[Clinical results of arthroscopic treatment for localized pigmented villonodular synovitis of knee].

OBJECTIVE: To explore the technique of arthroscopic treatment of localized pigmented villonodular synovitis of the knee and to evaluate its clinical results. METHODS: From February to December 2006, 22 cases of localized pigmented villonodular synovitis of the knee were treated by arthroscopic excision of the focus and partial synovectomy. There were 8 males and 14 females, with an average age of 24 years old (16 to 35 years old). Eight patients had a trauma history, the others had no obvious inducement. The disease course was from 1 month to 30 months with an average of 10 months. The Lyshol…

Pigmented villonodular synovitis of the talonavicular joint: a case report and review of the literature.

Pigmented villonodular synovitis (PVNS) is a locally aggressive synovial proliferative disorder of unknown aetiology affecting the linings of joints, tendon sheaths, and bursae. A 22-year-old female patient presented with a 3-year history of an increasingly painful swelling on the dorsum of her right foot. Examination revealed a 4 cm x 2 cm swelling that was fluctuant, tender on palpation, unattached to overlying skin and partially mobile. A firm, pedunculated intra-articular lesion from the talonavicular joint was removed at surgery. Histology revealed a nodular lesion of stromal cells and nu…

[Juvenile idiopathic arthritis with dry synovitis: clinical case and review of literature].

Juvenile idiopathic arthritis is a term that encompasses all forms of arthritis that begin before the age of 16 years, persist for more than 6 weeks and are of unknown cause. Dry synovitis is still not completely understood nor commonly described. It is associated with juvenile idiopathic arthritis and must be considered in patients with minimal swelling but pain and stiffness along with flexion contractures as well as other evidence of an inflammatory process (lab changes and/or other symptoms, such as uveitis or rash), and often follow a destructive course. The authors present a case of a br…

Liposome-based diclofenac for the treatment of inflammation in an acute synovitis model in horses.

Lameness as a result of joint disease is a major source of decreased athletic performance in the horse. Most treatment protocols include the administration of nonsteroidal anti-inflammatory drugs (NSAIDs). Phenylbutazone, alone or in combination with other treatments, is the most commonly and widely used NSAID, however it has the potential for serious side effects. The introduction of the liposome-based formulation of the NSAID diclofenac has shown promising effect as a safe and convenient treatment for lameness associated with osteoarthritis. The purpose of this study was to evaluate the effe

Use of polymethylmethacrylate to treat pigmented villonodular synovitis in the posterior compartment of the knee.

Pigmented villonodular synovitis (PVNS) is a benign process that mainly affects the knee joint. There are two types of PVNS, a localised and a diffuse form. Although adjuvant therapies are possible, the treatment consists of arthroscopic or open synovectomy of the affected area. The most common complication is local recurrence. We report the case of a patient with PVNS and osteolysis of the femoral condyle, treated with anterior arthroscopic synovectomy and posterior approach for the treatment of the femoral condyle erosion. The bone erosion was treated with polymethylmethacrylate (PMMA) cemen…

Pigmented villonodular synovitis in children: a report of six cases and review of the literature.

We report six children with pigmented villonodular synovitis. They ranged in age from seven to fifteen years. In four patients, the knee was involved. One patient had involvement of the ankle, and one had diffuse involvement along a metacarpal. In five cases, the diagnosis was not suspected clinically or radiographically, and the delay in making the correct diagnosis was as long as two years. Clinical diagnosis in these five patients was usually bacterial synovitis or juvenile rheumatoid arthritis. We feel that the diagnoses of pigmented villonodular synovitis should be considered in any child…

Radiosynovectomy in hemophilic synovitis: correlation of therapeutic response and blood-pool changes.

AIM: The aim of this study was to evaluate the efficacy of 90Y and 186Re radiosynovectomy in patients with hemophilic synovitis. METHODS: Radiosynovectomy was performed in 32 joints of 20 patients with hemophilic synovitis by using 90Y citrate colloid in the knee and 186Re sulfide colloid in the elbow, shoulder, and ankle. The indication for radiosynovectomy was the continuous presence of intra-articular blood or effusion and three or more hemorrhages into the same joint within the last 6 months. Response to therapy was first assessed at the 4th month with blood-pool imaging. Patients were fo…

Grading of chronic synovitis--a histopathological grading system for molecular and diagnostic pathology.

The following is a proposition for a simple histopathological classification system to measure inflammation in synovial tissue. This synovitis-score is employed in conventionally stained routine sections, and is applicable to every kind of synovitis, irrespective of etiology and including the following relevant morphological alterations. First: hyperplasia/enlargement of synovial lining cell layer. Second: activation of resident cells/synovial stroma. Third: inflammatory infiltration. All defined histopathological qualities are graded from absent (0), slight (1) and moderate (2) to strong (3),…

[Diagnosis and surgical therapy of diffuse pigmented villonodular synovitis of the hip joint].

