Extradural anterior temporal fossa approach to the paranasal sinuses, nasal cavities through the anterolateral and anteromedial triangles: Combined microscopic and endoscopic strategy.

OBJECTIVE: To demonstrate the utility and limitations of the extradural endoscopic-assisted anterior temporal fossa approach to the pterygopalatine fossa (PPF), infratemporal fossa (ITF), paranasal sinuses (PS), parapharyngeal region (PPR), nasal cavities (NC), epipharynx (EP), and clivus. METHODS: A frontotemporal orbitozygomatic craniotomy is performed. The dura is elevated from the cavernous sinus (CS). The anterior temporal fossa floor is drilled. Foramen rotundum and ovale are opened. The PPF is exposed and the lateral margin of inferior orbital fissure (IOF) is removed. The anterolatera

Morphometric Alterations of the Sphenoid Ostium and other Landmarks in Acromegaly: Anatomical Considerations and Implications in Endoscopic Pituitary Surgery.

BACKGROUND: The sphenoid ostium (SO) is an important landmark for the endoscopic surgeon. Changes in size and position of the SO and variations in other skull base landmarks in acromegalics have not been adequately evaluated. AIMS: The authors evaluated the morphometry and location of the SO and other landmarks in acromegaly and compared these findings with those in nonfunctioning pituitary adenomas (NFPAs). METHODS: In this retrospective case-control study, the dimensions and location of the SO and other skull base landmarks were radiologically evaluated in 18 patients with growth hormone (

Surgery as a safe and effective treatment option for spheno-orbital meningioma: a systematic review and meta-analysis of surgical techniques and outcomes.

PURPOSE: The effectiveness and safety of surgery for spheno-orbital meningiomas remains subject of debate, as studies often describe different surgical approaches and reconstruction techniques with very heterogeneous outcomes. We aimed to systematically summarize and analyse the literature on spheno-orbital meningiomas regarding presenting symptoms, surgical techniques, outcomes and complications. METHODS: Studies were retrieved from eight databases. Original articles were included if in ≥5 patients presenting symptoms, surgical treatment and outcomes were described. Fixed- and random-effects

Safety margin for the preservation of the nasoseptal artery during trans-nasal endoscopic approach.

BACKGROUND: The introduction of the nasoseptal flap for the reconstruction of extended endoscopic approaches decreased the incidence of postoperative fistula. In order to preserve the septal vascular pedicles, many have started to prepare the flap systematically, prior to the opening of the sphenoid rostrum. The aim of this study is to obtain an average measure of the location of the posterior septal artery using the upper edge of the choana as a landmark. METHODS: Ten cadaveric heads, fixed with formaldehyde and injected with colored silicone, were studied. The course, branching pattern and

The Endoscopic Prelacrimal Recess Approach to the Pterygopalatine Fossa and Infratemporal Fossa.

PURPOSE: The authors studied the anatomic importance of the endoscopic prelacrimal recess approach (PLRA) to the pterygopalatine fossa (PPF) and infratemporal fossa (ITF). METHODS: Ten adult heads (20 sides) from cadavers fixed in formalin were dissected using the PLRA. Anatomic dissections were detailed and several crucial landmarks measured. RESULTS: Identification of the infraorbital neurovascular bundle is the crucial step for the detection of other branches of the maxillary artery. The distance from the base of columella to inferior orifice of the nasolacrimal duct, sphenopalatine foram

Osteochondroma Presenting as a Calcified Mass in the Sellar Region and Review of the Literature.

Objective Osteochondroma (OC) is the most common benign bone neoplasm. It infrequently occurs in the cranial cavity as a calcified lesion and very rarely presents in the sellar region. The present study summarizes the knowledge about OCs of the sellar region. Methods We searched the literature search for the clinical appearance of OCs and other calcified pathologies occurring in the sellar region. Results A total of 21 English-language articles published from 1961 to 2015 documented cases of calcified lesions in the sellar region including cerebral aneurysm, chondroid chordoma, chondroma, cran

Feasibility of piezoelectric endoscopic transsphenoidal craniotomy: a cadaveric study.

OBJECTIVE: Endoscopic transsphenoidal approach has become the gold standard for surgical treatment of treating pituitary adenomas or other lesions in that area. Opening of bony skull base has been performed with burrs, chisels, and hammers or standard instruments like punches and circular top knives. The creation of primary bone flaps-as in external craniotomies-is difficult. The piezoelectric osteotomes used in the present study allows creating a bone flap for endoscopic transnasal approaches in certain areas. The aim of this study was to prove the feasibility of piezoelectric endoscopic trans

Endoscopic endonasal transpterygoid approaches: anatomical landmarks for planning the surgical corridor.

OBJECTIVES/HYPOTHESIS: Endoscopic endonasal transpterygoid approaches (EETA) use the pneumatization of the sinonasal corridor to control lesions of the middle and posterior skull base. These surgical areas are complex and the required surgical corridors are difficult to predict. AIM: Define anatomical landmarks for the preoperative planning of EETAs. STUDY DESIGN: Anatomical study. METHODS: We reviewed images from high-resolution maxillofacial CT scans with (0.6-mm axial slice acquisition). Cephalometric measurements were obtained using Kodak Carestream Image Software (Rochester, NY). RESU

[Re-evaluation of using acupuncture needle as sphenoidal electrode in temporal lobe epilepsy].

