP outcomes have not been explored. The objective of this study was to describe intraoperative abducens EMG characteristics and determine whether these electrophysiological profiles are associated with immediately postoperative…

Anatomical Triangles for Use in Skull Base Surgery: A Comprehensive Review.

Procedures performed along the skull base require technical prowess and a thorough knowledge of cranial anatomy to navigate the operative field. Anatomical triangles created by unique anatomical structures serve as landmarks to guide the surgeon during meticulous skull base procedures. The corridors rapidly orient the surgeon to the operative field and permit greater confidence regarding skull base position during dissection. A literature review was performed with use of the PubMed database and reference list searches from full-text reviewed articles, which resulted in the identification of 31…
