Non-invasive diagnosis of internal carotid artery dissections.

Arteriography is thought to be mandatory for the diagnosis of internal carotid artery (ICA) dissection. With the introduction of transcranial Doppler sonography (TCD) and magnetic resonance imaging (MRI), however, this is no longer the case. In 13 consecutive patients with ICA dissections the diagnosis was made by means of non-invasive tests including extracranial and transcranial Doppler sonography, contrast enhanced computed tomography (ceCT), and, in five patients, MRI. Intra-arterial digital subtraction angiography used as the gold standard in all cases was confirmative. Extracranial and t

Carotid body tumor: flow sensitive pulse sequences and MR angiography.

The magnetic resonance (MR) appearance of carotid body tumors has been previously described. Classically, these lesions are well circumscribed and highly vascular and cause a characteristic separation of the external and internal carotid arteries. Hypointense foci within these tumors are seen on T1-weighted images. The T2-weighted images snow a "salt and pepper" pattern. We report a carotid bifurcation glomus tumor that exhibited the above features on conventional MR. Multiplanar gadolinium enhanced MR as well as flow-sensitive MR techniques including small flip angle gradient refocused images

Breast MRI Physics Essentials: Signal Behavior, Suppression Approaches, and Appearance of Tissue and Implants.

Breast MRI provides the highest sensitivity for breast cancer detection and is widely used for high-risk screening, assessment of disease extent, treatment monitoring, and evaluation of silicone implant integrity. Despite the central role of fat suppression and tissue-specific signal behavior in determining image quality, the underlying physics principles and mechanisms of failure for these techniques are often underrecognized in clinical practice. This review provides a practical primer on MR signal behavior in commonly used T1 and T2weighted breast MRI sequences and outlines the major techni

High-resolution 7-Tesla magnetic resonance imaging and post-processing for 3-dimensional reconstruction of the membranous labyrinth in healthy adults.

The membranous labyrinth of the inner ear is a complex network of endolymph-filled structures critical for auditory and vestibular function. Pathological distension of these spaces, termed endolymphatic hydrops (EH), is associated with disorders such as Ménière's disease (MD). However, diagnosing inner ear pathologies remains challenging due to limitations in traditional imaging techniques, which lack the spatial resolution required to assess these intricate structures. Advances in 7-Tesla (7T) magnetic resonance imaging (MRI) now allow for high-resolution visualization of the inner ear. In th

Advanced imaging of head and neck infections.

When head and neck infection is suspected, appropriate imaging contributes to treatment decisions and prognosis. While contrast-enhanced CT is the standard imaging modality for evaluating head and neck infections, MRI can better characterize the skull base, intracranial involvement, and osteomyelitis, implying that these are complementary techniques for a comprehensive assessment. Both CT and MRI are useful in the evaluation of abscesses and thrombophlebitis, while MRI is especially useful in the evaluation of intracranial inflammatory spread/abscess formation, differentiation of abscess from

Advanced vascular imaging techniques.

This chapter outlines recent advances in imaging of disorders of the cervicocerebral vasculature that permit evaluation of the lumen, the vessel wall, and the patterns of blood flow within the vessel. Noninvasive MR techniques to evaluate the vessel lumen, such as noncontrast time-of-flight magnetic resonance angiography and contrast-enhanced magnetic resonance angiography (CEMRA) are routinely used in diagnosis, planning, and posttreatment follow-up. More recently, high-resolution vessel wall imaging MRI (VWMRI) has been developed, which provides additional information about the vessel wall o

[Current advances in spinal vascular disease].

During treatment of spinal arteriovenous malformations (AVMs), such as dural arteriovenous fistula (AVF), perimedullary AVF, and intramedullary AVM, it is often difficult to identify the feeding artery and draining vein because of presence of multiple dilated veins in the dural space and multiple shunts. Disruption of incorrect vessels may elicit major functional deficits in the spinal cord. To avoid such complications, it is extremely important to identify the correct vessels prior to and during surgical treatment. Recently, several types of techniques have been developed for diagnosis and tr

A comparison of three techniques to estimate the human dark-adapted cone electroretinogram.

The dark-adapted cone electroretinogram (ERG) is difficult to isolate because of unwanted rod intrusion. We compare dark-adapted cone estimates derived using three techniques. The first uses the cone response on a moderate rod saturating background to estimate the dark-adapted cone response. The second uses red and blue flashes to tease apart cone and rod responses (red-minus-blue technique, [Investigative Ophthalmology and Visual Science 31 (1990) 2283]). The third uses a bright flash to temporarily saturate rods, followed by a test flash that generates a putative cone-only response (2-flash

Pedicle subtraction osteotomy with patient-specific instruments.

BACKGROUND: Although the utility of patient-specific instruments (PSI) has been well established for complex osteotomies in orthopedic surgery, it is yet to be comparatively analyzed for complex spinal deformity correction, such as pedicle subtraction osteotomy (PSO). METHODS: Six thoracolumbar human cadavers were used to perform nine PSOs using the free-hand (FH) technique and nine with PSI (in total 18 PSOs). Osteotomy planes were planned on the basis of preoperative computed tomography (CT). A closing-wedge angle of 30° was targeted for each PSO. Postoperative CT scans were obtained to mea

Concordance rate differences of 3 noninvasive imaging techniques to measure carotid stenosis in clinical routine practice: results of the CARMEDAS multicenter study.

