Headache, drugs and sleep.

BACKGROUND: Headache and sleep mechanisms share multiple levels of physiological interaction. Pharmacological treatment of headache syndromes may be associated with a broad range of sleep disturbances, either as a direct result of the pharmacology of the drug used, or by unmasking physiological alterations in sleep propensity seen as part of the headache symptom complex. PURPOSE: This review summarises known sleep and circadian effects of various drugs commonly used in the management of headache disorders, with particular attention paid to abnormal sleep function emerging as a result of treat…

[Neurology and sleep medicine].

Neurology is frequently confronted with sleep disturbances and/or their effects. Sleep disturbances may be early, main- or escort symptom, in addition as well a factor of risk for a neurological illness. Goal of this contribution is to mediate a small overview of usual organic sleep disturbances and to give an insight into sleep medicine ways of thinking.

Nonepileptic paroxysmal sleep disorders.

Events occurring during nighttime sleep in children can be easily mislabeled, as witnesses are usually not immediately available. Even when observers are present, description of the events can be sketchy, as these individuals are frequently aroused from their own sleep. Errors of perception are thus common and can lead to diagnosis of epilepsy where other sleep-related conditions are present, sometimes initiating unnecessary therapeutic interventions, especially with antiepileptic drugs. Often not acknowledged, paroxysmal nonepileptic behavioral and motor episodes in sleep are encountered much…

Sleep and synaptic changes.

Several recent studies, using molecular, electrophysiological, or structural approaches, have investigated how synapses are affected by sleep, spontaneous wake, chronic sleep restriction, and acute sleep deprivation. Overall, the results have found that even a few hours of sleep or wake can modify the molecular composition of excitatory synapses, change their efficacy, and make synapses grow or shrink. Moreover, partial and total loss of sleep affect the ability of synapses to undergo long-term potentiation, an effect that may underlie some of the negative consequences of sleep deprivation on …

Classification of sleep disorders.

The classification of sleep disorders is necessary to discriminate between disorders and to facilitate an understanding of symptoms, etiology, and pathophysiology that allows for appropriate treatment. The earliest classification systems, largely organized according to major symptoms (insomnia, excessive sleepiness, and abnormal events that occur during sleep), were unable to be based on pathophysiology because the cause of most sleep disorders was unknown. These 3 symptom-based categories are easily understood by physicians and are therefore useful for developing a differential diagnosis. The…

Movement disorders and sleep.

This article summarizes what is currently known about sleep disturbances in several movement disorders including Parkinson disease, essential tremor, parkinsonism, dystonia, Huntington disease, myoclonus, and ataxias. There is an association between movement disorders and sleep. In some cases the prevalence of sleep disorders is much higher in patients with movement disorder, such as rapid eye movement sleep behavior disorder in Parkinson disease. In other cases, sleep difficulties worsen the involuntary movements. In many cases the medications used to treat patients with movement disorder dis…

Migraine and sleep disorders.

The burden of migraine strongly increases, considering its linkage with sleep disorders. Migraine is positively associated with many sleep-complaint disorders; some are confirmed by several studies, such as restless leg syndrome, whereas others still remain uncertain or controversial, e.g. narcolepsy. Many studies have investigated the association between headache and other sleep disturbances such as daytime sleepiness, insomnia, snoring and/or apnea, but only a few have focused on migraine. Highlighting the comorbidity between migraine and sleep disorders is important to improve treatment str…

Pediatric sleep-wake disorders.

Sleep-wake problems are common during childhood and adolescence. They are of diverse cause, and can contribute significantly to alterations in behavior, cognition, and learning. Obstructive sleep apnea, central hypoventilation syndrome, narcolepsy, periodic hypersomnia, delayed sleep phase syndrome, restless legs syndrome, parasomnias, and sleep disruption consequent to psychiatric disorders are some of the commonly encountered conditions. Some aspects of sleep architecture and its organization change with age and maturation. Diagnostic criteria and sleep laboratory techniques and findings for…

Sleep disturbance at altitude.

PURPOSE OF REVIEW: The aim is to describe the impact of altitude upon sleep, the physiology that underpins these changes and the therapeutic solutions that are currently in place. RECENT FINDINGS: On ascending to altitude, lowland residents commonly experience some degree of sleep disturbance. Occasionally, this can prove very uncomfortable and impact upon daytime activities. Historically, the underlying cause of sleep disturbance was thought to be due to the effect of periodic breathing. However, recent research has shown that the link between periodic breathing, lighter stages of sleep and …

Sleep, stroke and poststroke.

