A preserved neural code for temporal order between memory formation and recall in the human medial temporal lobe.

Temporal memory enables us to remember the temporal order of events happening in our life. The human medial temporal lobe (MTL) appears to contain neural representations supporting temporal memory formation, but the cellular mechanisms that preserve temporal order information for recall are largely unknown. Here, we examined whether human MTL neuronal activity represents the temporal position of events during memory formation and recall, using invasive single and multi-unit recordings in human epilepsy patients (n = 19). Participants freely navigated a virtual environment in order to explore a…

Does the temporal cortex make us human? A review of structural and functional diversity of the primate temporal lobe.

Temporal cortex is a primate specialization that shows considerable variation in size, morphology, and connectivity across species. Human temporal cortex is involved in many behaviors that are considered especially well developed in humans, including semantic processing, language, and theory of mind. Here, we ask whether the involvement of temporal cortex in these behaviors can be explained in the context of the 'general' primate organization of the temporal lobe or whether the human temporal lobe contains unique specializations indicative of a 'step change' in the lineage leading to modern hu…

The 'when' and 'where' of semantic coding in the anterior temporal lobe: Temporal representational similarity analysis of electrocorticogram data.

Electrocorticograms (ECoG) provide a unique opportunity to monitor neural activity directly at the cortical surface. Ten patients with subdural electrodes covering ventral and lateral anterior temporal regions (ATL) performed a picture naming task. Temporal representational similarity analysis (RSA) was used, for the first time, to compare spatio-temporal neural patterns from the ATL surface with pre-defined theoretical models. The results indicate that the neural activity in the ventral subregion of the ATL codes semantic representations from 250 msec after picture onset. The observed activat

Facial asymmetry in patients with temporal lobe epilepsy. A clinical sign useful in the lateralization of temporal epileptogenic foci.

While a clinical history is frequently sufficient for a localizing diagnosis of temporal lobe epilepsy, lateralization of the epileptogenic abnormality often is impossible on clinical grounds alone. Since we have noted facial asymmetry in such patients, 50 individuals with temporal lobe epilepsy were studied. In patients with unilateral foci, we found contralateral lower facial weakness of mild to severe degree in 73 percent while 13 percent had ipsilateral weakness, and in 13 percent the face was symmetrical. The facial weakness usually was more striking on emotional movement. In the group of…

The cognitive profile of occipital lobe epilepsy and the selective association of left temporal lobe hypometabolism with verbal memory impairment.

We investigated the cognitive profile of structural occipital lobe epilepsy (OLE) and whether verbal memory impairment is selectively associated with left temporal lobe hypometabolism on [18F]-fluorodeoxyglucose positron emission tomography (FDG-PET). Nine patients with OLE, ages 8-29 years, completed presurgical neuropsychological assessment. Composite measures were calculated for intelligence quotient (IQ), speed, attention, verbal memory, nonverbal memory, and executive functioning. In addition, the Wisconsin Card Sorting Test (WCST) was used as a specific measure of frontal lobe functionin…

Inter-ictal and post-ictal psychoses in frontal lobe epilepsy: a retrospective comparison with psychoses in temporal lobe epilepsy.

There have been few studies of the psychopathology of patients with frontal lobe epilepsy (FLE). The majority of studies of both inter-ictal and post-ictal psychoses have strongly suggested the influence of temporal lobe disturbance on psychoses. Patients with organic brain damage or schizophrenia, however, sometimes show frontal lobe dysfunction. The purpose of this study was to better understand the effect, if any, of frontal lobe disturbance and seizure on psychopathology. Patients were divided into four groups based on epilepsy type and preceding seizures; 8 with FLE/inter-ictal psychosis,…

The timing of associative memory formation: frontal lobe and anterior medial temporal lobe activity at associative binding predicts memory.

The process of associating items encountered over time and across variable time delays is fundamental for creating memories in daily life, such as for stories and episodes. Forming associative memory for temporally discontiguous items involves medial temporal lobe structures and additional neocortical processing regions, including prefrontal cortex, parietal lobe, and lateral occipital regions. However, most prior memory studies, using concurrently presented stimuli, have failed to examine the temporal aspect of successful associative memory formation to identify when activity in these brain r…

Neuropsychological outcomes of hippocampus-sparing anterior temporal lobectomy versus stereotactic laser amygdalohippocampectomy in language-dominant temporal lobe epilepsy.

