In Vitro Comparison between the Pulvertaft Weave and the Modified Core Suture Pulvertaft Weave.

Background: The Pulvertaft weave was described more than 50 years ago and is still used in tendon transfers. The aim of this study was to evaluate the strength of a modified core suture Pulvertaft weave technique and compare it to the original Pulvertaft weave traditionally used in tendon transfer surgery. Methods: 12 extensor pollicis longus tendons and extensor indices proprius tendons were harvested from fresh frozen cadavers. Six Pulvertaft weaves were performed using FiberWire 4.0 and six core suture tendon weave were performed using FiberLoop 4.0. Biomechanical analysis was performed and…

Traumatic index extensor tendon attenuation mimicking closed tendon rupture: two case reports.

BACKGROUND: While some traumatic closed index extensor tendon ruptures at the musclotendinous junction have been previously reported, closed … ss severe or, in severely attenuated tendons, with tendon grafting (ipsilateral palmaris longus) or tendon transfer. Six months after the operation, the active extension of the index metacarpophalangeal joints had recovered well. CONCLUSIONS: Two cases of traumatic index extensor tendon attenuation were treated successfully by shortening the attenuated tendon in …

Arthroscopic Latissimus Dorsi Transfer in the Lateral Decubitus Position for Irreparable Posterosuperior Massive Rotator Cuff Tears.

The management of irreparable posterosuperior massive rotator cuff tears remains challenging. With advances in arthroscopic techniques, minimally invasive arthroscopic latissimus dorsi tendon (LDT) transfer has been increasingly used for irreparable posterosuperior massive rotator cuff tears. The present article describes arthroscopic LDT transfer in the lateral decubitus position for irreparable posterosuperior massive rotator cuff tears. The dissection and release of the LDT were performed through the interval between long head of the biceps tendon and humeral shaft. When releasing the LDT,

Fracture of calcaneum following drop foot surgery--a case report.

The usual protocol for correction of drop foot in leprosy, a consequence of damage to the common peroneal nerve, is a tendon transfer, immobilisation to heal the tendon juncture and post-operative exercises to put the transfer into use. Tarsal disintegrations have been reported in literature in drop foot patients when the transferred tendons were inserted into the bone making a drill hole to ensure firm anchorage. Such disintegrations are rarely seen these days because bony insertion of the tendon transfers is not performed in the leprosy-affected foot. We report here a case of drop foot that …

Repair of acute extensor hallucis longus tendon injuries: a retrospective review.

BACKGROUND: Extensor hallucis longus (EHL) tendon injuries may occur with lacerations sustained over the dorsum of the foot and lead to hallux dysfunction. Primary repair is performed when tendon edges are opposable; however, if a gap exists between tendon edges, then reconstruction with tendon graft or tendon transfer may be necessary to restore hallux alignment and dorsiflexion. We describe the surgical technique and report the results on a large series of patients having undergone primary repair or reconstruction of EHL tendon lacerations. METHODS: We retrospectively reviewed all patients …

Case report: modified latissimus dorsi muscle transfer for radial palsy: presentation of a new technique.

OBJECTIVE: The latissimus dorsi muscle transfer finger and wrist extension is a well-known procedure for patients needing replantation or in brachial plexus injuries. To increase the length of the transferred muscle to reach to the finger and wrist extensors, the other authors suggest extending the muscle length by incorporating the iliac crest fascia, which not only prolongs the operation time but also minimizes the chances of a healthy and viable muscle-tendon junction. We present a modification of the standard latissimus dorsi transfer whereby the whole muscle (not partial harvest) is trans

Failed Reconstruction of the Extensor Pollicis Longus in a Patient with a Major Variation of the Extensor Indicis Proprius Tendon: A Case Report.

Tendon transfer with extensor indicis proprius (EIP) has been performed widely for reconstructing ruptures of the extensor pollicis longus because of its simplicity and clinical outcome. We experienced a rerupture of the extensor pollicis longus restored by incorrect tendon transfer because of a major unrecognized variation in the anatomy of the EIP. Surgeons should perform such transfer with a detailed knowledge of the possible anatomical variations to avoid such serious complications.

Finger flexion force and hand grip strength after tendon repair.

PURPOSE: To define the order and frequency of elongation in tendon junctions in extensor carpi radialis longus (ECRL) to flexor digitorum profundus tendon transfer and its correlation to grip strength and lack of finger flexion. METHODS: Forty-seven tetraplegic patients had surgery involving the reconstruction of finger flexion with the transfer of the ECRL in a total of 62 arms. During surgery metal markers were placed on both sides of the tendon-to-tendon attachment site. After surgery the distance between the markers was measured on radiographs. Any increase in the distance between the mar…

Management of digital tendon avulsion at the musculotendinous junction of the forearm: a systematic review.

BACKGROUND: Tendon avulsion at the musculotendinous junction caused by digit avulsion amputation or closed injury is a challenging problem, … males involved in work accidents. Treatment options were tendon resection, reattachment to muscle, tendon transfer, and side-to-side repair. When the digit was replanted, tendons avulsed through avulsion amputations were preferentially treated by reattachment in the case of the thumb, transfers for the index and long fingers, and resection for the ring and small …

An Extensor Indicis Proprius Transfer for Chronic Extensor Pollicis Longus Injury After Nearly 2 Decades: A Case Report.

