Swallowing dysfunction in patients receiving prolonged mechanical ventilation.

Several studies have suggested that swallowing dysfunction and pulmonary aspiration occur in patients receiving prolonged ventilation. However, the incidence of swallowing dysfunction, its rate of resolution, and the sensitivity of tests used to characterize swallowing abnormalities are not well defined. The goals of our study were to evaluate swallowing function in this group of patients by (1) defining the specific swallowing abnormalities that occur in this patient population, (2) comparing the sensitivity of bedside evaluations to modified barium swallow with videofluoroscopy (MBS/VF), (3)

Subglottic air pressure: a key component of swallowing efficiency.

The relationship between tracheostomy and swallowing dysfunction has been long recognized. Often this dysfunction is manifested by aspiration, for which a number of etiologic factors may be responsible. Disruption of glottic closure has been previously demonstrated in association with the presence of an indwelling tracheostomy tube. The plugging or removal of the tracheostomy tube, or the use of an expiratory air valve, has been demonstrated to decrease aspiration and improve swallowing function. Measurement of subglottic pressure through an indwelling tracheostomy tube during swallowing demon

Modified barium swallow examination in dysphagic stroke patients.

UNLABELLED: Aspiration in dysphagic stroke patients is common and causes significant morbidity and mortality. Bedside clinical assessment of aspiration risk is often unreliable, particularly in patients with silent aspiration. Twenty-five dysphagic stroke patients clinically assessed to have borderline risk of aspiration were studied using a Modified Barium Swallow (MBS) examination. MBS revealed definite aspiration of liquids in 6 patients (24%) and semisolids in 4 patients (16%). Combined clinical and MBS assessment revealed an additional 12 patients (48%) had problems swallowing liquids and

Tongue thrust swallowing in severely physically disabled children.

In five patients with severe physical and mental disabilities, a feeding pattern with tongue thrust was assessed videofluoroscopically. Forward and backward movements of the tongue associated with movements of the mandible propelled the bolus, and the insufficiently formed bolus spilled out anteriorly and laterally in part. The swallowing reflex was accompanied by tongue thrust. After the reflex, a part of the bolus frequently remained on the vallecula and pyriform sinuses. The suckle pattern shown by infants is suggested to remain and develop into this feeding pattern with tongue thrust.

Mechanism and timing of nasopharyngeal closure during swallowing and belching.

The mechanism(s) of nasopharyngeal closure (NPC) and its temporal relationship with other biomechanical events during swallowing and belching were studied in seven healthy volunteers, aged 26-39 yr, by concurrent videoendoscopic, videofluoroscopic, and manometric technique. Analysis of the videoendoscopic recordings showed that deglutitive NPC consisted of elevation of the soft palate and adduction of the superior pharyngeal constrictor muscle. Videofluoroscopy identified only the palatal elevation clearly. During belching, however, only palatal elevation occurred. Deglutitive NPC ranged betwe

The application of color Doppler technology to the study of fetal swallowing.

OBJECTIVE: To investigate the mechanics of fetal swallowing using color Doppler technology. METHODS: The fetal face and a profile were imaged sagitally in ten fetuses at 36-41 weeks' gestation. Color-flow Doppler signals were superimposed on each gray-scale image to assess the direction and location of amniotic fluid (AF) flow with respect to anatomic structures of the fetal upper gastrointestinal and respiratory tracts during the act of swallowing. Further analysis of fetal swallowing movements was performed by a review of the ultrasonography videotapes at a slow speed. RESULTS: An initial

Swallowing performance in patients with vocal fold motion impairment.

Twenty-seven patients with vocal fold motion impairment underwent detailed pharyngoesophagel manometry with a strain gauge assembly linked to a computer recorder. Nine were known to have lesions of the central vagal trunk or nucleus, 9 had recurrent laryngeal nerve (RLN) palsy, and the remainder were idiopathic. The site of the lesion was a more important determinant of subjective swallowing performance than the position of the involved cord at laryngoscopy. Patients with central lesions had lower tonic and contraction upper esophageal sphincter (UES) pressures than 25 age-matched controls, su

The location and tracking of swallowed dental appliances: the role of radiology.

Two cases of swallowing mishaps, one involving a spoon-denture and the other an orthodontic partial arch wire, are reported. The former was recovered following interventional radiology, the latter by proctoscopy. The role of radiographic investigation and possible measures to prevent such potential life-threatening emergencies are discussed.

Activation and coordination patterns of the suprahyoid muscles during swallowing.