OBJECTIVE: Postoperative neuropsychological deficits pose a significant challenge for temporal lobe epilepsy (TLE) surgery, particularly in the language-dominant hemisphere. Two surgical approaches have been suggested to mitigate such adverse outcomes: stereotactic laser amygdalohippocampectomy (SLAH) and hippocampus-sparing anterior temporal lobectomy (HSATL). This retrospective cohort study compares the seizure control and neuropsychological outcomes of HSATL and SLAH in language-dominant TLE. METHODS: A retrospective cohort study compared 27 patients (nine male, mean age = 35.8 years) with…

Preserved ictal responsiveness in right mesial temporal lobe epilepsy: metabolic correlates in posterior temporal networks with FDG-PET.

OBJECTIVE: Automatisms with preserved responsiveness (APR) represent a distinctive clinical feature in right mesial temporal lobe epilepsy (MTLE). This study aimed to investigate whether interictal FDG-PET hypometabolism correlates with impaired responsiveness during seizures. METHODS: We retrospectively analyzed 49 patients with right MTLE who underwent presurgical evaluation. Patients were stratified into APR+ (n = 16) and APR- (n = 33) groups based on a standardized four-domain assessment (orientation, memory, verbal command, motor execution). Interictal FDG-PET hypometabolism was visually…

Technical challenges to anterior temporal lobectomy after laser interstitial thermal therapy for mesial temporal lobe epilepsy: technical note.

Mesial temporal sclerosis (MTS) is a frequent cause of medically refractory epilepsy, for which laser interstitial thermal therapy (LITT) is an effective treatment. However, experience with the technical considerations posed by additional surgery after an initial LITT procedure is lacking. The authors present the case of a 12-year-old female with medically refractory temporal lobe epilepsy and left MTS who underwent LITT at a separate institution prior to referral. This patient had no change in early postoperative seizure control (Engel class IVB) and then her seizures worsened despite ongoing…

More than just statics: Static and temporal dynamic changes in intrinsic brain activity in unilateral temporal lobe epilepsy.

BACKGROUND: Temporal lobe epilepsy (TLE) is the most prevalent refractory focal epilepsy and is more likely accompanied by cognitive impairment. The fully understanding of the neuronal activity underlying TLE is of great significance. OBJECTIVE: This study aimed to comprehensively explore the potential brain activity abnormalities affected by TLE and detect whether the changes were associated with cognition. METHODS: Six static intrinsic brain activity (IBA) indicators [amplitude of low-frequency fluctuation (ALFF), fractional ALFF (fALFF), regional homogeneity (ReHo), degree centrality (DC)…

Visual field defects after radiosurgery versus temporal lobectomy for mesial temporal lobe epilepsy: Findings of the ROSE trial.

PURPOSE: Stereotactic radiosurgery (SRS) may be an alternative to anterior temporal lobectomy (ATL) for mesial temporal lobe epilepsy (MTLE). Visual field defects (VFD) occur in 9-100% of patients following open surgery for MTLE. Postoperative VFD after minimally invasive versus open surgery may differ. METHODS: This prospective trial randomized patients with unilateral hippocampal sclerosis and concordant video-EEG findings to SRS versus ATL. Humphries perimetry was obtained at 24 m after surgery. VFD ratios (VFDR = proportion of missing homonymous hemifield with 0 = no VFD, 0.5 = complete s…

Preoperative systemic immune-inflammation index predicts seizure outcomes after anterior temporal lobectomy in drug-resistant temporal lobe epilepsy.

BACKGROUND: Anterior temporal lobectomy remains the most effective treatment for drug-resistant temporal lobe epilepsy, yet 20-40 % of patients experience persistent seizures postoperatively. Neuroinflammation plays a critical role in epileptogenesis, but the prognostic value of preoperative peripheral inflammatory markers has not been systematically evaluated. The systemic immune-inflammation index (SII), a composite marker integrating neutrophil, lymphocyte, and platelet counts, has demonstrated prognostic utility in cardiovascular disease and stroke but remains unexplored in surgical epilep…

Baseline neuropsychological characteristics in patients with epilepsy with left temporal lobe encephaloceles compared with left mesial temporal sclerosis.

