A Novel Therapeutic Method in Gastro-esophageal Reflux Disease.

BACKGROUND: Gastro-esophageal reflux disease (GERD/GORD) is a chronic condition in which gastric acid flows backwards up into the esophagus, causing heart burn and a higher disposition to esophageal cancer. The reflux is caused by impairment of the lower esophageal sphincter (LES). Over the past century gastro-esophageal reflux has become the principal gastrointestinal condition of our time. The proton pump inhibitor class of drugs is effective in ameliorating the symptoms of reflux. The cost of investigation of patients in Europe is €100 billion per annum. The cost in days lost from work is €

Recurrent Cochlear Implant Electrode Exposure in a Smoker With Prior Canal Wall Mastoidectomy: A Case Report.

Cochlear implantation (CI) is an established and effective surgical intervention for patients with severe to profound sensorineural hearing loss. The procedure involves placement of an internal receiver-stimulator in a postauricular subperiosteal pocket with an electrode advanced through the mastoid and facial recess into the cochlea. Long-term device function depends on secure fixation and durable soft tissue coverage over both the implant and extracochlear electrode segment to prevent skin breakdown, infection, and electrode exposure. Although electrode exposure is uncommon, it represents a

A custom mandibular distraction device for the rat.

In the last several years, the rat model has been used as a reliable means of studying distraction osteogenesis (DO). This model has several benefits, including lower cost, the availability of molecular reagents for the rat, and the ability to perform a large number of operations in a short time. We have recently developed an external device that is custom-made for the rat mandible and secured at the mandibular ramus. The device consists of a Leone (Florence, Italy) jackscrew distraction assembly (0.2 mm per one-quarter turn), 2 Luhr L-shaped 0.8-mm five-hole microplates, and four 0.8-mm x 3-m

Endoscopic evaluation of a complex ballistic injury.

The current conflict in Afghanistan is characterised by significant injuries resulting from the use of Improvised Explosive Devices. Increasing survivability from battlefield injury, escalating musculoskeletal ballistic trauma and the use of blast weaponry combine to produce an injury profile which defines contemporary combat casualty care. Such complex multi-system trauma challenges current wound care rationale. Ballistic injury of the perineum, often associated with proximal femoral injury and significant tissue loss, raises particular management difficulties. These cases demand an individua

Percutaneous endoscopic sigmoidopexy for recurrent sigmoid volvulus in high-risk adults: a systematic review.

BACKGROUND: Recurrent sigmoid volvulus in frail, comorbid adults recurs frequently after detorsion and carries significant risks with resectional surgery. Percutaneous endoscopic sigmoidopexy (PES), defined as fixation of the sigmoid colon to the abdominal wall without an indwelling tube, has been proposed as a tube-free, minimally invasive alternative. OBJECTIVE: To synthesise outcomes of strict PES (fixation-only; no tube) for recurrent sigmoid volvulus in adults at high surgical risk. METHODS: PRISMA-guided review (2000-31 July 2025) registered prospectively with PROSPERO CRD420251111664.

Comparison of surgical outcomes of osteosynthesis using anatomical locking plates with proximal screws and smooth pegs for proximal humeral fractures.

BACKGROUND: Plate fixation is a widely used surgical procedure for proximal humeral fracture; however, relatively high complication rates have been reported. To prevent the occurrence of proximal screw cut out, pegs with blunt tips have been introduced to replace traditional screws for fixation of the humeral head. However, few reports are available on the effect of using smooth pegs on surgical outcomes. In this study, we aimed to compare the postoperative outcomes of osteosynthesis using anatomical plates using all proximal screws and using all proximal pegs. METHODS: We retrospectively iden

Retrospective study on the outcomes of Fassier-Duval nailing and osteotomy for the treatment of long bone fractures or deformities in the lower extremities in children with osteogenesis imperfecta.