This study was undertaken to determine the activation and coordination patterns of the three suprahyoid muscles--geniohyoid, mylohyoid, and anterior belly of the digastric muscle--in elevating the larynx during swallowing. Electromyographic activity was also recorded from two intrinsic laryngeal muscles (vocalis and lateral cricoarytenoid) and the anterior genioglossus. Ten adults served as participants. Each participant produced 15 swallows of 15 mL of tap water both normally and with a 12-mm bite block placed between the molars. The electromyographic data were ensemble-averaged with a labora…

Swallowing difficulty in primary progressive aphasia: a case report.

A 71-year-old right handed man showed insidious and progressive decline of language skills, in contrast with relatively preserved performance on tests and ability to be independent in daily living. Three years later, he developed swallowing difficulties. MRI scans showed widening of the left sylvian fissure. A SPECT scan demonstrated focal hypometabolism restricted to the left temporal lobe. Videofluoroesophagography revealed marked hesitation of oral preparation and oral phases, while the reflex phases were unimpaired. The patient failed to perform oral tasks on command and imitation without

Incoordination of sucking, swallowing, and breathing as an etiology for breastfeeding difficulty.

This case report describes an infant who presented with coughing and choking during breastfeeding. Comprehensive clinical evaluation of these difficulties included assessment of sucking, swallowing, and breathing. Stridor and difficulty interspersing breaths during sucking bursts were observed, although sucking mechanics were normal. Further evaluation of airway structure revealed asymmetric laryngomalacia. This structural problem, exacerbated by the breastfeeding position, impeded respiration and resulted in poor coordination of sucking, swallowing, and breathing. This case underscores the ne

Severe complications following a "barium swallow" investigation for dysphagia.

OBJECTIVE: To highlight the risks of investigation of patients with swallowing disorders by "barium swallow", when the disorders may arise from dysfunction of the upper swallowing tract. CLINICAL FEATURES: An 81-year-old Italian woman presented to her local doctor with a history of dysphagia of five days' duration. A barium swallow resulted in aspiration of a large amount of barium into the right main bronchus, causing severe problems. After a period of intensive respiratory care including intubation and continuous positive airway pressure, she recovered sufficiently to be referred to a rehab

Art and science of history taking in the patient with difficulty swallowing.

History taking is the first step in the evaluation of a patient. An analysis of the information obtained provides the basis for the choice and order of diagnostic tests. In addition, it provides the clinician with the necessary information to determine the relevance of "abnormal tests" to the patient's problem. Dysphagia is a reliable symptom that indicates an abnormality in the swallowing mechanism. The history should contain a detailed description of the symptoms associated with dysphagia from the onset. Especially relevant are questions to determine if dysphagia is experienced every day or

Barium swallow is a predictor of salivary fistula following laryngectomy.

Pharyngocutaneous salivary fistula after laryngectomy is a serious complication that can lead to prolonged hospitalization and increased patient morbidity. A postoperative barium swallow provides the surgeon with information regarding the integrity of the pharyngeal suture line. In an attempt to determine whether this information can be used to predict or prevent salivary fistula, we reviewed the records of 109 patients who underwent total laryngectomy, including 51 who had a barium swallow before they began oral intake. Ten patients (20%) demonstrated a sinus tract originating from the pharyn

[Subclinical swallowing disorder in patients with abnormal sensation in the throat].

Measurement of swallowing pressure was done in 75 patients who complained of abnormal sensation such as obstructive sensation, difficulty in swallowing, etc. in the throat without noticeable cause. Abnormal swallowing pressure curves were observed in 33 patients (44%), and 76% of them showed abnormal elevation of the pressure at the hypopharyngeal or cervical esophageal region. These results indicated that subclinical swallowing disorder, particularly abnormal high swallowing pressure, can be a possible cause of their complaints. The author assumed that abnormal elevation of swallowing pressur

Mis-swallowing of cleaning naphtha: report of three cases.

Cleaning naphtha, a product of Chinese Petroleum Corporation, is a complex hydrocarbon mixture which contains mainly aliphatic hydrocarbons. It is used as a cleaning fluid and solvent. Three toddlers mis-swallowed a mouth of cleaning naphtha accidentally. One developed multiple organ failure before death, another completely recovered without sequelae, and the other died very soon after mis-swallowing. Two fatal cases were both induced vomiting with aspiration immediately after mis-swallowing by their parents. Therefore, inducing vomiting should not be encouraged especially at home, and prevent

Esophageal tuberculosis: findings on barium swallow and computed tomography.

Esophageal tuberculosis secondary to tuberculous mediastinal lymphadenopathy is a very unusual presentation of adult tuberculosis. We report a patient presenting with fever of unknown origin and dysphagia. Barium swallow demonstrated esophageal displacement, mucosal ulceration and perforation with a fistulous tract into the mediastinum. CT of the mediastinum gave the most complete delineation of the tuberculous mediastinal lymphadenopathy which surrounded and displaced the esophagus. The fistulous tract extending from the esophagus into the nodal mass was also seen on CT.