In 1965, Feng of the Peking Union Hospital published an article entitled "Innovation in electroencephalography: the use of acupuncture needles as sphenoidal electrodes". It was a preliminary report, but surprisingly EEG records in the figures did not show definite spikes or sharp waves in the sphenoidal leads. In 1983, Feng and his colleagues reported a summary of 2,000 cases studied with acupuncture needle sphenoidal electrodes. This time, spikes or sharp waves were shown in EEG recordings. However, cases studied were several "paroxysmal disorders", including psychomotor seizure (155 cases),

An unusual mode of injury-implantation of a broken toothbrush medial to ramus: report of a case.

Impalement injury and implantation of a foreign body in the oral cavity is common in young children. Typically the child falls with the foreign body in the mouth resulting in implantation of the object at various sites in the oral cavity. It is quite unusual to have the head of a toothbrush with bristles implanted fully in the soft tissue medial to the ramus, in the region of pterygomandibular space, following an injury with a cricket ball without a history of fall. A case of an unusual foreign body implanted at an unusual site by an unusual injury is being presented.

[A case of sphenoid ridge meningioma presenting as subarachnoid hemorrhage].

The authors present a case of sphenoid ridge meningioma presenting as subarachnoid hemorrhage. A 56-year-old man came to our hospital with a complaint of severe headache developed during evacuation of the bowels. Computed tomogram on admission revealed a mass lesion of high density, which was homogeneously enhanced with contrast media, in the medial part of the left sphenoid ridge, but no evidence of the intracranial hemorrhage. Nevertheless, the cerebrospinal fluid obtained by lumbar puncture was bloody, indicating the occurrence of subarachnoid hemorrhage. Cerebral angiograms showed no abnor

[Two cases of total ophthalmoplegia after section of the vidian nerve (author's transl)].

The authors report two cases of complete ophthalmoplegia occurring during surgical section of the homolateral vidian nerve for chronic nasal obstruction due in one case to hypertrophic rhinitis and in the other to nasal polyposis. In both cases, whilst there was more or less complete regression of the oculomotor paralyses, unilateral amaurosis persisted as a permanent sequela. This paper is published with the aim of drawing attention to the possibility of complications which are out of proportion with the initial symptomatic discomfort and which may occur whatever the surgical approach used si

Optimal indications and limitations of endoscopic transorbital superior eyelid surgery for spheno-orbital meningiomas.

OBJECTIVE: Spheno-orbital meningiomas (SOMs) are complicated tumors that involve multiple structures at initial presentation, such as the orbit, temporalis muscle, sphenoidal bone, cavernous sinus, and temporal or infratemporal fossa. The infiltrative growth and complexity of this type of meningioma make total resection impossible. In this study, the authors evaluated the surgical outcome of the endoscopic transorbital approach (eTOA) for SOM. In addition, they identified optimal indications for the use of eTOA and analyzed the feasibility of this approach as a minimally invasive surgery for S

Metastasizing chordoma in early childhood: a pathological and immunohistochemical study with review of the literature.

A 2 1/2-year-old female with a sphenooccipital-vertebral chordoma presented with neck pain, torticollis, fever, a lytic lesion of C2 vertebra, and bilateral nodular infiltrates in the lung. The lung biopsy revealed multiple tumor emboli by an enigmatic epithelioid-appearing neoplasm with immunohistochemical staining for vimentin, cytokeratin, and epithelial membrane antigen. A thorough roentgenographic evaluation disclosed a destructive, prepontine mass in the region of the clivus, erosion of the odontoid process, and compression of the cervical spinal cord. The patient died after a clinical c

[Craniosynostoses in Egypt: clinical forms and treatment methods. A study of 55 surgically treated patients].

The relative incidence of the various craniosynostosis deformities in Egypt is given on a total of 55 patients who were operated upon. The most common deformity was oxycephaly, 25 cases (i.e. 45%). Various surgical methods have been applied depending on the deformity present. In isolated scaphocephalic deformities (9 cases), 3 pairs of strip craniectomies also called "the triple strip" were sufficient and gave satisfactory results. For other types of deformities, an attempt was made to improve the aesthetic results following simple forehead advancement, which was done for the first five cases.

Endoscopic prelacrimal approach to lateral recess of sphenoid sinus: feasibility study.

BACKGROUND: Various pathologies, including cerebrospinal fluid leaks and meningoencephaloceles, may arise in the lateral recess of the sphenoid sinus (LRSS), which may be accessed via an endonasal transpterygoid approach. The objective of this study was to evaluate the feasibility of accessing the LRSS via an endoscopic prelacrimal approach. Furthermore, we hypothesized that this approach may protect the pterygopalatine ganglion and vidian nerve. METHODS: Five cadaveric heads (9 sides) with a well-pneumatized LRSS were identified and an endonasal prelacrimal approach was performed. The infrao