CASE: A 36-year-old soldier presented for foreign training fitness with a history of a fall 18 years previously, that injured his nondominant left wrist. Initially, the injury was managed conservatively with a below elbow cast. Since then, he had been unable to extend the thumb at the interphalangeal joint but adapted well to daily activities. Surgical reconstruction using an extensor indicis proprius (EIP) to extensor pollicis longus (EPL) tendon transfer yielded favorable recovery. CONCLUSION: This case highlights neglected EPL injury over 18 years and confirms that EIP transfer, when preci…

Reconstruction of lateral pinch in an isolated paralysis of the first dorsal interosseous muscle - A new surgical technique.

Lack of function of the first interosseous muscle (FDI) might be responsible for insufficient stabilisation of the index finger during lateral pinch, and may induce disability in hand function. The first cause of FDI palsy is ulnar nerve palsy. We describe a new tendon transfer to reanimate the FDI muscle, using the extensor indicis proprius tendon. The tendon is sectioned at its distal insertion and rerouted in the first extensor tendon compartment. We report one case of isolated first interosseous muscle palsy secondary to direct trauma. Preoperatively, the patient complained of a severe lac…

Efficacy of Abductor Pollicis Longus Suspensionplasty Compared to Ligament Reconstruction and Tendon Interposition.

Background: Multiple surgical techniques are described for basal joint osteoarthritis. This study compares clinical outcomes and patient satisfaction with trapeziectomy and abductor pollicis longus (APL) suspensionplasty compared to trapeziectomy with ligament reconstruction and tendon interposition (LRTI) when performed by 2 fellowship-trained hand surgeons. Methods: A retrospective review of 51 consecutive patients undergoing APL suspensionplasty (53 hands) was performed. With this technique, a distally based APL slip is brought through and sewn to the flexor carpi radialis (FCR). The remain

Peroneus longus transfer for drop foot in Hansen disease.

Leprosy or Hansen's disease is a chronic infectious disease caused by the Mycobacterium leprae. Nerve injury is a central feature of the pat … the superficial peroneal branch. The article discusses the advantages of using the peroneus longus tendon transfer to the dorsum of the foot instead of the posterior tibial tendon for the correction of dropfoot in selected cases where the peroneals tendons are intact.

Surgical technique for combined Dwyer calcaneal osteotomy and peroneal tendon repair for correction of peroneal tendon pathology associated with cavus foot deformity.

Peroneal tendon pathology is commonly seen in patients with underlying pes cavus. The Dwyer calcaneal osteotomy is a useful adjunctive procedure to address the heel varus component of the cavus foot deformity, especially in the presence of concomitant peroneal tendon pathology. The lateralizing heel osteotomy using a wedge resection can effectively reduce future stress on the repaired peroneal tendons, although technical challenges arise when attempting to perform both tendon repair and heel osteotomy through the same incision. Specific principles must be followed to achieve adequate exposure

Surgical treatment of peroneal tendon tears.

The focus of this article is the diagnosis and treatment of peroneal tendon tears. The article first describes mechanisms of injuries resulting in peroneal brevis and longus tears. Associated pathologies, such as ankle instability, hindfoot varus, hypertrophied peroneal tubercle, are discussed. Following sections on diagnosis and conservative treatment, the article describes operative treatment for isolated peroneus brevis tear, isolated peroneus longus tear, and tears of both the peroneus longus and brevis. The authors also discuss hamstring allograft reconstruction, the silicone rod techniqu

Endoscopic single-incision extensor indicis transposition for extensor pollicis longus reconstruction.

Traditional methods for harvesting the extensor indicis (EI) tendon for transposition to the extensor pollicis longus (EPL) tendon using direct visualization commonly require 3 incisions. The authors describe an endoscopic EI tendon transposition for EPL tendon reconstruction as an alternative technique for the traditional open technique. This new modification of endoscopic EI transposition to EPL has the benefit over the standard harvesting technique of only 1 incision to accomplish the complete procedure and results in less scarring. To date, the authors have only operated on ruptures after

Congenital anomaly of the thumb: aplasia of the flexor pollicis longus.

A five-year-old boy, with congenital aplasia of the flexor pollicis longus, without other associated anomalies is reported. A two-stage procedure was done using a silicone rubber rod and pulley reconstruction, followed by transfer of the flexor digitorum superficialis tendon from the ring finger to the distal phalanx of the thumb. This resulted in 0/55 degrees active flexion at the interphalangeal joint.

Congenital absence of flexor pollicis longus without hypoplasia of thenar muscles.

A 7-year-old girl who was unable to flex her left thumb at the interphalangeal joint proved to have an extremely hypoplastic flexor pollicis longus with normal thenar muscles, which is very rare. The flexor digitorum superficialis of the ring finger was transferred to the inserting portion of the flexor pollicis longus tendon with good results. The patient's cooperation seems to be a factor determining the prognosis.
