[Bilateral rupture of the quadriceps tendon].

Simultaneous, bilateral, and spontaneous rupture of the quadriceps tendon is a very rare injury. Individuals with this injury are usually predisposed by chronic renal disease with secondary hyperparathyroidism, gout, diabetes, and lupus erythematosus. Often, primary diagnostic confusion can lead to a delay in treatment. Two cases of a bilateral lesion of the quadriceps tendon, that were treated surgically using the technique of Scuderi, followed by an intense rehabilitation programme, are presented. Surgical treatment yields satisfactory results.

[Rehabilitation of extensor tendon injuries].

The extensor mechanism of the hand consists of different tendons, originating from extrinsic and intrinsic muscles of the hand and forearm. Because of their course near to skin and bone separated only by a thin layer of paratenon, they are very sensitive to injuries and adhesions. Treatment protocols have to be adapted according to the different zones. This paper explains the basic principles of rehabilitation in extensor tendon injuries.

[Rupture of the gluteus medius tendon].

AIM: Rupture of the gluteus medius tendon is a rare, even neglected condition. Typical findings include local tenderness and weakness of hip … . Furthermore, a valgus position of the proximal femur was detected. Transosseous refixation of the tendons was performed in all cases. 3 months after operation local tenderness near the greater trochanter disappeared and full strength of the pelvitrochanteric muscles was achieved in 11 out of 13 cases and in the remaining cases after one year. CONCLUSIO …

Splinting programmes for tendon injuries.

It is beyond doubt that splinting programmes have often been an integral and important part of the rehabilitation process in tendon injuries. Over the past three decades, hand splints for tendon injuries of various designs and different mobilisation programmes have been developed in the hope of pursuing better clinical and functional outcome for patients. In this paper, the development of different splinting programmes in flexor and extensor tendon injuries and the current practice in some acute hospitals in Hong Kong were discussed.

Anterior tibial tendon injuries.

Daily activities, such as walking or running, put a certain degree of stress and demand on the tendons of the lower extremities. To date, there are very few reported cases in the literature concerning pathology of the anterior tibial tendon or muscle. Of those cases reported and discussed, the majority were complete ruptures as a result of an acute forced injury, with very few reports of secondary pathological conditions or laceration injuries. This article is a review of the literature, as well as conservative versus surgical treatment and the sequelae following such injuries.

Arthroscopic treatment of infrapatellar tendonitis.

Infrapatellar tendonitis is a chronic overload lesion in the patellar ligament at the attachment to the lower pole of the patella. This lesion is found primarily in athletes who participate in jumping sports. Magnetic resonance imaging or ultrasound can show the extent of tendon pathology. Patellar tendonitis is treated with modification of activities, medications, and therapy. When conservative measures fail, operative debridement has been recommended. Previous reports have described a technique of open debridement of the patellar tendon, followed by an extended period of rehabilitation befor

[Physical diagnostics--tendon reflexes].

Clinical examination of the tendon reflexes is helpful in localising lesions in the nervous system, which is an important step in the diagnosis of neurological disorders. The large intra- and interindividual variations of reflex amplitudes make interpretation difficult. Published interobserver agreement studies provide conflicting results. The diagnostic specificity of unequivocally abnormal reflexes is moderate to good (70-95%, decreasing with increasing age), but the sensitivity is lower (50-70%), both for peripheral and for central lesions. This implies that the finding of a clearly abnorma

Biceps tendon disorders in athletes.

It has been proposed that the long head of the biceps functions as a humeral head depressor and stabilizer. In addition, in many overhead sports, the biceps helps to accelerate and decelerate the arm. With improper training or fatigue, inordinate stresses can be placed on the biceps as it attempts to compensate for other muscles. This can lead to attrition and failure, either within the tendon substance or at its origin. Bicipital problems in athletes usually occur in conjunction with other types of shoulder disorders, such as rotator cuff impingement and glenohumeral instability, making deter

Gene transfer to the patellar tendon.

Growth factors have the potential to enhance native repair responses in ligamentous lesions. However, methods for applying these cytokines to sites of injury for extended periods are lacking. We suggest that local transfer of genes which encode the relevant healing factors merits investigation as a potential solution to this problem. In the present study, the retroviral vectors MFG lacZ and BAG lacZ neo(r) and adenovirus LacZ were evaluated for their ability to deliver genes to cells of ligamentous origin. The posterior and anterior cruciate ligaments, medial collateral ligament, semitendinosu

[Close injury of the tendon at wrist].

Because of the complicated causes and variable clinical signs, closed injury of tendons at wrist is difficult to diagnosis and treat. Twenty-six cases of tendon ruptur were reported. Among them, 11 cases were caused by bone fracture or dislocation, 8 cases were caused by rheumatoid synovitis, 5 cases were caused by synovial tuberculosis, and 2 cases caused by other. The pathogenesis and clinical signs were analyzed. Twenty-three cases were treated by tendon transfer and 3 cases were treated by tendon transplantation. By average follow-up of 16 months (ranged 6 months to 4 years), the results w…

Complete quadriceps tendon ruptures.

Forty-four patients with 53 quadriceps tendon ruptures were studied retrospectively with an average follow up of 67.2 months (range: 24 to 155). The type of repair was not associated with differences in functional outcome, patient satisfaction, range of motion, or isokinetic testing. Patients with a delay in surgical treatment in comparison to those immediately repaired had significantly worse functional results (P < .05), lower satisfaction scores (P < .05), and lower isokinetic data for both the injured and uninjured extremities, however, their range of motion and comparative extensor power

Isolated rupture of the subscapularis tendon.

Sixteen consecutive patients were managed operatively for repair of an isolated traumatic rupture of the subscapularis tendon in the absence of avulsion of the lesser tuberosity. All of the patients were men. The diagnosis was made for each patient on the basis of the clinical examination and was confirmed by imaging studies and operative exploration. The operative treatment consisted of mobilization of the subscapularis after exploration and protection of the axillary nerve, transosseous reinsertion of the tendon to a trough created at the lesser tuberosity, closure of the rotator interval, a

Bipolar infrapatellar tendon rupture.

Traumatic patella alta in children occurs either distal to the patellar tendon as a tibial tubercle apophyseal fracture or proximally as an osteochondral sleeve fracture of the inferior patellar pole. Acute surgical exploration in a pediatric case of a knee extensor mechanism rupture revealed both proximal and distal (bipolar) patellar tendon pathology.

Isolated avulsion of the popliteus tendon.

We report four patients with a mean age of 17 years (14 to 22) with external rotation injuries of the knee in slight flexion. Radiographs showed a small fragment in the area of the lateral femoral condyle. At operation, the fragment, consisting of the femoral insertion of the popliteus, was anatomically reduced and fixed. At a mean follow-up of 35 months all the knees had an excellent function score. An isolated lesion of the popliteus often presents as a tendon avulsion whereas major damage to the posterolateral corner of the knee involves combined ligamentous injuries. In patients with an ac
