Screening for schistosomiasis, filariasis, and strongyloidiasis among expatriates returning from the tropics.

The clinical utility of eosinophil determinations, stool examinations, and serological studies for detection of parasitic infection was examined in an asymptomatic expatriate population by retrospective chart review. The screened population consisted of 1,605 patients attending a tropical medicine clinic. The sensitivity of eosinophil counts as a screening test for infection with filaria, schistosomes, or Strongyloides stercoralis was 38%, and its positive predictive value was 9%. A cost-efficacy analysis of the use of three diagnostic tests in screening for infection with filaria (n = 23), sc

Chronic strongyloidiasis in Tasmanian veterans--clinical diagnosis by the use of a screening index.

Chronic infection with Strongyloides stercoralis presents a diagnostic problem because of the low recovery of the organism from stools and the insensitivity of current serological tests. Of 150 former Far East POWs, 26 (17%) had S. stercoralis in stools. The clinical features in 18 stool-positive patients were compared to those in 24 stool-negative patients and showed that strongyloidiasis was associated with a higher frequency of alteration in bowel habit, upper abdominal discomfort, rash and eosinophilia. On the basis of these features, a screening index was devised which largely separated s…

Predictive value of an enzyme-linked immunosorbent assay (ELISA) for the serodiagnosis of strongyloidiasis.

Strongyloides stercoralis is an opportunistic parasite of increasing importance in immunocompromised hosts. Current parasitologic methods for diagnosis are unsatisfactory, and the serologic screening of candidates for immunosuppression and other individuals at risk might prevent fatal disseminated infections. The predictive value of an enzyme-linked immunosorbent assay (ELISA) using S. stercoralis antigens has been evaluated on 268 infected patients, 571 noninfected controls, and 78 individuals with other parasitic infections. A positive test was accurate in predicting the presence of S. sterc

Comparative sensitivity and specificity of ELISA and IHA for serodiagnosis of strongyloidiasis with larval antigens.

Sera from 68 patients with parasitologically proven strongyloidiasis were tested by the ELISA and IHA tests using larval antigens prepared from Strongyloides stercoralis and Strongyloides ratti. The ELISA using the S. stercoralis antigen detected the greatest number of sero-reactors (83.8%), whereas the IHA using the S. ratti antigen detected the fewest (55.9%). In addition, the S. stercoralis antigen had higher geometric mean titers than the S. ratti antigen in both the ELISA and the IHA tests. Sera from 37 presumed normal individuals also were tested by IHA and ELISA and nonspecific reaction…

Diagnostic implications of parasite-specific immune responses in immunocompromised patients with strongyloidiasis.

Parasite-specific humoral and cellular immune responses were evaluated in in nine immunosuppressed patients with Strongyloides stercoralis i … antigens. Our results indicated that the possibility of a protective role of IgE and eosinophils in strongyloidiasis deserves further investigation. In addition, we suggest that the enzyme-linked immunosorbent assay for specific IgG antibodies may have an important place in the diagnosis of strongyloidiasis in immunocompetent as well as in immunocompromised patien …

Comparison of larval antigens in an enzyme-linked immunosorbent assay for strongyloidiasis in humans.

An enzyme-linked immunosorbent assay for specific antibodies in patients with strongyloidiasis was developed using antigens extracted from the third-stage larvae of Strongyloides stercoralis and Strongyloides ratti. Sera from two groups of patients with strongyloidiasis proven by parasitologic methods, including one group of 29 coded positive sera, and from presumed normal subjects as controls were tested. Sera with optical density values greater than two standard deviations above the mean optical density value for the sera from control subjects were considered to be positive. Using this crite…

Beyond antibody titers: Corticosteroid effects on IgG avidity dynamics in experimental strongyloidiasis.

Strongyloidiasis is a neglected tropical disease caused by Strongyloides spp., capable of lifelong persistence and fatal hyperinfection or dissemination in immunosuppressed individuals. Animal models play a key role in understanding host immune responses and improving diagnostics. This study evaluated the effects of dexamethasone and prednisolone, administered orally or subcutaneously, on the humoral immune response in a Strongyloides venezuelensis rat model. IgG kinetics and antibody avidity were analyzed by ELISA. Infected control animals seroconverted 8 days post infection (dpi), with all p…

Routine stool microscopy unmasking strongyloidiasis: a case report from Eastern Uttar Pradesh.

Strongyloidiasis is an intestinal parasitic disease caused by Strongyloides stercoralis, a soil-transmitted nematode. It is a common infection in Southeast Asia, including India, and is related to poor sanitation, rural residence, barefoot walking, and occupational exposure to soil. We report a case of a 71-year-old male farmer from rural Gorakhpur, Eastern Uttar Pradesh, who presented with intermittent watery diarrhoea for 2 years and tenesmus for 15 days. He was admitted with a diagnosis of fever of unknown origin. Routine stool microscopy revealed motile rhabditiform larvae of Strongyloides…

Clinical Characteristics of Strongyloidiasis during the COVID-19 Pandemic: Systematic Scoping Review.

The clinical impact of Strongyloides stercoralis hyperinfection secondary to immunosuppressive therapy for coronavirus disease 2019 (COVID-19) has been an emerging topic of interest, although characteristics of Strongyloides infection in COVID-19 patients are not yet well characterized. This study summarizes the existing evidence of Strongyloides infection in COVID-19 patients and recommends future areas of research. According to the PRISMA Extension for Scoping Reviews, we performed a search on MEDLINE and EMBASE for articles with keywords including "Strongyloides," "Strongyloidiasis," and "C…

Chronic strongyloidiasis in World War II Far East ex-prisoners of war.

Fifty-two of 142 (37%) American ex-prisoners of war that worked on the Burma-Thailand Railroad during World War II were found to have previo … cure in 93% and a microscopic cure in 100%; but was associated with frequent side effects. Chronic strongyloidiasis should be considered in veterans of Far East conflicts and in others with intimate soil contact in rural Strongyloides stercoralis-endemic areas who present with recurrent creeping skin eruption, abdominal pain, and eosinophilia.
