[Ruptured dermoid cyst].

We report the case of a 28-year-old male admitted to the emergency department for generalized seizure. A brain computed tomography (CT) revealed a ruptured dermoid cyst of the supra-tentorial stage (multiple drop-shaped fat structures were found in the subarachnoid space, basal cisterns and in ventricular system). An additional magnetic resonance imaging (MRI) was conducted to confirm the CT findings.

Fibrin degradation products in the cerebrospinal fluid of patients with pneumococcal meningitis.

Raised levels of fibrin degradation products were found in the cerebrospinal fluid of nearly all of 35 patients with pneumococcal meningitis. The mean level was higher in patients who died subsequently than in those who survived. Cerebrospinal fluid from patients with pneumococcal meningitis showed increased fibrinolytic activity as assessed by clot lysis, suggesting local production of fibrin degradation products within the subarachnoid space.

Decompressive bifrontal craniectomy for malignant intracranial pressure following anterior communicating artery aneurysm rupture: two case reports.

There are a number of causes of raised intracranial pressure (ICP) following aneurysm rupture. These include primary and diffuse hypoxic brain injury, intracranial hematomas, cerebral ischemia or infarction, and obstructive hydrocephalus. More localized brain swelling can also occur: the result of vasogenic and cytotoxic edema resulting from overlying bleeding in the subarachnoid spaces. In the case of rupture of an anterior communicating artery (ACommA) aneurysm and interhemispheric subarachnoid hemorrhage (SAH), this swelling can occur in both frontal lobes and when extensive, and the result…

Intraventricular and subarachnoid racemose cysticercosis.

Cysticercosis is endemic in India. Neurocysticercosis most commonly affects the brain parenchyma, which presents as focal lesions with the surrounding edema which later calcify. Rarely, it may affect the ventricular system and subarachnoid spaces and this form is known as racemose cysticercosis. We present magnetic resonance findings in a case of racemose cysticercosis.

Non-Aneurysmal Subarachnoid Hemorrhage in a Patient With Type 1 Diabetes Mellitus: A Case Report.

Non-aneurysmal subarachnoid hemorrhage (NASAH) is defined as bleeding into the subarachnoid space without an identifiable aneurysm. In patients with long-standing diabetes mellitus (DM), chronic microvascular damage may contribute to vascular fragility and hemorrhage. Emerging evidence suggests a possible association between NASAH and diabetes, particularly through microangiopathic mechanisms. We report the case of a 40-year-old female patient with a 14-year history of poorly controlled Type 1 DM (T1DM) who presented with sudden-onset severe occipital headache, neck stiffness, and vomiting. On…

Non-aneurysmal Subarachnoid Hemorrhage Presenting With Isolated Neck and Back Pain: A Rare Case Report.

Subarachnoid hemorrhage (SAH) is defined as bleeding into the subarachnoid space between the arachnoid and pia mater and is most commonly caused by rupture of an intracranial aneurysm if it occurs spontaneously. SAH typically presents with sudden, severe headache, often accompanied by nausea, vomiting, and neck stiffness. We report a rare case of non-aneurysmal SAH (NASAH) in an 83-year-old woman presenting with isolated acute neck pain and worsening chronic back pain, without headache or neurological deficits. Laboratory evaluation revealed hemorrhagic cerebrospinal fluid. Head and neck MRI r…

Developmental features of the neonatal brain: MR imaging. Part II. Ventricular size and extracerebral space.

Magnetic resonance (MR) imaging with a 0.6-T magnet was performed on 51 neonates, aged 29-42 weeks postconception. In 45 neonates, the ventr … ral space widths of 5-6 mm were seen in three other infants with severe asphyxia. Prominence of the subarachnoid space overlying the posterior parietal lobes is normal in neonates and should not be confused with cerebral atrophy. The authors conclude that V/B ratios of 0.26-0.34 and extracerebral space widths of 0-4 mm represent the normal range, and that neonates w …

Subarachnoid cisterns as surgical corridors: Integrating microsurgical anatomy and neuroimaging for intracranial navigation.

BACKGROUND: The subarachnoid space comprises a complex system of cerebrospinal fluid (CSF)-filled compartments known as cisterns, which contain critical neurovascular structures and serve as essential anatomical corridors in neurosurgery. Despite their importance, current literature often lacks integrated descriptions correlating microsurgical anatomy with modern neuroimaging, limiting their applicability in surgical planning and intraoperative navigation. METHODS: A comprehensive narrative review of the supratentorial, tentorial, and infratentorial subarachnoid cisterns was performed, integr…

Experimental production of leptomeningeal heterotopias from dissociated fetal tissue.

Subarachnoid heterotopias were produced experimentally in rats by intracisternal injection of dissociated fetal brain tissue. Heterotopias contained neurons and glial tissue and also mesodermal tissue including striated skeletal muscle. The data show that particles of disintegrated germinal tissue can survive, redistribute, settle and mature within the CSF spaces.

Subarachnoid spread of ependymoma: diagnosis and evaluation of therapy by computerized tomography.

The potential of ependymomas to spread throughout the intracranial subarachnoid space is well documented, though the incidence is disputed. This relates in part to the time required for such metastases to produce symptoms. The diagnosis of such spread, however, is important for therapeutic management. This report demonstrates the effectiveness of computerized tomography in diagnosing spread of ependymoma throughout the basal cisterns and cerebral fissures.

Adhesive arachnoiditis after percutaneous fibrin glue treatment of a sacral meningeal cyst.

The authors present the case of a 64-year-old woman who was referred for severe sacral pain. She reported that her pain had been longstanding, and had greatly increased after percutaneous fibrin glue placement therapy for a sacral meningeal cyst 2 months earlier at a different hospital. An MRI scan obtained immediately after fibrin glue placement at that hospital suggested that fibrin glue had migrated superiorly into the subarachnoid space from the sacral cyst to the level of L-4. On admission to the authors' institution, physical examination demonstrated no abnormal findings except for peria…

Lateral recumbent head-down posture for epidural catheter insertion reduces intravascular injection.

PURPOSE: The unintentional and unrecognized cannulation of an extradural vein is a potentially serious complication of an epidural anesthetic. The present study was undertaken to assess the incidence of blood vessel puncture related to epidural catheterization performed in three different body positions. METHODS: The study was conducted in 900 (three groups of 300) obstetric patients undergoing continuous epidural analgesia during their labour and who were randomly allocated to three groups. Epidural catheterization was performed with patients in the sitting, lateral recumbent horizontal, or

Lateral sinus thrombosis in neurocysticercosis.

Cysticercosis is a frequent parasitic infection in developing countries including Nepal and is related to poverty, ignorance and pig-rearing practices at home in the community. A variety of structural involvements such as brain parenchyma, cerebellum, ventricular system, brain stem, meninges, subarachnoid space, spinal cord and orbit have been reported, but cysticercus involving the lateral sinus causing thrombosis is extremely rare.