BACKGROUND: Temporal lobe encephaloceles (TE) are increasingly recognized as a cause of drug-resistant temporal lobe epilepsy. Improved recognition of these lesions offers an opportunity to treat them with a limited resection sparing the hippocampus. However, as they can be difficult to identify on imaging, additional clues pointing to the diagnosis can be helpful. We sought to understand the baseline cognitive/neuropsychological profile in patients with left temporal lobe epilepsy caused by encephaloceles compared with that caused by mesial temporal sclerosis (MTS), a common entity in the dif…

Symptom-related damage patterns in mesial temporal lobe epilepsy with hippocampal sclerosis.

OBJECTIVE: This study was undertaken to elucidate the neuroanatomical and network substrates underlying diverse seizure symptoms in mesial temporal lobe epilepsy with hippocampal sclerosis (mTLE-HS) by applying multivariate support vector regression (SVR) to individualized structural symptom mapping and atrophy network mapping (ANM). METHODS: We retrospectively enrolled 216 patients with mTLE-HS and 460 healthy controls. Six non-mutually exclusive seizure symptoms were assessed. We performed left-right flipping of all images of right-sided mTLE-HS patients along the midline before processing …

The usefulness of intraoperative electrocorticography (iECoG) in pediatric temporal lobe epilepsy surgery.

OBJECTIVE: This study aimed to evaluate the usefulness of intraoperative electrocorticography (iECoG) in providing a more accurate surgical strategy, thereby yielding seizure freedom following resective surgery in children with temporal lobe epilepsy (TLE). METHODS: The authors conducted a retrospective review of pediatric patients with drug-resistant TLE due to various etiologies, within a relatively long follow-up (range 8.5-11.5 years). Patients were divided into two groups based on whether they were operated on using iECoG or not, which was employed in cases of uncertain delineation of th…

Ictal Bruxism - A Sign of Temporal Lobe Ictal Localization: A Case Series.

Ictal bruxism (IB) is a rare oro-alimentary automatism in focal seizures. We report a series of six patients with IB in drug-resistant temporal lobe epilepsy (TLE) treated subsequently with temporal lobectomy. All patients had ictal teeth grinding with semiological features, suggestive of temporal lobe origin. Ictal electroencephalogram (EEG) onset showed artifacts corresponding to bruxism; one patient had a "checkerboard" pattern of EEG artifacts corresponding with lateral jaw movements. Five patients had a lesion in the mesial temporal lobe, while one patient had a neocortical temporal lobe …

White matter resection and verbal memory deficits after temporal lobe epilepsy surgery.

Temporal lobe resection for focal, drug-resistant temporal lobe epilepsy (TLE) causes verbal memory deficits in 30% of left hemisphere-operated patients. Structural, functional and computational modelling have shown a widespread structural and functional memory network with hubs in critical brain regions including the hippocampus, subcortical and neocortical regions. We hypothesized that damage to white matter pathways forming a network involving cortical and subcortical regions may be responsible for postoperative memory problems. In this study, we measured verbal memory encoding (immediate r…

Thalamic glutamatergic neurons regulate seizure onset and generalization in temporal lobe epilepsy.

Temporal lobe epilepsy (TLE) is a common form of epilepsy in adults. The anterior nucleus of the thalamus (ANT) is a potential therapeutic target due to its role in epileptic networks. Using calcium imaging, we show that glutamatergic neurons in the anterodorsal thalamic nucleus (AD), a subarea of the ANT, are activated during hippocampal seizures. Chemogenetic inhibition of AD glutamatergic neurons suppresses seizure onset and generalization in TLE models, whereas activation promotes seizures. Trans-monosynaptic tracing identifies the postsubiculum (PoSub) as an important downstream target of…

Dissecting medial temporal lobe from diencephalic sub-volumes: The amnesia dichotomy revisited.

Severe episodic memory deficits have historically been categorized as diencephalic amnesia, like in Korsakoff's Syndrome (KS), or as medial temporal lobe (MTL) amnesia, like in Alzheimer's Disease (AD). However, recent research has highlighted that both MTL and thalamus contribute to episodic memory. We aimed to challenge the traditional distinction by assessing whether subregional volume loss in the MTL and diencephalon reflects the distinct amnesia profiles traditionally associated with KS and AD. This cross-sectional observational study includes 203 subjects, comprising 81 healthy control p…
