Obstructive sleep apnea, seizures, and childhood apraxia of speech.

Associations between obstructive sleep apnea and motor speech disorders in adults have been suggested, though little has been written about possible effects of sleep apnea on speech acquisition in children with motor speech disorders. This report details the medical and speech history of a nonverbal child with seizures and severe apraxia of speech. For 6 years, he made no functional gains in speech production, despite intensive speech therapy. After tonsillectomy for obstructive sleep apnea at age 6 years, he experienced a reduction in seizures and rapid growth in speech production. The findin…

Achieving the next level in cleft speech intervention: A protocol of a randomized sham-controlled trial to provide guidelines for a personalized approach in children with cleft palate.

BACKGROUND: Compensatory cleft speech disorders can severely impact speech understandability and speech acceptability. Speech intervention is necessary to eliminate these disorders. There is, however, currently no consensus on the most effective speech therapy approach to eliminate the different subtypes of compensatory cleft speech disorders. AIMS: To compare the immediate, short- and long-term effects of three well-defined speech intervention approaches (i.e., a motor-phonetic approach, a linguistic-phonological approach and a combined phonetic-phonological approach) on the speech and healt…

Nonpsychotic thought disorder: objective clinical identification of somatization and antisocial personality in language patterns.

This report describes a new method of using language patterns to identify somatization and antisocial personality (ASPD) disorders in clinical practice. A set of definitions describing characteristic speech patterns was developed to identify "nonpsychotic thought disorder" (NPTD). Speech patterns of subjects with somatization disorder and/or ASPD were compared with those of controls. Blind raters assessed audiotaped samples of speech obtained through open-ended interviews for instances of elements of NPTD. Women with somatization or ASPD had significantly more NPTD speech responses than contro

Prevalence of articulatory disorders of different sounds in a group of Finnish first-graders.

Occurrence and type of articulatory speech disorders were studied in 157 7-year-old first-graders found to have articulatory disorders at school and referred for further treatment to the Department of Phoniatrics, University Central Hospital. The control group consisted of all 130 first-graders in two elementary schools in Kuopio. About one third of the control group had articulatory speech disorders, one fifth of them expressing need for speech therapy. In both groups distortions of the /s/-sound were the most common disorders, distortions of the /r/-sound being rather uncommon and those of t…

Speech and Language Markers as Longitudinal Predictors of Youth Mental Health: A Systematic Review.

INTRODUCTION: Severe mental disorders in young people (< 25 years) are often preceded by subtle changes in communication and thinking, detectable in speech. Speech and language markers are promising for early detection; however, no systematic review has evaluated their prospective utility in predicting mental disorders in youth. We comprehensively reviewed longitudinal studies assessing speech/language markers as predictors of major mental disorder onset or symptom progression in youth. METHODS: We searched for longitudinal studies using recorded speech samples from youth or family members to

The supplementary motor region and speech emission.

Speech disorders occur with left supplementary motor region (SMR) lesions. Right SMR lesions probably produce speech disorders only when the right cerebral hemisphere is speech-dominant. Paroxysmal SMR dysfunction produces both uncontrollable vocalization and speech arrest. Slowly evolving SMR lesions produce reduced and labored speech output with word-finding difficulties. Suddenly acquired SMR lesions produce marked speech suppression and aphonia; during recovery nonpropositional "automatic" speech may be initiated easily, even involuntarily, while initiation of propositional speech is still…

Effects of oropharyngeal surgery on velopharyngeal competence.

PURPOSE OF REVIEW: This article addresses the question of whether it is safe to perform pharyngeal surgery on children who are at risk for speech disorders. Patients with orofacial clefts or craniofacial disorders often have speech and resonance disorders that affect their understandability. They are also prone to develop sleep apnea or other diseases that affect Waldeyer's ring. RECENT FINDINGS: There is increasing recognition of sleep disordered breathing and obstructive sleep apnea syndrome (OSAS) in the group of children who have, or are at risk of developing, speech disorders. Often, the…

Voice disorders in children with classic galactosemia.

Children with classic galactosemia are at risk for motor speech disorders resulting from disruptions in motor planning and programming (childhood apraxia of speech or CAS) or motor execution (dysarthria). In the present study of 33 children with classic galactosemia, 21% were diagnosed with CAS, 3% with ataxic dysarthria, and 3% with mixed CAS-dysarthria. Voice disorders due to laryngeal insufficiency were common in children with dysarthria and co-occurred with CAS. Most (58%) of the children with classic galactosemia had decreased respiratory-phonatory support for speech, and 33% had disturbe…

Speech treatment for Parkinson's disease.

Researchers estimate that 89% of people with Parkinson's disease (PD) have speech and voice disorders including disorders of laryngeal, respiratory and articulatory function. Despite the high incidence of speech and voice impairment, studies suggest that only 3-4% of people with PD receive speech treatment. Here, we review the literature on the characteristics and features of speech and voice disorders in people with PD and the types of treatment techniques available (medical, surgical and behavioral), with a focus on behavioral therapies. We provide a summary of the current status of the fiel

A Framework of Motoric Complexity: An Investigation in Children With Typical and Impaired Speech Development.

Introduction The current work presents a framework of motoric complexity where stimuli differ according to movement elements across a sound sequence (i.e., consonant transitions and vowel direction). This framework was then examined in children with childhood apraxia of speech (CAS), other speech sound disorders (SSDs), and typical development (TD). Method Twenty-four children (CAS, n = 8; SSD, n = 8; TD, n = 8), 5-6 years of age, participated in this study. The children produced words that varied in motoric complexity while transcription, acoustic, and kinematic data were collected. Multidime

EPG research and therapy: further developments.

Electropalatography (EPG) has been used in the past 50 years for studying the patterns of contact between the tongue and the palate during speech production in typical speakers and those with speech disorders due to different causes. At the 7th EPG Symposium in Japan that was held online on 24 January 2021 (see: https://epg-research.sakura.ne.jp/), a panel of invited experts discussed their views regarding further developments and application of the technique. This paper provides a summary of this discussion. EPG offers information on articulation which cannot be replaced by other instrumental…