BACKGROUND: Osteogenesis imperfecta (OI) is a rare hereditary disorder characterized by bone fragility and deformity, frequently requiring surgical intervention to restore limb alignment and stability. Traditional intramedullary fixation devices are limited in growing children. The Fassier-Duval (FD) nail, designed to elongate with growth, offers potential advantages when combined with corrective osteotomy. This study retrospectively evaluated the clinical outcomes of FD nail fixation with osteotomy for long bone fractures and deformities in children with OI. METHODS: We retrospectively revie

Evaluation of Post-Operative Outcomes of Femoral Neck Fracture Interventions: A Systematic Review.

INTRODUCTION: Femoral neck fractures (FNF) represent a significant challenge in orthopedic practice, demanding prompt intervention to restore function and mobility in affected individuals. Numerous surgical interventions have been developed to address these fractures, including internal fixation with devices such as the Femoral Neck System (FNS, DePuy Synthes, Inc., West Chester, PA, USA). However, the optimal fixation system remains unclear. Understanding the postoperative outcomes associated with these interventions is crucial for optimizing patient care and informing treatment decisions. S

Comparing Perioperative Outcome Measures of the Dynamic Hip Screw and the Femoral Neck System.

Background and Objective: Various fixation devices and surgical techniques are available for the management of proximal femur fractures. Recently, the femoral neck system (FNS) was introduced, and was promoted on the basis of less invasiveness, shorter operating time, and less fluoroscopy time compared to previous systems. The aim of this study was to compare two systems for the internal fixation of femoral neck fractures (FNF), namely the dynamic hip screw (DHS) with an anti-rotation screw (ARS) and an FNS. The outcome measures included operating room time (ORT), dose−area product (DAP), leng

Economic impact of syndesmosis hardware removal.

Ankle syndesmosis injuries are commonly seen with 5-10% of sprains and 10% of ankle fractures involving injury to the ankle syndesmosis. Anatomic reduction has been shown to be the most important predictor of clinical outcomes. Optimal surgical management has been a subject of debate in the literature. The method of fixation, number of screws, screw size, and number of cortices are all controversial. Postoperative hardware removal has also been widely debated in the literature. Some surgeons advocate for elective hardware removal prior to resuming full weightbearing. Returning to the operating

Progressive Brachial Plexus Palsy after Osteosynthesis of an Inveterate Clavicular Fracture.

INTRODUCTION: The thoracic outlet syndrome (TOS) is a rare complication of clavicular fracture, occurring in 0.5-9% of cases. In the literature from 1965 - 2010, 425 cases of TOS complicating a claviclular fracture were described. However, only 5 were observed after a surgical procedure of reduction and fixation. The causes of this complication were due to the presence of an exuberant callus, to technical surgery errors or to vascular lesions. In this paper we describe a case of brachial plexus plasy after osteosynthesis of clavicle fracture. CASE REPORT: A 48 year old female, presented to us

A Novel Hybrid Training Model for Open Fracture Management in Rwanda.

➢ Open fractures are a critical global health challenge that disproportionately affect individuals in low- and middle-income countries (LMICs), primarily due to road traffic collisions. Surgical management of open fractures is 1 of the 3 essential bellwether procedures identified by The Lancet Commission on Global Surgery. ➢ We developed and evaluated a novel hybrid course on open fracture management for surgical trainees and practicing surgeons in Rwanda, combining a self-directed, virtual, pre-course curriculum with a live, in-person workshop in Kigali in June 2025 that was simultaneously li

Posterior Fixation Without Debridement for Vertebral Body Osteomyelitis and Discitis: A 10-Year Retrospective Review.

BACKGROUND: Surgical treatment of vertebral osteomyelitis, discitis, and epidural abscesses is indicated in the setting of failure of antibiotic therapy, neurological deficits, epidural abscess, or spinal instability/deformity. Historically, surgical treatment mandated aggressive debridement and spinal stabilization. However, there is growing evidence that direct debridement may not be necessary and may contribute to morbidity. The purpose of this study was to evaluate the efficacy of posterior instrumentation without debridement in treating spinal infections. METHODS: A retrospective medical