There is abundant and still evolving scientific clinical evidence linking sleep apnea with stroke risk factors, cardiovascular disease, and stroke. Sleep apnea is a modifiable risk factor and efforts to control this condition should be pursued vigorously, particularly in patients at risk of vascular disease. Emerging evidence has also linked other sleep disorders, such as periodic limb movements of sleep and restless legs syndrome, with vascular risk.

Sleep-related movement disorders.

Several movement disorders may occur during nocturnal rest disrupting sleep. A part of these complaints is characterized by relatively simple, non-purposeful and usually stereotyped movements. The last version of the International Classification of Sleep Disorders includes these clinical conditions (i.e. restless legs syndrome, periodic limb movement disorder, sleep-related leg cramps, sleep-related bruxism and sleep-related rhythmic movement disorder) under the category entitled sleep-related movement disorders. Moreover, apparently physiological movements (e.g. alternating leg muscle activat…

Medication-induced sleep disturbances.

OBJECTIVE: To review the literature for medication-induced sleep disturbances. DATA SOURCE: MEDLINE/PUBMED searches (January 2000-May 2010) were conducted to identify pertinent English-language studies. STUDY SELECTION AND DATA EXTRACTION: References were reviewed to identify literature that provided evidence of sleep disturbances caused by prescription medications. DATA SYNTHESIS: Review of studies was performed to determine the effect a given medication had on subjects' sleep architecture or those that induced common sleep disorders. Tables were constructed for specific sleep disturbances…

[Sleep medicine in pneumology].

Diagnosis and treatment of sleep related breathing disorders have become an essential challenge of internal medicine. They are highly important clinically because of the impairment of daytime performance, attention and concentration with elevated risk of accidents in workplace and traffic and because of their consequences on cardiovascular and metabolic disorders. The obstructive sleep apnoea syndrome (OSAS) has proven to be one of the most important risk factors for arteriosclerosis, especially in the cerebral vessels. OSAS induces arterial hypertention and increases mortality due to cardiova…

[Sleep related breathing disorders].

Sleep related breathing disorders are common conditions and the management of patients with sleep apnea is an essential component of routine patient care. Daytime sleepiness is the leading symptom of sleep apnea but not mandatory. Especially patients with cardiac, pulmonary or metabolic comorbidities can benefit from treatment of sleep apnea and those patients should be considered for cardio-respiratory screening even with mild clinical symptoms. Continuous positive airway pressure therapy is still the most efficient treatment for sleep apnea and standard treatment for severe forms. With patie…

Sleep disorders in children.

Although sleep disorders such as insomnia and obstructive sleep apnea are common in both children and adults, the clinical features and treatments for these conditions differ considerably between these two populations. Whereas an adult with obstructive sleep apnea typically presents with a history of obesity, snoring, and prominent daytime somnolence, a child with the condition is more likely to present with normal body weight, tonsillar hypertrophy, and inattentiveness during school classes. The adult with suspected sleep apnea almost always undergoes a baseline polysomnogram and proceeds to …

[Sleep changes with aging].

Many factors contribute to the alteration of sleep in older adults. Most of their complaints can be explained by the modifications of the sleep organisation observed in this population. Sleep architecture is altered with aging. Insomnia and excessive daytime sleepiness can reflect an alteration of the circadian rhythm. This population is also frequently exposed to specific sleep disorders such as the restless leg syndrome, periodic limb movements or obstructive sleep apnea syndrome. Sleep disorders in patients with Alzheimer's disease is a daily preoccupation at home or in institution. Circadi…

[Sleep related eating disorder].

Nighttime eating is categorized as either sleep-related eating disorder (SRED) or night eating syndrome (NES). Critical reviews of the literature on both disorders have suggested that they are situated at opposite poles of a disordered eating spectrum. The feeding behavior in SRED is characterized by recurrent episodes of eating after an arousal from nighttime sleep with amnesia. Conversely, NES could be considered as an abnormality in the circadian rhythm of meal timing with a normal circadian timing of sleep onset. Both conditions clearly concentrate to occur during young adulthood, and are …
