Comprehensive analysis of the clinical and urodynamic outcomes of secondary tethered spinal cord before and after spinal cord untethering.

OBJECTIVE: Secondary tethering of the spinal cord (TSC) occurs in 3-30% of patients with history of spinal cord dysraphism repair. As many patients with spinal cord dysraphism already have symptoms, it might be difficult to diagnose secondary TSC. Regular clinical/urodynamic (UDS) evaluation is therefore advised to pick-up changes early before they progress. This study aimed at presenting clinical/UDS outcomes for patients with secondary TSC who underwent spinal cord untethering (SCU). STUDY DESIGN: Charts of patients with secondary TSC between 1998 and 2010 were reviewed retrospectively. Pat…

Microdialysis to Optimize Cord Perfusion and Drug Delivery in Spinal Cord Injury.

OBJECTIVE: There is lack of monitoring from the injury site to guide management of patients with acute traumatic spinal cord injury. Here, we describe a bedside microdialysis monitoring technique for optimizing spinal cord perfusion and drug delivery at the injury site. METHODS: Fourteen patients were recruited within 72 hours of severe spinal cord injury. We inserted intradurally at the injury site a pressure probe, to monitor continuously spinal cord perfusion pressure, and a microdialysis catheter, to monitor hourly glycerol, glutamate, glucose, lactate, and pyruvate. The pressure probe an…

Extensibility of the lumbar and sacral cord. Pathophysiology of the tethered spinal cord in cats.

Tethered spinal cord, or tethered cord syndrome, describes a disorder manifested by progressive motor and sensory deficit in the legs and by incontinence. Tethered cord syndrome occurs when the elongated spinal cord is anchored by a thick filum terminale or other pathological structures. The underlying mechanism is impairment of oxidative metabolism in the lumbosacral cord. The authors studied the extensibility of various parts of lumbar, sacral, and coccygeal segments in experimental animals and correlated this with the oxidative metabolism in these segments. The filum terminale possesses far…

Adult tethered cord syndrome: relative to spinal cord length and filum thickness.

The adults with tethered cord syndrome (TCS) are divided into two groups: group-1 adult TCS patients with spinal dysraphism; and group-2 patients without dysraphism who develop signs and symptoms in adulthood. A significant number of group-2 patients failed to show elongation of the spinal cord and abnormally thick filum terminale. This paper reports the combinations of the location of the caudal end of the spinal cord and the thickness of the filum terminale in 104 patients with group-2 adult TCS. The caudal end of the spinal cord was found at the L2-3 intervertebral space or above in 37 pati…

Parallel serotonergic pathways influencing spinal cord circuits.

In this issue of Neuron, Fenstermacher et al.1 reveal that subregions of the descending serotonin system innervate separate structures in the spinal cord and are differentially active during locomotion. The results suggest fine-grained neuromodulatory control of sensation and movement.

Treatment Strategies of Intramedullary Spinal Cord Tumors.

Primary intramedullary spinal cord tumors are rare in both pediatric and adult patients. Surgical resection offers the best outcomes when safe and feasible; however, other treatment strategies are often needed. These represent a challenge due to lack of standard of care, largely due to rarity of these tumors.

Magnetoelectric microrobots for spinal cord injury regeneration.

Spinal cord injury remains difficult to treat because of the intrinsically limited regenerative capacity of neurons. Although neural progenitor cell (NPC) therapies are promising, inadequate graft survival, uncontrolled differentiation and weak functional integration continue to restrict outcomes. Here we report biohybrid microrobots called NPCbots, fabricated by integrating human-induced pluripotent-stem-cell-derived NPCs with magnetoelectric nanoparticles, enabling wireless magnetic navigation and non-invasive neuronal stimulation. A lab-on-a-chip platform allows scalable fabrication and mai…

Intramedullary Spinal Cord Tumors in Pediatric Patients.

Intramedullary spinal cord tumors in children are rare but important lesions representing fewer than 10% of pediatric central nervous system tumors. Low-grade astrocytomas and ependymomas predominate, with modern molecular markers-such as MAPK pathway alterations, H3K27M mutations, and MYCN amplification-refining diagnosis and prognosis. Children often present with progressive pain, motor deficits, or scoliosis, leading to delayed recognition. MRI, including diffusion tensor imaging, is essential for evaluation and surgical planning. Maximal safe resection remains the cornerstone of management…

Neuromodulation in Spinal Cord Injury: Clinical Review.

Spinal cord injury is a devastating consequence of trauma. It disrupts connections within the spinal cord resulting in a constellation of symptoms. Neuromodulation approaches have shown promising results in restoring function. The techniques differ in their neuronal targets, applications, and functional outcomes. This review compares the different neuromodulatory modalities including the noninvasive transcutaneous spinal cord stimulation and functional electrical stimulation to the surgically implanted epidural spinal cord stimulation and vagus nerve stimulation. Further research is needed on …

Non-traumatic spinal cord ischaemia - Surfer's myelopathy.

A previously healthy surfer apprentice in his late 30s presented to our emergency department with acute paraplegia and loss of sensation in the lower limbs following sudden back pain during his third surfing lesson. The patient reported a hyperextension movement of the back while trying to stand up on the surfboard. On examination, he was classified as grade A on the American Spinal Injury Association (ASIA) Impairment Scale, with a sensory level at T10, hypotonic paraplegia, urinary retention and constipation. T2-weighted MRI revealed hyperintensity in the central spinal cord between T7 and T…

Spinal cord ischemia: The "snake bite sign".

Descriptive signs in radiology can aid in easier pattern recognition and quicker diagnosis. In spinal cord ischemia, paired anterior-horn T2-hyperintensities have traditionally been known as the "owl's eyes" or "snake eyes" sign. We discuss how these signs, while visually apt, convey no pathophysiologic context and propose renaming this finding the "snake bite sign". The image still evokes two punctate marks, yet the metaphor extends to a snake bite (two fang-like dots) rather than two bright foci (eyes) staring back at the viewer. Moreover, besides the sign metaphorically resembling a traumat…

Spinal Cord Injury Remyelination: Pathways to Therapies.

Spinal cord injury (SCI) is a debilitating condition that results from a culmination of acute and chronic damage to neural tissue, specifically the myelin sheath, thus impacting neurons' abilities to synergistically perform their physiological roles. This review explores the molecular underpinnings of myelination, demyelination, and remyelination, emphasizing the role of oligodendrocyte progenitor cells (OPCs), astrocytes, and microglia in physiological, and pathophysiological, healing. Furthermore, we link these processes with emerging therapeutic strategies currently under investigation in a…

Arachidonic acid metabolism in spinal cord injury.

BACKGROUND: Spinal cord injury (SCI) initiates a cascade of secondary pathological processes significantly impairs functional recovery and leads to complications such as neuropathic pain. Arachidonic acid (AA) and its metabolites play crucial roles through various metabolic pathways. These pathways contribute to neuroinflammation, tissue damage, and functional impairment following SCI. AIM: We aim to provide a comprehensive outline on AA metabolic pathways, enzymes and signaling cascades in spinal cord injury pathophysiology, in order to develop new perspectives in treatment and diagnosis. K…
