Ponseti Cast Removal: Video Technique.

INTRODUCTION: Ponseti casting method is the gold standard for the management of congenital talipes equinovarus in neonates, which consists of weekly manipulation and serial casting. Using oscillatory saw for cast removal is user-dependant and has been fraught with complications consisting of saw burns, thermal abrasions, anxiety, and fear of the saw (especially in children). The aim of this article is to describe our technique of cast removal using visual media format. TECHNIQUE: This video describes an easy method of cast soakage with lukewarm water. Once the cast is soaked in water, the kno…

Microcephaly/Trigonocephaly, Intellectual Disability, Autism Spectrum Disorder, and Atypical Dysmorphic Features in a Boy with Xp22.31 Duplication.

The Xp22.31 segment of the short arm of the human X chromosome is a region of high instability with frequent rearrangement. The duplication of this region has been found in healthy people as well as in individuals with varying degrees of neurological impairment. The incidence has been reported in a range of 0.4-0.44% of the patients with neurological impairment. Moreover, there is evidence that Xp22.31 duplication may cause a common phenotype including developmental delay, intellectual disability, feeding difficulty, autistic spectrum disorders, hypotonia, seizures, and talipes. We report on a…

Significance of fibrotic bands in utero--Amniotic band sequence with limb body wall complex: A rare case of fetal autopsy.

Amniotic band sequence (ABS) includes a wide spectrum of abnormalities resulting from entrapment of various fetal parts from a disrupted amn … for an autopsy. The fetus also showed scoliosis, gastroschisis, lumbosacral meningocele, congenital talipes equinovarus, and cleft palate, thus having features of placenta cranial and placenta abdominal phenotype which is very rare.

A case of infantile neuroaxonal dystrophy: connatal Seitelberger disease.

We present a male stillbirth with infantile neuroaxonal dystrophy (connatal Seitelberger disease). Following the development of polyhydramnios with an ultrasound scan showing severe distal arthrogryposis, the mother was induced at 38 weeks. A moderately macerated severely intrauterine growth restricted male stillbirth was delivered. External microcephaly, sloping forehead, simplified palmar skin creases, fixed flexion deformities of the knees, severe talipes equinovarus, spinal scoliosis, and empty scrotum were present. The brain was microcephalic with normal gyration, having a hypoplastic cor…

[Congenital myotonic dystrophy--the significance of a handshake].

Three neonates, all girls, were presented immediately after birth with severe hypotonia. Two of them needed artificial ventilation because of respiratory insufficiency. All three pregnancies had been complicated by reduced fetal movements and moderate cerebral ventricular dilatation and in two of the three there was also polyhydramnios and congenital talipes. In all three infants congenital myotonic dystrophy was suspected after diagnosing myotonia in the mother. This was done by observing that none of the mothers were unable to release their grip immediately on command after shaking hands. Op…

[Conservative treatment of clubfoot: the Functional Method and its long-term follow-up].

Despite the presence of a considerable number of papers published in the international literature, talipes equinovarus keeps its secrets. Conservative methods of treatment are currently accepted as advantageous over surgery. Prediction of the future of a clubfoot after treatment is not possible, for this reason, a long-term follow-up is needful. A true Functional Method based solely on gentle manipulations has been used for more than thirty years. Its aim is to correct the deformity and to allow the child to walk without delay. The principle of this method is to distract joint contractures pri…

We describe a very unusual combination, and previously unreported, of malformations in an 18-week, spontaneously aborted male fetus. The fetus had a reversed dorsoventral positioning of the head and upper limbs relative to the body axis with the head and both upper limbs directed dorsally, and an abrupt rotation of the vertebral bones at the level of CZ The fetus also had asplenia, single ventricle, and conotruncus. The fetus also had flexion deformities at the wrist, reduction deformity of the left second digit, anomalies in ossification of the bones of the left hand, and bilateral talipes ca…

[Compartment syndrome after impact trauma from a car bumper].

