First ray injuries.

The first ray differs from the other rays in its position and its importance with weight bearing. Because it is a critical structure in the formation of the tripod of the foot, injuries to this ray can cause critical alterations in the biomechanics of the foot. This allows for pathologic weight-bearing points of contact and deformity that lead to a disabling gait. Physicians who are involved in the care of the foot and ankle should be familiar with the spectrum of injuries that concern the first ray. These injuries, their management, and sequelae are reviewed.

Interventions for treating chronic ankle instability.

BACKGROUND: Chronic lateral ankle instability occurs in 10% to 20% of people after an acute ankle sprain. The initial form of treatment is conservative but if this fails and ligament laxity is present, surgical intervention is considered. OBJECTIVES: To compare different treatments, both conservative and surgical, for chronic lateral ankle instability. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialized Register (to July 2005), the Cochrane Central Register of Controlled Trials (The Cochrane Library 2005, Issue 3), and MEDLINE (1966 to April 2006), EMBAS

11. Fractures and minor head injuries: minor injuries in children II.

Fractures in children are common, but the plasticity of children's bones means that they may be incomplete. If a child has deformity, swelling or bony point tenderness in a limb after a fall, it is likely to be fractured. A fractured limb that appears deformed will most probably need to be reduced. Effective splinting, using whatever means is readily available, and early, adequate analgesia, can ameliorate the severe pain associated with a fracture. In young children with open growth plates, Salter-Harris type I injuries of the distal fibula are more common than ligament injuries of the ankle.

Midfoot injury in children related to mini scooters.

The remarkable rise in popularity of mini scooters has been accompanied by an increase in the number of foot and ankle injuries. We have observed unique pediatric foot injuries related to the use of mini scooters. An association may be present linking the design of the mini scooter and the riding practices of children. We report on two unique midfoot injuries in children occurring with mini scooter use and discuss how these foot injuries may be related to scooter design and children's riding practices. Safety guidelines aimed at the prevention of foot and ankle injuries while riding mini scoot

Sprains about the foot and ankle encountered in the workmans' compensation patient.

Soft tissue injury in the workmans' compensation patient population has been previously poorly understood. Injuries that do not show up easily on radiographs are often undertriaged and undertreated. With the advent of improved imaging techniques, an enhanced understanding of soft tissue injuries has evolved over the past few years. Coupled with improved surgical and rehabilitation techniques, severely injured patients are able to experience remarkable recoveries. Many cannot return to previous function. These patients pose challenges to the astute clinician with regards to psychosocial issues

Injuries initially misdiagnosed as sprained wrist (beware the sprained wrist).

OBJECTIVES: To describe the injuries misdiagnosed as a sprain of the wrist and to determine the approximate incidence of misdiagnosis in patients diagnosed as having a sprain of the wrist. METHODS: All diagnostic errors in an accident and emergency (A&E) department were noted on a computerised database. Injuries initially misdiagnosed as wrist sprain are described and compared with the number of diagnoses of sprained wrist on the A&E department computer. RESULTS: 57 injuries initially diagnosed as a sprained wrist had a different diagnosis (1.76% of all diagnoses of sprained wrists). This is

Comparison of search strategies and quality of medical information of the Internet: a study relating to ankle sprain.

OBJECTIVES: to evaluate the quality of web sites on ankle sprain diagnosis and treatment and to assess the impact of Internet search expertise on quality of retrieved information. METHOD: two internet search strategies were conducted - one developed by an experienced information officer (expert's search) and the other based on the search terms used by orthopaedic medical staff (doctors' search). RESULTS: the expert's search revealed 32 web sites, of which nine were relevant, whereas the doctors' search revealed 61 web sites of which 27 were relevant. Lateral ankle ligaments and tibiofibular syndesmosis. 13-MHz high-frequency sonography and MRI compared in 20 patients.

To test the ability of ultra-high frequency ultrasound (13 MHz scanner) to distinguish between intact and ruptured ligaments on the lateral side of the ankle, we examined 20 patients with an acute inversion injury with MRI and ultrasound. When judged by the MRI diagnosis, an injured anterior talofibular ligament was correctly diagnosed by ultrasound in 13 of 14 and an intact anterior talofibular ligament in 5 of 6 patients. In the case of the calcaneofibular ligament, 4 ruptured and 16 intact ligaments were diagnosed equally well with both methods. The injured anterior tibiofibular ligament wa

The evaluation and treatment of the injured acromioclavicular joint in athletes.

