[Hysterical emergencies].

Most probably common in emergency departments, but still not well studied in this context, the disorders formerly called "hysteria" are now included in the group "Somatization, undifferentiated somatoform, conversion and dissociative disorders" (SSCD disorders) DSM IV. Their common presentation is that of idiopathic somatic symptoms linked with mental disorders. In the emergency department these symptoms confront physicians who generally do not have extensive psychiatric training. The symptoms occur, and disappear, undetermined by the patients, who are genuinely ill and not malingering. Aside

Gender differences in disorders comorbid with posttraumatic stress disorder among U.S. Sailors and Marines.

Psychological comorbidity, the co-occurrence of mental health disorders, is more often the rule than the exception among individuals with posttraumatic stress disorder (PTSD). Research shows that prevalence estimates for specific psychological disorders differ by gender; however, little is known about whether these patterns persist in the presence of a comorbid PTSD diagnosis. This study examined gender differences in prevalence estimates for conditions comorbid with PTSD using medical records for 523,626 active duty U.S. Sailors and Marines who entered the military over an 8-year period. Neurological and Psychiatric Comorbidities in Chronic Obstructive Pulmonary Disease.

BACKGROUND AND PURPOSE: Chronic obstructive pulmonary disease (COPD) is often accompanied by different neurological and psychiatric comorbidities. The purpose of this study was to examine which of them are the most frequent and to explore whether their manifestation can be explained by underlying latent variables. METHODS: Data about patients with COPD and their neurological and psychiatric comorbidities were extracted from an electronic database of the National Health Insurance Fund of Lithuania for the period between January 1, 2012, and June 30, 2014. Exploratory factor analysis (EFA) was

Predictors of suicide attempt within 30 days of first medically documented major depression diagnosis in U.S. army soldiers with no prior suicidal ideation.

BACKGROUND: Understanding mental health predictors of imminent suicide attempt (SA; within 30 days) among soldiers with depression and no prior suicide ideation (SI) can inform prevention and treatment. The current study aimed to identify sociodemographic and service-related characteristics and mental disorder predictors associated with imminent SA among U.S. Army soldiers following first documented major depression diagnosis (MDD) with no history of SI. METHODS: In this case-control study using Army Study to Assess Risk and Resilience in Servicemembers (STARRS) administrative data, we identi

[Dispute over the multiple personality disorder: theoretical or practical dilemma?].

Dissociative identity disorder (DID) could also be referred to as multiple personality disorder (MPD). Due to rare occurrence and difficulty in its' identification it is infrequently diagnosed in Poland. The indicated disorder has been portrayed by the authors throughout the historical context, referring to initial 18th century's references concerning dissociation. A typical dissociatively disordered person has been characterized along with his individual personality categories such as: original personality, altered personality, host and personality fragment. Electrophysiological evaluation of psychogenic movement disorders.

Psychogenic movement disorders (PMD) include a group of neurological symptoms which cannot be explained by any organic syndrome. The diagnosis of PMD is challenging for both neurologist and psychiatrist. Electrophysiological examination is a useful tool to evaluate and support a diagnosis PMD. It includes a set of tests which are chosen appropriate to the clinical setting that provides objective criteria for the diagnosis of PMD. The various tests available include accelerometry, surface electromyography, electroencephalography, jerk locked back averaging and pre-movement potentials, somatosen

Physiology of psychogenic movement disorders.

Psychogenic movement disorders (PMDs) are common, but their physiology is largely unknown. In most situations, the movement is involuntary, but in a minority, when the disorder is malingering or factitious, the patient is lying and the movement is voluntary. Physiologically, we cannot tell the difference between voluntary and involuntary. The Bereitschaftspotential (BP) is indicative of certain brain mechanisms for generating movement, and is seen with ordinarily voluntary movements, but by itself does not indicate that a movement is voluntary. There are good clinical neurophysiological method

A cross-cultural study of the structure of comorbidity among common psychopathological syndromes in the general health care setting.

This study presents analyses of 7 common psychopathological syndromes in the World Health Organization (WHO) Collaborative Study of Psychological Problems in General Health Care (T. B. Ustun & N. Sartorius, 1995). Data on depression, somatization, hypochondriasis, neurasthenia, anxious worry, anxious arousal, and hazardous use of alcohol were analyzed for 14 countries (Ns for each country ranged from 196 to 800). Four models were evaluated: a 1-factor model; a 2-factor model in which all syndromes except hazardous use of alcohol represented internalizing problems; and two 3-factor models. The

The report is based on 1,165 medical inpatients seen by the consultant psychiatrist within a 3-year period. During the last year evaluated for this study psychiatric consultation was requested for 2.8% of the total non-psychiatric hospital admissions. Questions to the consultant are most often referred to psychogenic factors, depression and suicidality. The most frequent psychiatric diagnosis was neuroses/reactions/ personality disorders followed by suicidality, organic psychoses, psychosomatic illnesses, endogenous psychoses, somatopsychic syndromes and addictions. A brief review of organic diseases masquerading as functional illness.

A wide variety of physical diseases masquerade as psychiatric disorders. Clinicians may be distracted by a patient's chief complaint of anxiety, depression, or personality change and fail to pursue the possibility of organic disease. The author provides a brief review of physical diseases that produce psychiatric symptoms. He stresses the danger of misdiagnosis and urges clinicians to remain alert to etiological clues during examination, especially when treating patients who have a history of depression, neurotic reactions, or psychosis.

[Depression in rheumatic diseases or somatic symptoms in depression].

Depression can be produced in many different ways. Via cerebral nuclear-specific imbalance a depression can be associated with disturbances in nuclear regions regulating muscular tone. Such syndromes appear chiefly as syndromes of the cervical vertebral column or lumbal vertebral column or as a general "non-specific-pain-syndrome". Depression may also cause manifestations of "latent" rheumatic complaints by lowering the pain threshold. Chronic physical diseases may be assumed to produce depression via the "common pathway" (nuclear-specific imbalance of biogenic amines). Masked depression can a

Childhood chronic daily headache: a biopsychosocial perspective.

The aim of our observational study was to highlight some clinical observations on chronic daily headache (CDH) in children and adolescents. Data on patients < or =18 years aged presenting with CDH to the Pediatric Headache Clinic at the Royal University Hospital, Saskatoon, Canada, were collected prospectively and sequentially from February 2004 to July 2006. Standardized data sheets and definitions were used. Follow-up information on the 70 participants (22 males, 48 females) was current to February 2007. Fifty-four participants (77%) had had recurrent headaches before transformation to CDH.

Predictors of Treatment Success of Psychotherapy in Functional Disorders: A Systematic Review of the Literature.

OBJECTIVE: Functional disorders (FDs) benefit from psychotherapy. However, the determinants predicting their efficacy remain largely unexplored. METHODS: A systematic literature review was conducted. Disease phobia and disease conviction are separate dimensions underlying hypochondriasis.

The current study uses data from a large nonclinical college student sample (N = 503) to examine a structural model of hypochondriasis (HC). This model predicts the distinctiveness of two dimensions (disease phobia and disease conviction) purported to underlie the disorder, and that these two dimensions are differentially related to variables important to health anxiety and somatoform disorders, respectively. Results were generally consistent with the hypothesized model. Specifically, (a) body perception variables (somatosensory amplification and anxiety sensitivity - physical) emerged as sign…