A 67-year-old woman, having been hit in the lower leg by a car bumper in a road accident, developed a compartment syndrome in the lower leg without any bone injury. In the following weeks typical contractures of the affected muscles occurred with talipes equinus and clawfoot deformities. In addition there developed insertion tendinitis of the affected muscle groups of the anterior tibia. A year later magnetic resonance imaging (with normal radiological findings) revealed fatty degeneration and muscle fibrosis. The patient complained of pain at rest and on movement in the proximal tibia, and th…

[Long-term results of Grice subtalar arthrodesis in children].

Talipes valgus, mostly resulted from poliomyelitis, of 44 feet in 43 children, aged 4 to 12 years, were treated by Grice extra-articular arthrodesis of the subtalar joint, and on 36 of them tendon transfer was done as well at the same time. The result in all of them, after a period of 15.3 years (ranging 5 to 23 yrs.), was found satisfactory in 32 feet (72.7%), and unsatisfactory in 12 (27.3%). Eight of the latter were found to be due to secondary varus deformity, a consequence of imbalance of muscle power following medial transfer of peroneal tendon (s). It is emphasized that in correcting fo…

A fetus with a chromosome 13 ring and placenta with chromosome 13 rod/ring mosaicism.

A fetus was identified by prenatal cytogenetic diagnosis as having a karyotype 46,XY,r(13) (p11q13). Termination of the pregnancy yielded a severely malformed fetus. Fetal abnormalities included anencephaly, imperforate anus and urethral meatus, severe talipes, syndactyly, cardiac defects and other anomalies. Confirmatory studies on cultured placental villi cells indicated a second cell line, 46,XY, -13,+13qter leads to cen::13q13 leads to qter. This cell line was not detectable in cells derived from the fetus despite extensive studies. It seems likely that the two cell lines arose simultaneou…

Morphological Similarities of Chronic Ankle Instability and Cavus Foot Type Using Statistical Shape Modeling.

Chronic ankle instability (CAI) can develop in up to 40% of patients after a first-time ankle sprain. Advanced imaging and statistical shape modeling (SSM) provide the opportunity to evaluate if subtle differences in foot and ankle morphology, such as hindfoot alignment or cavus features, may contribute to structural predispositions for recurrent instability. In this study, a 14-bone SSM was created from weight-bearing CT data (n = 80) from patients with CAI (n = 23), cavus foot type (n = 29), and rectus (n = 28) foot type. Scans for the CAI group were from pre-operative imaging of patients un

Functional, Radiological, and Biomechanical Outcomes Following Deformity-Driven Multilevel Surgical Correction for Fixed Cavovarus Foot Deformity.

BACKGROUND: Fixed cavovarus foot deformity is characterised by a plantar-flexed first ray, hindfoot varus, and abnormal plantar loading, often resulting in pain, instability, and gait dysfunction. The surgical correction we performed involved a deformity-driven approach to address the structural apex and associated components. Evidence linking this multilevel correction strategy to objective biomechanical improvement remains limited. METHODS: This prospective case series evaluated outcomes following a uniform deformity-driven multilevel surgical correction protocol for fixed cavovarus deformi

Talocalcaneonavicular Realignment: The Foundation for Comprehensive Reconstruction of Severe, Resistant Neurologic Cavovarus, and Equinocavovarus Foot Deformities in Children and Adolescents.

INTRODUCTION: The progression of neurologic cavovarus (NCV) and equinocavovarus (NECV) foot deformities during growth can eventually lead to non-reducible severe talonavicular (TN) and subtalar (ST) joint rotational malalignments. This study aimed to describe a technique to correct severe, resistant inversion deformity of the ST joint complex as the foundational procedure in comprehensive reconstruction of multi-segmental NCV and NECV deformities in pediatric patients and to analyze mid-term radiologic, functional, and satisfaction outcomes. METHODS: This prospective study included children w

The Association of Cavovarus Foot Deformity With an Os Peroneum.

ObjectiveWe sought to assess whether the presence of an os peroneum is correlated with cavovarus foot alignment in patients without a neurologic explanation for their foot shape. We hypothesized that a large os peroneum would increase the power of the peroneus longus and lead to a forefoot-driven, hindfoot varus deformity.Materials and MethodsThis was a retrospective cohort study conducted at a single institution and reviewed patients with 3 weightbearing views of the foot on plain radiography. Patients were characterized into having either no os peroneum (235), a small os peroneum (18), or a