BACKGROUND AND PURPOSE: To replace digital subtraction angiography (DSA) in carotid stenosis evaluation, noninvasive imaging techniques have to reach a high concordance rate. Our purpose is to compare the concordance rates of contrast-enhanced MR angiography (CEMRA) and CT angiography (CTA) with Doppler ultrasound (DUS) in clinical routine practice. METHODS: We evaluated prospectively with DUS, CEMRA, and CTA 150 patients suspected of carotid stenosis. The overall concordance rates of the 3 techniques were calculated for symptomatic stenosis > or =50% and > or =70%, for asymptomatic stenosis

Evaluation of root resorption following rapid maxillary expansion using cone-beam computed tomography.

OBJECTIVE: To evaluate the root resorption after rapid maxillary expansion (RME) via cone-beam computed tomography (CBCT). MATERIALS AND METHODS: Records of 25 patients who had undergone RME with tooth-borne banded expander were obtained from the archive of the orthodontic department. CBCT data were reconstructed with surface and volume rendering, and the volumetric images were manipulated to display the root surfaces from various orientations. On these three-dimensional images, permanent first molars and first and second premolars were segmented, and their roots were isolated. Volumes of roo

Density value means in the evaluation of external apical root resorption: an in vitro study for early detection in orthodontic case simulations.

OBJECTIVES: The purpose of this study was to develop an alternative diagnostic tool for the early detection of external apical root resorption (EARR). METHODS: Mandibular incisors (n=36) with and without simulated EARR lesions were used. 18 teeth with facial and proximal windows, each with a range of 2 sizes, were placed in 6 N hydrochloric acid (HCl) baths for 10 min. A sample of the acid solution was analysed for calcium concentration by atomic absorption spectrophotometry. Incisors were imaged at 80 degrees, 90 degrees and 100 degrees under 3 test conditions (bracketed, non-bracketed and w

Resection of parathyroid tumor in the aorticopulmonary window without prior neck exploration.

Of 522 patients with hyperparathyroidism operated on from 1973 to 1987 at our institution, there were seven (1.3%), each with an ectopic, hyperfunctioning mediastinal parathyroid adenoma, who required median sternotomy. In three of these seven patients, the tumor was located in the aorticopulmonary window. A 61-year-old woman with primary hyperparathyroidism had a preoperative thallium-technetium subtraction scan that showed thallium uptake at the base of the heart without any uptake in the neck. After further workup and without prior neck exploration, a parathyroid adenoma was found in the ao

MR angiography in carotid stenosis: a comparison of three techniques.

PURPOSE: To compare the accuracy of three different magnetic resonance angiography (MRA) techniques for studying steno-occlusive disease of carotid arteries. METHODS: 64 patients were evaluated with three MRA techniques- three-dimensional (3D) time-of-flight (TOF), two-dimensional (2D) TOF, and 3D Phase-Contrast (PC); the acquisition was in the axial plane, the volume included the carotid bifurcation. Digital subtraction angiography (DSA) was considered the 'gold standard'. The MRA images were reprojected with a maximum intensity pixel ray-tracing (MIP) algorithm. The three MRA techniques wer

Evaluating and Managing the Painful Total Ankle Replacement.

The number of total ankle replacements (TARs) performed in the United States has dramatically increased in the past 2 decades due to improvements in implant design and surgical technique. Yet as the prevalence of TAR increases, so does the likelihood of encountering complications and the need for further surgery. Patients with new-onset or persistent pain after TAR should be approached systematically to identify the cause: infection, fracture, loosening/subsidence, cysts/osteolysis, impingement, and nerve injury. The alignment of the foot and ankle must also be reassessed, as malalignment or a

Advanced imaging in acute ischemic stroke.

The evaluation and management of acute ischemic stroke has primarily relied on the use of conventional CT and MRI techniques as well as lumen imaging sequences such as CT angiography (CTA) and MR angiography (MRA). Several newer or less-established imaging modalities, including vessel wall MRI, transcranial Doppler ultrasonography, and 4D CTA and MRA, are being developed to complement conventional CT and MRI techniques. Vessel wall MRI provides high-resolution analysis of both extracranial and intracranial vasculature to help identify previously occult lesions or characteristics of lesions tha

Treatment of Madelung's deformity.

Treatment of Madelung's deformity is still controversial. We reviewed retrospectively 19 patients with Madelung's deformity (two bilateral, 21 cases) who underwent surgery to the radius and ulna to improve range of motion, decrease pain and improve appearance of the wrist. Nineteen patients underwent 21 distal radial osteotomy procedures using three different techniques: subtraction, addition or dome osteotomy. Ulnar shortening and redirection of the distal ulna was performed in 12 cases; a long oblique osteotomy was used in 10 of these cases. The Sauvé-Kapandji technique was performed in five

Role of preoperative localization in the management of primary hyperparathyroidism.

BACKGROUND: The advantages of preoperative localization in the management of primary hyperparathyroidism have not been clearly demonstrated. The aim of this study was to investigate prospectively the accuracy of three localization techniques in patients with this condition. METHODS: Forty-nine consecutive patients with primary hyperparathyroidism underwent ultrasonography, magnetic resonance imaging (MRI) and technetium-thallium (Tc-Tl) subtraction scanning before surgery, during which an attempt was made to identify all parathyroid glands. A scan was regarded as correct if it identified an e

The role of pulse-wave Doppler sonography in quantifying internal carotid stenosis.

To determine the accuracy of pulse-wave Doppler (PWD) sonography in quantifying cervical internal carotid artery (ICA) stenosis, we compared results of PWD with conventional digital subtraction angiography (DSA). A total of 202 carotid arteries were evaluated by means of these two techniques. The patients were divided into four groups according to the percentage of angiographic carotid stenosis as suggested by NASCET. A mathematic model of quadratic relationship between DSA stenosis and blood flow velocity, recorded by PWD, was worked out; peak systolic velocity and mean velocity values, corre