Injuries to the acromioclavicular joint are among the most commonly occurring problems in the athletic patient population. However, these injuries are often confused with other problems associated with the shoulder complex. This confusion was noted by Hippocrates (460-377 BC), who realized that acromioclavicular dislocation often was misdiagnosed as a glenohumeral injury. Galen (129-199 AD) experienced an acromioclavicular dislocation and could not tolerate the tight bandaging recommended at the time and thus became one of the earliest noncompliant patients. The understanding of acromioclavicu

Epiphyseal separation of the coracoid process without acromioclavicular dislocation.

A patient with an epiphyseal separation through the base of the coracoid process of the shoulder associated with a grade I acromioclavicular sprain is reported. The epiphyseal separation was not visible on conventional roentgenograms. A CT scan demonstrated the abnormality. In patients with considerable intractable pain due to contusion of the shoulder, early use of CT should be considered to determine whether symptoms are caused by epiphyseal separation of the coracoid process.

Dislocation of the tibialis posterior tendon: diagnosis and treatment.

Traumatic dislocation of the tibialis tendon occurred from minor ankle sprains in a 37-year-old male and a 53-year-old female. Both complained of local pain at the medial malleolus, and both walked with a limp. The diagnosis was suspected by clinical examination, in one case with 2 months' delay, and verified by ultrasound, computed tomography, and magnetic resonance imaging. The male patient was initially treated for an "uncomplicated ankle sprain." For various reasons surgery was delayed 4 months. During this interval the male patient complained of pain and severe dysfunction, requiring anal

Arthroscopic stabilization of the ankle.

Arthroscopic lateral ankle stabilization is a method for repair of the anterior talofibular ligament which involves decreased morbidity and minimal soft tissue disruption while restoring function and stability to the joint. This technique utilizes soft tissue anchoring systems that reinforce or repair the anterior talofibular ligament. Several techniques are described for the various types of ligamentous injuries. These methods are compared and contrasted with standard open reconstructive and reparative techniques. Advantages of the newer anchoring systems are discussed.

Peripheral arterial pseudoaneurysm treated by ultrasound-guided compression.

Ultrasound-guided compression repair of a pseudoaneurysm is a simple, safe and effective therapy, and has been proposed as a first-line treatment for pseudoaneurysms caused by catheterization. Herein, we report a case of a pseudoaneurysm arising from a sport injury which was treated by this method. Obliteration of the track of the pseudoaneurysm induced manual thrombosis and successfully converted the pseudoaneurysm to haematoma. Understanding the mechanism and the procedure is the basis of success in the use of this treatment.

Taping of the ankle--the effect on postural sway during perturbation, before and after a training session.

The application of adhesive tape to the ankle prevents sprains and is therefore frequently used by athletes with functional instability of that joint. The purpose of the study was to investigate the effect of taping on postural sway during a sudden perturbation in subjects with unilateral functional instability of the ankle joint. Nine Swedish male elite soccer players with unilateral functional instability of the ankle joint after a sprain participated in the study. Their mean age was 25 years. All but one had sustained an ankle sprain during the last 1-12 months, and they applied adhesive ta

[The unstable upper ankle joint. Treatment of acute ligament rupture--a review of the literature].

Acute lateral ankle ligament sprains represent a very frequent examinated and treated injury in the daily traumatological routine. A cursory review of recent articles published on this subject gives a confusing picture regarding diagnosis and treatment of such injuries. An air of controversy surrounds the interpretation of diagnostic x-rays and the management of these common ankle problems. This review article is to summarize current thoughts of different aspects concerning the treatment of acute lateral ankle ligament sprains.

Low back injury.

Low back injuries occur commonly in tennis, but the pathophysiologic, biomechanical, and clinical characteristics are not well defined. Tennis players may be at an increased risk of lumbar disc pathology from rotational and hyperextension shearing effects. Treatment of low back injury at present is empiric, but sport-specific lumbar stabilization and unloading of the lumbar disc should be developed. Prospective longitudinal research protocols are needed to study the lumbar spine in tennis players.

[Sprains of the wrist: current data and therapeutic orientations].

Despite the high incidence of wrist trauma, severe sprains with disruption of the ligaments are actually rare, apart from lesions of the scapholunate joint line. However, the diagnosis of severity should now be established, if not as an emergency, at least soon after the trauma, as deterioration of carpal instability always leads top osteoarthritis after a variable interval of time. When a ligament lesion is suspected, initial pain-free immobilisation, plain and dynamic x-rays followed by at least arthrography of CT arthrography, should be performed. After describing scapholunate sprain, the c
