Disulfiram treatment increases plasma and red blood cell acetaldehyde in abstinent alcoholics.

BACKGROUND: Much of alcohol's toxicity is due to its product, acetaldehyde. The role of acetaldehyde derived from endogenous sources was assessed in alcoholic patients administered disulfiram, an inhibitor of aldehyde dehydrogenase. METHODS: The first part of the study included 23 subjects without biochemical or clinical evidence of chronic liver disease who were abstinent for 2 weeks; 11 patients were started on disulfiram (250 mg/day), whereas the other 12 were not given disulfiram and served as controls. The second part of the study included 13 alcoholic patients with clinical or pathologi

Naltrexone: new preparation. Transient preventive efficacy on alcoholic relapse.

(1) Naltrexone is an opiate receptor antagonist licensed for the maintenance of alcohol abstinence after withdrawal. (2) The clinical file in this indication is limited to three placebo-controlled trials lasting three months. (3) The results show that naltrexone prevents the resumption of alcohol consumption during treatment, but a post-trial follow-up study showed that the preventive effect disappeared a few months after treatment cessation. (4) Naltrexone has not been compared with acamprosate, a better-assessed drug used in the same indication. (5) The safety profile of naltrexone is accept

Utility of carbohydrate-deficient transferrin as a marker of relapse in alcoholic patients.

Carbohydrate-deficient transferrin (CDT) has been proposed as a marker of alcoholism. However, its role in monitoring alcoholic patients for relapse has not been extensively studied. We therefore performed sequential serum CDT measurements using a microcolumn/radioimmunoassay method (Kabi Pharmacia, Piscataway, NJ) in 86 male alcoholics participating in a hepatitis vaccination program who were monitored for relapse using self-report and collateral history (when available). The maximum serum CDT was significantly higher in patients who relapsed (n = 38) (33.1 +/- 3.1 mg/liter), as compared with

Chronic alcoholic gastritis. Roles of alcohol and Helicobacter pylori.

We assessed the relative roles of alcohol and infection with Helicobacter pylori in the pathogenesis of chronic gastritis in alcoholic patients. Helicobacter pylori was found in 14 of 18 alcoholics with dyspepsia and was associated with chronic antral gastritis. Gastric biopsy specimens were normal in four H pylori-negative alcoholics. Studies were repeated 3 to 4 weeks after controlled abstinence. There was no change in histologic findings during this period, indicating that alcohol itself was not the major causative agent. We then eliminated H pylori in 10 subjects by giving triple therapy (

Digest: Rapid evolution of an annual invader, the African fig fly Zaprionus indianus.

Little is known about evolutionary dynamics shortly after species introduction. Gray et al. (2025) found that the African fig fly (Zaprionus indianus) evolved smaller wings and a decreased wing:thorax ratio within one season of colonizing a temperate region of the United States, but did not differ along a latitudinal cline. This demonstrates the species' capability for rapid adaptation and potential for enhanced invasiveness.

Yellow fever and dengue: a threat to Europe?

The introduction and rapidly expanding range of Aedes albopictus in Europe is an iconic example of the growing risk of the globalization of vectors and vector-borne diseases. The history of yellow fever and dengue in temperate regions confirms that transmission of both diseases could recur, particularly if Ae. aegypti, a more effective vector, were to be re-introduced. The article is a broad overview of the natural history and epidemiology of both diseases in the context of these risks.

[The ecology and prevalence of arboviruses on the territory of the Saratov Region].

The territorial spread of Tahyna, Batai, Sindbis, West Nile fever and Crimean-Congo hemorrhagic fever viruses throughout the Saratov region in 1998 - 2000 was analyzed. The characteristics of the epizootic activity of the natural foci of these arboviruses in different landscape zones (temperate forest-steppes, steppes and semi-deserts) were calculated. The species composition of small mammals, the natural reservoirs of the causative agents of arbovirus infections, was determined.

Rethinking the prognostic significance of bile spillage in gallbladder cancer.

The largest multi-institutional cohort analysis of bile spillage in incidental gallbladder cancer was presented by van Dooren et al The study offers important insights, though certain methodological limitations and interpretative challenges temper the strength of its conclusions. We address these, clarify how statistical findings intersect with clinical relevance for bile spillage, propose a refined classification system, and provide global epidemiological context.

Primary Pyomyositis in Children is No More a Rare Condition: Presentation of 2 Clinical Cases.

Primary pyomyositis is a bacterial muscle infection which may lead to abscess formation and severe complications. Although this condition has long been considered "tropical" and rare, mostly affecting immunocompromised patients, cases of pyomyositis have recently raised significantly among healthy children in temperate climates. With these 2 cases we highlight the importance of an early recognition of this condition, allowing an immediate treatment and reducing complications.

Petechiae and purpura: the ominous and the not-so-obvious?

Petechiae and purpura are among the most alarming findings a pediatrician will commonly observe in the office. Severity of illness can range from a temper tantrum, to common viral infections, to the most deadly infections and diseases. To avoid many of the pitfalls in diagnosis, practitioners will need to be thorough in history taking, assessing fever and immunization status, and physical examination. In addition, a few simple laboratory tests will usually be needed and possibly a manual differential.

Two uncommon manifestations of leptospirosis: Sweet's syndrome and central nervous system vasculitis.

