Pectus carinatum.

Pectus carinatum or keel chest is a spectrum of progressive inborn anomalies of the anterior chest wall, named after the keel (carina) of ancient Roman ships. It defines a wide spectrum of inborn protrusion anomalies of the sternum and/or the adjacent costal cartilages. Pectus carinatum is often associated with various conditions, notably Marfan disease, homocystinuria, prune belly, Morquio syndrome, osteogenesis imperfecta, Noonan syndrome, and mitral valve prolapse. Treatment of pectus carinatum by nonsurgical methods such as exercise and casting has not been worthwhile, whereas surgical man

The relationship between the stomatognathic system and body posture.

In recent years, many researchers have investigated the various factors that can influence body posture: mood states, anxiety, head and neck positions, oral functions (respiration, swallowing), oculomotor and visual systems, and the inner ear. Recent studies indicate a role for trigeminal afferents on body posture, but this has not yet been demonstrated conclusively. The present study aims to review the papers that have shown a relationship between the stomatognathic system and body posture. These studies suggest that tension in the stomatognathic system can contribute to impaired neural contr

Lower limb involvement in spinal function and low back pain.

AIM: To evaluate evidence for involvement of the lower limb in spinal function and low back pain (LBP). DESIGN: A hypothesis based on a critical review of the relevant biomechanical and clinical literature. RESULTS: The spine resembles an inverted pendulum that supports the weight of the upper body; its stability requires a moving base that is provided by the joints of the lower limb, especially the hip. However, the sacroiliac joints are unlikely to be important for spinal function. The changing pattern of gait and development of lumbar lordosis, in early childhood, provide evidence for the

Traction in the treatment of spinal deformity.

An independent study was undertaken whereby all members of the Scoliosis Research Society as well as orthopedic surgeons known by the authors to be involved in the treatment of spinal deformities were queried to determine 1) the role of various forms of traction, 2) methods and timing ot application, 3) the indications for use with respect to type of spinal deformity and angle of curvature, and 4) the complications encountered during use. Our results indicated a majority of spinal surgeons, 85%, used preoperative traction when curves were greater than 80° . Cotrel's traction was considered ine

Development of thoracic spine kyphosis and lumbar spine lordosis in the growing child from birth to adulthood: protocol for a systematic review.

INTRODUCTION: Thoracic kyphosis, a posterior curvature of the thoracic spine and lumbar lordosis, an anterior curvature of the lumbar spine, are essential components of spinal alignment that facilitate a biomechanically efficient upright stance. Understanding the normal growth of the spine and its constituents, especially during periods of rapid growth, is important as deviations from normal growth are thought to be initiating factors of spinal conditions such as scoliosis. While in the coronal plane the spine is straight, there is variability in the reported normative values of sagittal spina

Treating the Aging Spine.

Demographic trends make it incumbent on orthopaedic spine surgeons to recognize the special challenges involved in caring for older patients with spine pathology. Unique pathologies, such as osteoporosis and degenerative deformities, must be recognized and dealt with. Recent treatment options and recommendations for the medical optimization of bone health include vitamin D and calcium supplementation, diphosphonates, and teriparatide. Optimizing spinal fixation in elderly patients with osteoporosis is critical; cement augmentation of pedicle screws is promising. In the management of geriatric

Si-MSPDNet: A multiscale Siamese network with parallel partial decoders for the 3-D measurement of spines in 3D ultrasonic images.

Early screening and frequent monitoring effectively decrease the risk of severe scoliosis, but radiation exposure is a consequence of traditional radiograph examinations. Additionally, traditional X-ray images on the coronal or sagittal plane have difficulty providing three-dimensional (3-D) information on spinal deformities. The Scolioscan system provides an innovative 3-D spine imaging approach via ultrasonic scanning, and its feasibility has been demonstrated in numerous studies. In this paper, to further examine the potential of spinal ultrasonic data for describing 3-D spinal deformities,

An Inter- and Intra-Rater Agreement Assessment of Two Novel Classification Systems for Coronal Imbalance in Adult Scoliosis and Spine Deformity.

OBJECTIVE: Coronal malalignment is a common feature of adult spinal deformity, and accurate classification is essential for diagnosis and treatment planning. However, variations in interpretation among clinicians can impact classification consistency. By assessing the reliability and applicability of these systems across different medical experts, this study seeks to establish a standardized approach to enhance clinical outcomes. This study aimed to evaluate the inter- and intra-observer agreement of two classification systems for coronal malalignment in adult spinal deformity patients, as pro

Trunk Injuries in Athletes.

Trunk pain is a common cause of performance limitation and time away from sport in athletes. However, atraumatic trunk injuries are underrepresented in medical literature and underrecognized clinically. Delays in diagnosis and initiation of appropriate treatment can increase injury morbidity and return-to-play time. Currently, evidence-based guidelines for diagnosis and treatment of trunk pain in athletes are limited. Thus, we provide an overview of atraumatic sport-related injuries to the thoracic spine (disc herniation, scoliosis, kyphosis), ribcage (bone stress injury, costochondritis, Tiet

Posterior thoracic osteotomies.

Spinal osteotomies are used to treat partially flexible and fixed deformities. Fixed thoracic spinal deformities have been traditionally treated with anterior release and posterior correction with fusion. In recent decades, it has been shown that posterior-only osteotomies might be sufficient to achieve proper deformity correction with lower complication rates than with combined anterior and posterior procedures. Different types of osteotomies have been described to treat spinal deformities through a single posterior approach. These include posterior column osteotomies such as the Smith-Peters

Practice advisory on the management of the perioperative period of scoliosis correction surgery.

Scoliosis correction surgery is a complex procedure aimed at rectifying the spinal lateral curvature and rotation. Perioperative anesthetic management is a critical determinant of both surgical success and patient safety. As surgical and anesthetic techniques continue to advance, scoliosis correction has become increasingly established in clinical practice. However, there remains a paucity of standardized guidelines specifically addressing anesthetic practices in this procedure. This article aims to review several frequently encountered challenges during the perioperative phase of scoliosis co

Scoliosis, spondylolysis, and spondylolisthesis. Their relationship as reviewed in 539 patients.

Five hundred consecutive idiopathic scoliosis patients in whom routine oblique roentgenograms of the lumbosacral area were obtained, plus 39 patients with a presenting problem of spondylolisthesis, were reviewed in an attempt to answer questions concerning the best therapeutic approaches in patients having coincident scoliosis and pars defects. The incidence of pars defects in patients with idiopathic scoliosis was found to be 6.2%, slightly higher than in the white female population at large. The indications for a surgical approach to the pars defect and/or the spinal curvature parallel the i