Pigmented villonodular synovitis (PVNS) of the hip is a monoarticular proliferative process involving the synovial membrane. A chronic inflammation as well as a neoplastic process have been proposed in the literature. PVNS is usually found in adults aged 20-50 years, without sex predilection. The knee is by far the commonest location, followed by the hip. We present a detailed case report of a 25-year-old man with PVNS of the hip. The physical examination was completely normal. Radiographs of the hip show erosions in the head and neck of the femur and the acetabulum. Magnetic resonance imaging…

Chronic synovitis caused by a date palm thorn: An unusual clinical picture.

Synovitis related to plant thorns is a rare pathology. As in this case, thorn prick injury, which is particularly caused by thin palm thorns, is frequently missed in clinical history. Clinical presentation of cases may simulate juvenile rheumatoid arthritis with monoarticular involvement and septic arthritis. Removal of all thorns with partial or total synovectomy is essential in definitive treatment. Arthroscopy is the most valuable method of meeting both diagnostic and therapeutic purposes as it provides the best visibility of the joint with lower morbidity.

The clinical entity of remitting seronegative synovitis with pitting edema (RS3PE syndrome).

Remitting seronegative symmetrical synovitis with pitting edema is a clinical entity distinct from polymyalgia rheumatica arthritis appearing in elderly patients. Distal pitting edema and tenosynovitis of the digitorum tendons are characteristically seen in this disorder. In contrast to polymyalgia rheumatica and rheumatoid arthritis, treatment with low-dose glucocorticoids usually produces a prompt and enduring remission after several months. Follow-up evaluations are necessary not only to assure this diagnosis is posterior, but also to observe for the possible occurrence of associated neopla…

[Villonodular synovitis of the knee in a 5-year-old child. Apropos of a case].

PURPOSE OF THE STUDY: Case report of a five-year-old boy presenting with a painless swelling of the left knee with decreased range of motion. METHOD: Joint aspiration: fluid rich in red blood cells. X-ray--CT-scan--MRI: pigmented villonodular synovitis suspected. Arthroscopy with synovectomy and biopsy confirmed diagnosis. Because of remaining swelling one month after arthroscopy, intra-articular injection of triamcinolone hexacetonide gave a good result which has been maintained after 2 years. DISCUSSION: Hypothesis on etiology and pathophysiology, differential diagnosis and treatment of Pi…

Lead "bursogram" and focal synovitis secondary to a retained intraarticular bullet fragment.

We present an unusual case in which retained lead bullet in the wrist joint dissolved and migrated into a synovial recess, producing reactive focal synovitis. The clinical presentation suggested a soft tissue mass or ganglion cyst. The radiologic findings of dense metallic fragments within a recess of the wrist joint established the correct diagnosis and excluded other causes of soft tissue masses. Radiologic recognition of retained intra-articular lead fragments is important, as the clinical features of systemic lead intoxication, lead arthopathy, or reactive focal synovitis may be subtle or …

Pigmented villonodular synovitis of the temporomandibular joint. Case report and review of the literature.

A case of pigmented villonodular synovitis of the right temporomandibular joint with extension through the base of the skull into the middle cranial fossa is described. The patient initially had right-sided hearing loss. Physical examination revealed a mass in the right ear canal. Computerized axial tomography demonstrated a destructive lesion of the right temporomandibular joint, with extension into the middle cranial fossa. The surgical approach is discussed, and the 11 previously reported cases are reviewed.

Incidence and management of transient synovitis of the hip: a study in Dutch general practice.

Little is known about transient synovitis of the hip in the community. Using data from the Dutch national survey of morbidity and interventions in general practice, a study was undertaken to look at the incidence and management of transient synovitis of the hip in children under 15 years of age. Transient synovitis of the hip was diagnosed in 19 children, 17 of whom were new cases. The mean age of the children was six years six months with a sex ratio of 2.8:1.0 boys to girls. An incidence rate of 1.1 per 1000 person years was calculated. General practitioners prescribed drug treatment for six…

Correlation between pain and synovitis in patients with internal derangement of the temporomandibular joint.

A correlation between pain scores by questionnaire and synovitis index via arthroscopic inspection was made in 28 patients with internal derangement of the temporomandibular joint (TMJ) with closed lock. A statistically significant but weak correlation was found between the visual analog scale of pain and the index of distribution of synovitis. However, in the questionnaire, pain on chewing had a positive correlation with both the intensity and distribution of synovitis in the TMJ. This study indicates that synovitis is one cause of TMJ pain in patients with closed lock.