To leptospirosis is the commonest spirocheatal infection in the tropical and temperate countries of Indian sub-continent and Africa and the most common zoonosis worldwide. The protean manifestation of this infectious disease is a challenge for practising clinicians across the world. In poor developing countries, at most clinical suspicion it is essential in the diagnosis of this disease. In this report, we are able to document two uncommon manifestations of leptospirosis, namely Sweet's syndrome and central nervous system vasculitis.

Update on sporotrichosis.

Sporotrichosis is endemic in temperate and tropical regions of the world, including Australia. Although uncommon, it is frequently encountered in Northern Australia, especially in Queensland. All ages are affected, although most cases occur from occupational exposure to vegetation or soil. This article reviews the cause, diagnosis and treatment options of this fungal disease which affects the skin and is often misdiagnosed. It classically presents in gardeners, especially those handling roses.

Heat-exercise hyperpyrexia.

A fatal case of "heat stroke" occurring on exercise in temperate conditions is described. Possible mechanisms for increased susceptibility are discussed and attention is drawn to a possible overlap with the malignant hyperpyrexia (MH) syndrome. The second day of strenuous exercise may be a time of special risk. More frequent consideration of dantrolene treatment in heat injury is suggested and the term "heat-exercise hyperpyrexia" is proposed to replace "heat stroke" and "exertional hyperpyrexia".

Australia's current wildfire crisis linked to colonial land-use change.

The global wildfire crisis1 is often attributed to climate change, a framing that overlooks crucial historical drivers. Biomes at the forefront of this crisis, such as the temperate eucalypt forests of Australia, have seen alarming increases in large, intense wildfires in recent decades. Yet evidence of how colonial land-use change and the disruption of Indigenous fire use altered vegetation structure and fuel dynamics remains scarce. Here we reconstruct 1,000 years of vegetation and fire change in a temperate eucalypt forest in southeastern Australia. Before the British invasion (from 1788 CE

Rising herbivory pressure of Sarpa salpa and the emerging threat to Southern European marine forests.

Climate change (CC) is driving shifts in marine ecosystems, particularly affecting temperate and subtropical marine forests, which are critical for biodiversity and ecosystem stability. Dominated by canopy-forming seaweeds, these forests are sensitive to CC-induced stressors, such as rising temperatures and tropicalisation, which favour turf-forming algae and increase the abundance of herbivorous fish, threatening ecosystem structure and function. Along the western coast of Portugal in Southern Europe, the herbivorous fish Sarpa salpa plays a key role in tropicalisation, potentially exacerbati

Persistence, Seasonal Dynamics, and Regional Differentiation of the Parasitic Mite Tropilaelaps mercedesae in Apis mellifera L. Colonies Under Non-Endemic Climatic Conditions.

Tropilaelaps mercedesae is an emerging parasite of Apis mellifera L. that is expanding beyond its traditional tropical range, yet its ecology in temperate regions remains poorly understood. This study investigated the establishment, seasonal infestation dynamics, morphometric variation, and virus carriage of T. mercedesae populations in three climatically contrasting regions of temperate Eurasia: Sochi and Makhachkala (Russia) and Zhetisay (Kazakhstan). Honey bee colonies were monitored throughout the annual cycle to assess infestation in relation to brood availability and seasonal climatic co

Detection of cryptic Taenia species from domestic and sylvatic transmission cycles in southern pudu deer (Pudu puda), Chile.

Anthropogenic landscape fragmentation promotes interactions between domestic animals and wildlife, facilitating cross-species parasite transmission. Through molecular identification, we report cryptic infection by three Taenia species (T. hydatigena, T. krabbei, and T. omissa) in the Southern pudu (Pudu puda), a threatened cervid endemic to temperate rainforests of South America. From necropsies of twelve pudus from rural areas of Southern Chile, we recovered 44 metacestode cysts from diverse organs. Molecular and phylogenetic analyses of the cytochrome c oxidase subunit I (COX-1) gene confirm

Improving influenza forecast in the tropics and subtropics: a case study of Hong Kong.

Influenza forecasts could aid public health response as shown for temperate regions, but such efforts are more challenging in the tropics and subtropics due to more irregular influenza activities. Here, we built six forecast approaches for influenza in the (sub)tropics, with six model forms designed to model seasonal infection risk (i.e. seasonality) based on the dependence of virus survival on climate conditions and to flexibly account for immunity waning. We ran the models jointly with the ensemble adjustment Kalman filter to generate retrospective forecasts of influenza incidence in subtrop

Characterization of the gut phageome of Japanese patients with ulcerative colitis under endoscopic remission.

This study aimed to analyze the gut phageome in Japanese patients with ulcerative colitis (UC) in endoscopic remission. Fecal samples were collected from 35 UC patients and 22 healthy controls. The gut microbiome was analyzed using 16S rRNA amplicon sequencing, and the phageome was profiled through shotgun metagenomic sequencing. Compared to healthy controls, UC patients showed a significant reduction in phageome richness (observed species and Chao1 index). Principal coordinate analysis revealed a significant difference in beta-diversity between UC and healthy controls (p = 0.001). The abundan

The impact of current and future climates on spatiotemporal dynamics of influenza in a tropical setting.

Although the drivers of influenza have been well studied in high-income settings in temperate regions, many open questions remain about the burden, seasonality, and drivers of influenza dynamics in the tropics. In temperate climates, the inverse relationship between specific humidity and transmission can explain much of the observed temporal and spatial patterns of influenza outbreaks. Yet, this relationship fails to explain seasonality, or lack there-of, in tropical and subtropical countries. Here, we analyzed eight years of influenza surveillance data from 12 locations in Bangladesh to quant
